1.A Case of Acquired Ichthyosis Developed During Cholesterol-lowering Treatment.

A 42-year-old woman presented with ichthyosiform eruptions on her trunk and buttock which developed 20 days prior to consulatation. She had taken the cholesterol-lowering drug(lovastatin, a 3-hydroxy-3-methylglutaryl-coenzyme A (HMG CoA) reductase inhibitor) for 4 months. After stopping the drug, the skin lesions gradually improved and after two months most of the lesions had disappeared. After 3 months of follow up the skin lesions could not be seen any more. Acquired ichthyosis in our patient could be an untoward effect of HMG CoA reductase inhibitor by disturbing the skin lipid metabolism.

2.The Effect of Minizide(R) in the Hypertensive Patients.

The effect of Minizide(R) (prazosine 1mg and polythiazide 0.5mg) was studied in 34 hypertensive patients (female 16 cases, male 18 cases) at the Department of Internal Medicine, Paik Hospital, In-Je Medical College between July, 1979 and August 1980 for 14 months. The patients didn't take the any drugs for one week before the clinical trial with Minizide(R), and thereafter they took minizide 1 tablet 3 times daily for 2 weeks. The blood pressure was checked before and after the trial in the supine and standing position. The results were as followings: 1) The systolic blood pressure before the treatment with Minizide in the supine position was 196+/-24mmHg and in the standing position 188+/-22mmHg. The systolic blood pressure after the treatment with minizide in the supine position was 148+/-16mmHg and in the standing position 139+/-9mmHg. 2) The diastolic blood pressure before the treatment with minizide in the supine position was 118+/-11mmHg, and in the standing position 113+/-10mmHg. The diastolic blood pressure after the treatment with minizide in the supine position was 87+/-6mmHg and in the standing position 84+/-5mmHg. 3) There was no significant orthostatic hypotension after the 34 patients. 4) There was no changes of laboratory findings after the treatment. 5) 3 of 34 hypertensive patients after the treatment complained of headache, 3 palpitation, 2 edema, 2 drowsiness, and 1 fatigability and 1 patient complained of weakness.

3.Giant Osteoma of the Anterior Cranial Fossa: Case Report .

Osteoma is the most common benign tumor of the cranium, and the frontal sinus is its most frequent location in the cranium. A 39-year-old woman presented with left exophthalmos was found to have orbital and intracranial extension of a giant osteoma involving anterior cranial fossa. The etiology, presenting features, diagnosis and treatment of this tumor are reviewed.

5.Atypical Eruption Due to Chemotherapeutic Agent.

We report a case of atypical eruption due to chemotherapeutic agent in a 60-year-old man who presented with asymptomatic, erythematous, 0.5cm in diameter, confluent, and elevated papules and plaques confined to the face. The patient was previously diagnosed with small cell carcinoma of the lung with liver metastasis. Two months after the diagnosis, a first course of chemotherapy including etoposide was started. Five days after starting the chemotherapy, the patient developed a facial eruption. Histopathologic examination demonstrated increased epidermal mitotic figures, cells in metaphase arrest, basal cell layer hyperpigmentation, prominent dyskeratosis, and squamous atypia. The most distinctive histologic feature was the presence of starburst cells, which are markedly enlarged pale staining keratinocytes containing small basophilic fragments of nuclear debris haphazardly scattered throughout the cytoplasm in a starburst pattern.

BACKGROUND: Percutaneous vertebroplasty (PV) is a minimally invasive, image-guided therapy used to relieve pain from osteoporotic vertebral compression fractures. Venography before injection of bone cement has been advocated as a means of identifying sites of potential venous leakage during the procedure. However, venography has been used only in selected situations, and its need is debatable. We aimed to analyze the usefulness of venography with percutaneous vertebroplasties for osteoporotic compression fractures and to report our recent experiences in treating such patients. METHODS: One hundred PVs performed on 93 patients were evaluated. To identify the usefulness of venography, our cases were divided into 2 groups. Group A patients had venographies before the PVs, whereas Group B patients were treated without venography. We analyzed their clinical status, pain status, and complications linked to leakage of bone cement. RESULTS: There were no significant differences in any of the collected data for the two groups. CONCLUSION: Our results indicate that PVs can be performed safely without venography beforehand. However, venography may be beneficial for less experienced physicians or trainees.
