2.Overgrowth after Open Reduction of Femoral Fracture in Children

Femoral fractures are difficult to treat in the child with head or multiple injuries. Traction may fail due to spasticity and restlessness. Poor results after closed reduction have led some authors to advocate open reduction and internal fixation which facilitates overall care. Anatomical reduction with internal fixation leads to limb overgrowth. We report our experience in anatomical open reduction of femoral fracture which had head or multiple injuries or difficult to maintain traction in children at the Kang-Nam St. Mary's Hospital. The results are as follows : 1. Among the 28 cases, there were 20 cases in male and 8 cases in female. Average age was 9.4 years. 2. Plate fixation was used in 27 cases and Kuntscher Kuentscher nail was used in one case. 3. Average union period was 11.5 weeks. There was one delayed union which was completely united in 20 weeks. 4. Average overgrowth of femur was 5.2mm. The overgrowth was related with age and handedness but it was not related with fracture site. For children who have head or multiple injuries or difficult to maintain the skin traction, open reduction with plate fixation appears the most satisfactory treatment. The overgrowth of the femur after open reduction was not significant problems clinically.

3.An Ethnographic Study on Eating Styles of Adult Diabetics in Korea.

PURPOSE: The purpose of this study was to explore adult diabetics' eating styles and factors which influence them. METHOD: The study adopted an ethnographic method based on a perspective which views the eating style as a cultural phenomenon. Data was collected through a personal interview, participant observation, and documented materials from Oct.2001 to Sept. 2002. In this study, fifteen adult diabetics, with an average age of 57, participated. Data analysis was done by the Spradley's taxonomic analysis technique. RESULT: The patients' eating styles were rooted in their viewpoint on illness as well as the meaning of food. Eating styles were classified into 4 types: Pathology-centered, symptom-centered, need-centered, and role-centered. CONCLUSION: A conventional approach to the treatment and management of diabetes did not consider the patient's inner world which may play an important role in the successful management of the disease. We found that it was critical for health care personnel to understand patients' values, beliefs and their way of life in order to facilitate the most successful self-care diet.

4.A Case of Woolly Hair.

We report a case of woolly hair. Woolly hair is found frequently in most blacks but is unusual in individuals of non-negroid origin. A 12-year-old female patient visited our clinic complaining of a hair abnormality. It had been tightly curled, fine, light brown, short and easily broken since birth. On scanning electron microscopy, many of the hairs showed damaged cuticles with cuticular splintering, and most hair shafts were round to oval on cross sectional examination.

6.Cross Training Effect Following Unilateral Leg Strengthening Exercise.

OBJECTIVE: To examine the effects of unilateral leg exercise on the contralateral leg and the cross training effect according to the training intensity. METHOD: Nineteen healthy males volunteered to be subjects for this investigation and were divided into a training group (N=13) and a control group (N=6). One leg of each subject in the training group was randomly assigned to a six week, three day/week isokinetic strengthening training program for concentric knee extension-flexion performed at 60 degrees/second (group A, N=6) and 180 degrees/second (group B, N=7). The control group did not train for six weeks. The strength of the knee extensor and flexor was tested before and after the six week period training by Cybex 770 dynamometer. RESULTS: In both groups A and B, the training resulted in significant increase of knee extensor strength in trained limb compared to control group. However, the strength increment of untrained limb was not significant compared to control group. CONCLUSION: There was no significant cross training effect following unilateral leg strength exercise.

9.Characteristics of 23 MV Photon Beam from a Mevatron KD 8067 Dual Energy Linear Accelerator.

The characteristics of 23 MV photon beam have been presented with respect to clinical parameters of central axis depth dose, tissue-maximum ratis, scatter-maximum ratios, surface dose and scatter correction factors. The nominal accelerating potential was found to be 18.5+/-0.5 MV on the central axis. The half-value layer (HVL) of this photon beam was measured with narrow beam geometry from central axis, and it has been showed the thickness of 24.5 g/cm2. The tissue-maximum, ratio values have been determined from measured percentage depth dose data. In our experimental dosimetry, the surface dose of maximum showed only 9.6% of maximum dose at 10x10cm2, 100cm SSD, without blocking tray in. The TMR'S of 0x0 field size have been determined to get average 2.3% uncertainties from three different methodis; are zero effective attenuation coefficient, non-ilnear least square fil of TMR's data and effective linear attenuation coefficient from the HVL of 23 MV photon beams of dual energy linear accelerator.

10.A Case of Fibrolipoma on the Palm.

Fibrolipoma is a rare histological variant of lipoma characterized by proliferation of mature fat and fibrous tissue. Most of the benign lipomatous tumors in the palm reported in the literature developed in the nervous tissue, especially the median nerve sheath. We report an unusual form of fibrolipoma on the palm which arose from the subcutaneous tissue or superficial palmar fascia in the palm, not the nerve or tendon sheath. There was no recurrence 3 years after surgical removal was performed.
