Between April, 1984 and September 1988, 459 patients underwent cardiovascular surgery at the Yeungnam University Hospital. Of these, 355 cases were open heart surgeries and 104 cases were non-open heart surgeries. There were 237 patients of acyanotic congenital cardiac anomalies, 40 patients of cyanotic congenital cardiac anomalies, and 85 patients of acquired heart diseases. The sex ratio of cardiovascular diseases was represented as 1:1.3 in male and female. The age distribution was ranged from 1 day to 65 years old. The common congenital cardiovascular anomalies were ventricular septal defect (38.7%), patent ductus arteriosus (25.5%), atrial septal defect (20.7%), Tetralogy of Fallot (8.3%), and pulmonary stenosis (2.4%) in order of frequency. Among 87 acquired cardiovascular diseases, 81 patients underwent operation for cardiac valvular lesions, 51 patients had mitral valve replacement and 13 patients had aortic valve replacement and 17 patients had double valve replacement. The overall mortality of cardiovascular surgery was 3.3% and mortality of open heart surgery was 3.9%.

Fractures of the capitellum humeri are rare and the recommendations for treatment vary. It can involve a significant portion of the articular surface, rendering the elbow joint unstable. In this situation, it is desirable to reduce and internally fix the capitellar fragment, because this restores the articular surface and augments joint stability. We experienced two cases of capitellar fractures which one case was spontaneously anatomical reduced and the other case was treated by open reduction. In one case the capitellar fragment was spontaneous reduced to a stable position although it was noticed radiographically as an unstable displaced fracture preoperatively. The other case was treated by open reduction and internal fixation with 3.5mm, small, AO, cannulated screw and K-wire. Both cases are reported here with references.

PURPOSE: The purpose of this study was to evaluate and compare the scintigraphic findings and diagnostic accuracy of double-phase Tc-99m sestamibi scan in primary and secondary hyperparathyroidism (HPT). MATERIALS AND METHODS: We retrospectively reviewed 16 cases of primary (18 lesions) and 11 cases of secondary HPT (44 lesions) who underwent Tc-99m-sestamibi scan before the surgical intervention. Scan was performed using LEM camera (Siemens, Germany) after the injection of 740MBq of Tc-99m sestamibi. Routine image consisted of baseline and 3-hour delayed images and each image was obtained using both parallel and pine hole collimator. The study population was 27 patients (male/female=5/22, age: 49.1+/-10.8). RESULTS: Eighteen lesions of primary HPT consisted of 13 adenomas and 5 hyperplasias, while all lesions of secondary HPT were hyperplasias. Among the case of primary HPT, we could detect all the lesions of 13 adenomas but only 2 lesions of 5 hyperplasias (40%) could be detected by double phase scintigraphy. Three cases of primary lesion showed decreased uptake in delayed images compared with baseline. The sensitivity, specificity, positive predictive value and accuracy of primary and secondary HPT were 58.8% (10/17), 83.3% (10/12), 83.3% (10/12), 75.9% (22/29), and 37.5% (15/40), 50% (2/4), 88.2% (15/17), 38.6% (17/44), respectively. Overall sensitivity, specificity, positive predictive value and accuracy were 43.9% (25/57), 75% (12/16), 86.2% (25/29), and 53.4% (39/73). There were no statistical difference between the weight of primary and secondary HPT lesion (p>0.05). CONCLUSION: Tc-99m sestamibi scan is fairly good modality to detect parathyroid lesion in patient with primary HPT before the surgical intervention. However, since some of cases may reveal decreased uptake in delayed image, a careful attention to the findings of baseline images may be helpful. Still the low accuracy of sestamibi scan in diagnosis of secondary HPT prohibits routine use of it for this disease.

Cerebral aspergillosis is a rare condition that affects primarily the immunocompromised host. Most of cerebral aspergillosis is developed by hematogenous dissemination from extracranial foci, but aspergillosis of sino-nasal origin rarely affects the CNS. In case 1, wel symptom of tumor recurrence was unilateral numbness of the chin. A 65-year-old male was admitted because of paresthesia around the left chin and left lower lip. Neurologic examination revealed hypesthesia on the left side of chin, lower lip and buccal mucous mem-brane. Bone scan (Tc-99m MDP) showed focal hot uptakes on the left mandible and left first rib. Brain CT with bone window setting showed a focal osteolytic lesion in the bone marrow of the left mandibular canal without destruction of bone cortex. Both coronal T1 weighted image and axial T2 weighted image showed focal low signal intensities on the left ramus. The pathophysiologic mechanism could be understood by identification of the pathologic focus.

Computed tomographic findings of 24 cases of cervical tuberculous lymphadenitis were retrospectively analyzed. We classified the CT findings as 4 types: type 1(homogeneous soft tissue density mass), type 2(central low density with peripheral rim enhancement and with preservation of surrounding fat plane), type 3(central low density with peripheral rim enhancement and with obliteration of surrounding fat plane), type 4(large confluent low density mass, so called "cold abscess") As a result, there were 2 cases(8.3%) of type 1, 7 cases(29.2%) of type 2, 9 cases(37.5%) of type 3, 3 cases(12.5%) of type 4, 1 cases(4.2%) of type 1 combined with type 3, and 2 cases(8.3%) of type 2 combined with type 4. So 22 cases(91.7%) revealed central low density and peripheral rim enhancement, which suggest necrosis. The maximum thickness of enhancing rim was above 2mm in all of 64 definable necrotic lymph nodes and above 4mm in 42(65.5%) lymph nodes, suggesting that the wall of necrotic tuberculous lymphadenitis tends to be thick. We conclude that CT is useful not only for the diagnosis but also for the evaluation of the extent and the status of cervical tuberculous lymphadenitis.

BACKGROUND: Operations for patients with eye injuries frequently require rapid sequence induction of general anesthesia with succinylcholine (Sch), but Sch as well as endotracheal intubation produce increases in intraocular pressure (IOP). The purpose of this study was to examine whether the increase in dosage of propofol from a usual dosage would prevent the increase in IOP due to endotracheal intubation with Sch during a rapid sequence induction of general anesthesia. METHODS: Changes in IOP were measured in three groups of 15 patients each. The patients randomly received thiopental 5 mg/kg (group 1, n = 15), propofol 2.5 mg/kg (group 2, n = 15) or propofol 3.0 mg/kg (group 3, n = 15), followed by Sch 1.0 mg/kg and endotracheal intubation. IOP, systolic arterial pressure (SAP) and heart rate (HR) were measured before induction (baseline), just after intubation, 3 minutes and 10 minutes after intubation. RESULTS: IOP and SAP just after intubaton in groups 1 and 2 increased significantly from baseline, but did not increase significantly from baseline in group 3. IOP and SAP at 3 minutes after intubation in group 1 did not decrease significantly from baseline, but decreased significantly from baseline in groups 2 and 3. IOP and SAP at 10 minutes after intubaton in all groups decreased significantly from baseline. HR just after intubaton in groups 1 and 2 increased significantly from baseline, but did not increase significantly from baseline in group 3. HR at 3 minutes and 10 minutes after intubation in all groups did not change significantly from baseline. CONCLUSIONS: The authors concluded that propofol 3.0 mg/kg could prevent the increase in IOP, systolic arterial pressure and heart rate just after intubation during a rapid sequence induction of general anesthesia.

Non-steroidal anti-inflammatory agents are used to relieve pain and to reduce swelling in dental clinics. This experiment was performed to study the effect of non-steroidal anti-inflammatory agents in Sprague-Dawley rats on orthodontic tooth movement. Thirty rats were used and divided six groups of five rats each. The first group, administered saline and no orthodontic force, served as a normal group. The second group, administered saline and applied experimental force, was control group. The other four groups were administered Aspirin, Pontal, Tyrenol and Indomethacin each, and applied experimental orthodontic force by 1/4 inch elastic, inserted into the interproximal space between maxillary first and second molar in rats. All experimental rats were sacrificed after three days, and the specimens were sectioned horizontally five times serially, and counted the number of osteoclasts appeared at the compressed surface of interradicular bone on first buccal root of first molar on light microscope. The obtained results were as follows: 1. The number of osteoclast on the compressed surface of the interradicular bone on first buccal root of the first molar in the four non-steroidal anti-inflammatory agents groups decreased in contrast to control group. 2. In non-steroidal anti-inflammatory agents group, the number of osteoclast in Indomethacin group was least among the all non-steroidal anti-inflammatory agents groups. From the above results, it was believed that the non-steroidal anti-inflammatory agents may have the inhibitory effect of tooth movement during orthodontic treatment.

Fifteen young pigs were used in this study. The animals were divided into three groups; 1. Group for removal of the temporal muscle, 2. Group for removal of the masseter muscle, 3. Group for removal of masseter and internal pterygoid muscles. The animals were anesthetized with 3.5% chloral hydrate intraperitoneaily. In the right side the head was shaved. The masticatory muscle was removed. The animals were sacrificed four .months later. The head was separated from trunk and cleaned by boiling in a solution of potassiumm hydroxide. The results were as follows; 1. In the group for removal of the temporal muscle, the Coronoid process of the mandible was resorbed. 2. In the group for removal of the masseter muscle, there was produced asymmetrical growth of the mandible, attrition of the molar teeth in the control side, and resorption of the mandibular angle. 3. In the group for removahof the Masseter and Internal muscles, the changes were more severe than that of the group for removal of the masseter muscle. The mandibular angle was completely absent. 4. The growth of the bone seems definitely related to the presence of the muscular tissue actively pulling upon it.

Facial esthetics is one of the most important goal of the orthodontic treatment and main concern of many patients. Facial esthetics should be considered in orthodontic diagnosis and treatment planning. Prediction of soft tissue profile changes after orthodontic tooth movement should be considered as well. The purpose of this study was to find out the effect of orthodontic treatment on lip profile in adult patient. The pre and post treatment cephalometric reontgenograms of 87 female adult with bimaxillary protrusion were used to analyze lip profile change. All subjects were treated with four bicuspids extraction. Obtained results were as follows: 1. Lip thickness changes after incisor retraction showed different patterns according to areas of the lip. The thickness of the red lip area showed 2.78 mm increase in average. In contrast the thickness of the cutaneous area showed 0.65 - 0.7 mm decrease according to the different cutaneous areas. 2. The length of the red lip area decreased(1.3mm) after incisor retraction. 3. The length of the cutaneous lip area increased(2.9mm) after incisor retraction.

In order to assess the reliability of ultrasonographic criteria of clonorchiasis, a prospective study was performed in 14 patients who were diagnosed as clonorchiasis by ultrasonogram only, using the criteria of peripheral duct dilatation without dilatation of extrah epatic duct. Eleven out of 14 patients were turned out to be clonorchiasis, showing 93% accuracy. Authors in tend to stress these ultrasonographic signs are very well correlated with the CT and E RCP findings and are proved to be highly reliable for clonorchiasis, and thus clonorchiasis could be diagnosed by ultrasonogram only.
