1.Intrascrotal Dermoid Cyst.

Dermoid cyst is a benign cystic teratoma consisting of 3 germ cell origin tissues with predominant ectodermal origin and most commonly found in the ovary but rare in the scrotum. We reported two cases of intrascrotal dermoid cysts developed in 7 months and 3 years old boys which were confirmed hiatologically. In one case, the cyst developed from the testis and in another from the paratesticular tissue.

2.A Clinical Study on Ligamentous Injuries of the Knee

The knee joint is one of the most susceptable joint to ligamentous injury. For good results, it is very important to make an early diagnosis and to give prompt management. The aim of treatment is to restore the ligaments to their previous anatomic position and tension. The authors reviewed 420 cases of ligamentous injuries in 400 patients who were admitted and managed for 6 years from March, 1981 to February, 1987. The results were as follows : 1. The athletic injury was the most commcn cause of injury(55%), and fall-down was next(26%). 2. The most common ruptured individual ligament was the medial collateral ligament. Combined ligament rupture was more common in medial collateral ligament and anterior cruciate ligament. 3. The ruptured sites of the individual ligament were found to be the following ; a) the medial collateral ligament was ruptured most frequently at the femoral attachment(43%), with the superficial layer ruptured in the midsubstance and the deep layer at the femoral attachment; b) the lateral collateral ligament torn at the fibular attachment(51%); c) the anterior cruciate ligament torn at the femoral attachment(39%); d) the posterior cruciate ligament torn at the femoral attachment(64%). 4. Many ligamentous injuries were associated with fracture(44%) and tear of meniscus- (42%). 5. In stress-radiogram, the significance of ligament injuries was present in more than 10 and 10 mm. 6. Excellent and good results were obtained in 94% by non-surgical treatment and in 92% by surgical treatment. 7. Early surgical repair of the ligament within 2 weeks after injury, gave much better result than later repairs.

3.A Clinical Observation on Injuries of Genitourinary Tract.

A clinical observation was made on the injuries of genitourinary tract of the in-patients in the Department of Urology, Kyung Hee University Hospital during the period from July 1, 1972 to June 30. 1977. The following results were obtained. 1) Of 689 cases hospitalized, 61 cases were injuries of genitourinary tract, giving a rate of 8.9%. 2) The age ranged from 3 to 70 years and most favorable age was in from 20 to 39 years for 47.6%. The sex ratio, male to female, was 11.2 : 1. 3) The urethra was involved most frequently in 39.3 %, the kidney in 32.8 %, the bladder in 14.8 %, the external genitalia in 11.5 % and the ureter in only one case which was caused by grnecologic operation. 4) Traffic accident was most common cause of the injury in 39.3% and the next was kick and blow in 26.2 %, fail in 13.1 %. 5) Operation in renal injuries was performed in 5 of 20 cases. 6) In bladder injuries, the extraperitoneal type was in 5 cases, the intraperitoneal in 2 cases and Contusion in only one case. 7) 66.7 % of urethral injuries were associated with pubic bone fracture.

4.Fracture of Penis.

Fracture of the penis a rare condition, properly defined as a tear of the fibrous sheath of the corpora cavernosa occurred by direct blow or bending of the penis during erection. Its characteristic pathology is a tear in the tunica albuginea of the corpora cavernosa with hemorrhage, hematoma formation and distortion of the organ. Herein we reported a case of fracture of the penis, occurred by rolling in bed during erection in 26 years old man and repaired surgically.

5.Clinical Observation on Endoscopy.

A clinical observation was made on one thousand and seventeen cases of endoscopy, performed in the Department of Urology, Kyung Hee University Hospital during the period from January 1, 1977 to December 31, 1978. The results are summarized as follows. 1) During the period, of 5,480 patients visited, 987 patients were examined endoscopically, giving a rate of 18%. 2) The age was ranged from 3 to 91 years old and the most favorable age was in 30 to 49 years old in 49.9%. The sex ratio, male to female, was 1 : 1.43) The common diseases, diagnosed endoscopically in male, were bladder neck contracture 126 cases (29.2%), posterior urethritis 109 cases (25.3%), benign prostatic hypertrophy 51 cases (11.8%) but in female, chronic urethritis was the most common disease in 419 cases(71.5%) and the next subacute cystitis 37 cases (6. 3%), bladder tumor 13 cases (2.2). 4) In male, endoscopy was indicated most commonly due to infravesical obstruction symptoms such as hesitancy, forceful urination, small urinary stream in 78 cases (20.6%) but in female most commonly due to bladder irritation such as frequency, tenesmus, urgency in 239 cases (48.4%). 5) Main symptoms in female urethritis were frequency in 197 cases (47.0%), tenesmus in 156 cases (37.2 %), suprapubic discomfort in 59 cases (14 %). 6) The most common cystoscopic finding in female urethritis was trabeculation in 226 cases (53.9%). 7) The panendoscopic findings in female urethritis were classified into 4 groups by Kim's method The most common group was group IV in 189 cases (45.1%) which showed marked inflammatory changes on bladder neck and urethra. 8) The meatal stenosis was observed in 27.8% of 806 female patients, checked endcscopically.

6.Epoxy Resin Contact Dermatitis.

Epoxy resins,which are irritant and allergen, are now used for surface coatings, electrical insulations, adhesives, and construction industry throughout the world. However, epoxy resin contact dermatitis has not been reported in the Korean literature. We report two cases of epoxy resin contact dermatitis which were occupationally related. They have been in the same work, painting dye and resin mixtures on the glasses for decoration. They showed positive patch test reactions to epoxy resin 1, and their own materials containing resins and accelerators which were proved to be -countinue-

7.Two Cases of Juvenile Xanthogranuloma.

Two Cases of Juvenile XanthogranuIoma(JXG) are reported with review of literatures. JXG is a rare disease, and have not yet been reported in this country. Case I: Two months oId healthy female infant has two firm, discrete, brownish yellow waxy nodules on the left postauricular region and the left forearm. The lesion had begun as erythematous pinhead sized papules, and have been enlarged rapidly to pea siez during last three weeks. Case II: Three year old healthy male has firm, discrete, pinhead to pea sized scattered 16 papules and nodules on the trunk and right side of neck. Laboratory findmgs and X-ray studies were normal in both cases and there were no evidence of metabolie disorder or bony abnormality. Histopathologic studies show similar findings in both cases. In hematoxylin-eosin staining, epidermis shows parekeratosis and slight elongation of rete ridges with suprapapillary thinning with focal spongiosis and invasion of inflammatory cells. The entire dermis, from dermal papillae to sbcutis, is replaced by granulomatous lesion composed with histioeytes and a few giant cell reaction, in addition to histiocytic proliferation, considerable amount of eosinophils are infiltrated with some lymphocytes and plasma cells. The dermal collagen fibers are partly degenerated. In the upper most portion, some foamy histiocytes are also seen. In frozen section and Sudan III fat staining, the tissue shows rnild reactivities.

10.Neopterin as a Marker of Disease Activity in Childhood Acute Lymphocytic Leukemia.

PURPOSE: Neopterin is a degradation product derived from guanosin triphosphate (GTP) during biosynthesis of tetrahydrobiopterin. It is produced by human monocytes/macrophages upon stimulation with interferon-gamma from activated T lymphocytes. Therefore, increased neopterin concentration suggests the activation of cell-mediated immune response during viral infections, autoimmune diseases, allograft rejection, immunodeficiencies and certain types of malignancy. In these diseases, increased neopterin concentrations in serum and urine correlate to the stage of disease and predict worse prognosis. It has been proved that in various malignant disorders, higher neopterin concentrations are associated with more rapid disease progression and are a valuable predictor of stage and extension of the tumor. METHODS: Serum neopterin concentrations were measured by ELISA test in 20 patients with acute lymphocytic leukemia (ALL) at diagnosis and at complete remission after induction chemotherapy, and in 20 normal children that served as the control group. RESULTS: Serum neopterin levels were significantly (P=0.021) increased in the ALL group at diagnosis (9.48 nmol/L) compared to the control group (4.49 nmol/L). In the ALL group, serum neopterin levels were significantly (P=0.015) decreased at complete remission state (6.84 nmol/L) compared to the time of diagnosis (11.96 nmol/L). CONCLUSION: Neopterin was shown to be a valuable indicator of disease activity in childhood ALL and will contribute to predicting the treatment response.
