The 1224 cataractous patients who had extracapsular cataract extraction and posterior chamber lens implantations were evaluated retrospectively irrespective of style, postoperative position of intraocular lens and surgeon. The 100 cataractous patients who had extracapsular cataract extraction and posterior chamber lens implantations in the bag accurately using single lens style by one surgeon(E.H.L) were evaluated prospectively. And then the predictive accuracy of the SRK/T intraocular lens power calculation formula was compared with other formulas (SRK, SRKII and Holladay) without consideration of individual A constant. The SRK/T formula was more accurate than other 3 formulas in all axial length, but there were no significant differences statistically. Especially for short and long eyes, SRK/T formula was more accurate than other 3 formulas.

Purpose: This study sought to evaluate the clinical effectiveness of the shortening effect of the modified Weil osteotomy for the treatment of Freiberg’s disease. Materials and Methods: We reviewed 21 cases treated with the modified Weil osteotomy for Freiberg’s disease from November 2005 to June 2019. The average follow-up period was 32.5 months and the mean age of the patients was 38.3 years. The clinical results were analyzed using the American Orthopaedic Foot and Ankle Society (AOFAS) lesser metatarsophalangeal-interphalangeal scale, the visual analogue scale (VAS), and the range of motion (ROM) of the metatarsophalangeal joint. In the radiologic evaluation, the length of preoperative and postoperative metatarsal shortening was compared. Results: The average AOFAS lesser metatarsophalangeal-interphalangeal scale showed an improvement from 60.5 preoperatively to 90.9 at the latest follow-up. VAS showed a decrease from 5.4 preoperatively to 0.9 at the latest follow-up. ROM of the affected metatarsophalangeal joint increased from 40.2 degrees preoperatively to 58.6 degrees at the latest follow-up. The mean length of metatarsal shortening was 6.7 mm. There was no transfer metatarsalgia, osteonecrosis, and definite joint space narrowing. Conclusion Modified Weil osteotomy with second layer cutting is an effective treatment option to restore the joint surface and painless joint motion for patients with Freiberg’s disease.

One case of unilaterally developed acute retinal necrosis syndrome was studied with retina and vitreous from biopsy during operation through electronmicroscope pathologically and virolo gically. In photoreceptor cells, generalized necrosis was found: loss of outer segments, fragmentation of inner segments, and decrease of mitochondria in ellipsoids. Bipolar cells and axons were well preserved. Internal limiting membrane was intact, and covered by an unidentified cell. Degenerated retinal proper had various sized vacuoles. Around the intraretinal vessels, neutrophils and lymphocytes were infiltrated considering vasculitis. In ganglion cells, swelling of mitochondrias were seen, and their cell organelles were decreased, No virus particle was detected in all layers of the retina with electronmicroscope.

The monopolar and bipolar coagulation lesions were generated in the spinal cord of rabbits. Microscopic study, and the distance from surface lesion center to its margin, the lesion depth, the extent of lesion in cross section, and the area of cross section through the lesion center of monopolar and bipolar electrocoagulation in generation of spinal cord lesion were studied by Evans blue and hematoxylin-eosin stain. The results were as follows : 1) Microscopically, the lesions were characterized by ground substance depigmentation, vacuolization and neuronal damage with pyknotic and angulated nuclei and loss of detail. In section stained with Evans blue, the lesion was demarcated with blue-green discoloration of the parenchyma and superficial necrosis was at coagulation sites. 2) The distance from surface lesion center to its margin was proportionally increased as the increase of magnitude of electric current for monopolar and bipolar coagulation, while the distance was independent of the mode coagulation and stain. 3) The depth of lesion was proportionally increased as the increase of magnitude of electric current for monopolar and bipolar coagulation, while the depth was also independent of the mode coagulation, but the depth of Evans blue stain was 2 times than H-E stain. 4) The extent of lesion in cross section was increased as the increase of magnitude of electric current for both coagulation modes, while the extent was independent of the mode of coagulation and stain, too. 5) The area of cross section through the lesion center was gradually increased as the increase of magnitude of electric current for both coagulation modes, while the area was independent of the mode of coagulation and stain.

We rotated the nucleus up to 270 degree and dislocated it into anterior chamber through the diameter of capsulorhexis after hydrodissection by using the planned extracapsular cataract extraction. And later on we delivered the nucleus with hydroexpression using the irrigating Vectis or with rotation using a lens spatuala. As a result of this procedure, the nucleus was safely delivered in all cases without zonular rupture and vitreous prolapse.

We treated a case of extramammary Paget's disease that affected the scalp of a 45-year-old female. It is extremely rare that the disease arises in areas other than the anogenital region and the axillae. The lesion was a round erythematous oozing crusted hairless patch. Histopathologically, many pagetoid cells were found within the epidermis and dermis. The cytoplasms of these cells stained with alcian blue at pH 2.5, CEA, EMA and low-molecular-weight-cytokeratin. The patient underwent a wide local excision. We used a mapping technique to reveal the distribution of microscopically involved lesions.

BACKGROUND: To estimate real time concentration of drugs during TIVA is theoretical, but it is not easy and inefficient. To maintain designed target concentration with continuous infusion using methods that account for the multicompartmental pharmacokinetic profile of fentanyl, isoconcentration nomogram is one of the methods. We evaluated the clinical usefulness of the isoconcentration nomogram using two different expected concentration of fentanyl. METHODS: Thirty ASA class I or II adult patients scheduled for spine fusion were randomly allocated into two groups according to 1.5 or 3 ng/ml of expected fentanyl concentration. Using isoconcentration nomogram, fentanyl concentration was adjusted and the propofol concentration was fixed to 3.5 g/ml according to Prys-Roberts method. Vital signs were titrated using variable flow rate of propofol. Fentanyl and propofol were discontinued 15 min before the end of operation. And, IV-PCA using fentanyl were applicated for postoperative pain control. The dosage of propofol and fentanyl, recovery time of consciousness and orientation were checked. Also, first buttoning time and 24hr fentanyl dosage in IV-PCA were checked. RESULTS: Average flow rate of propofol used were 7.5 1.2 mg/kg/hr in group 1, 5.7 1.1 mg/kg/hr in group 2 which was significantly lower than group 1 (p<0.05). Spontaneous eye opening and recovery of orientation was delayed 1.8 times in group 2. First buttoning time and 24hr fentanyl requirement for postoperative pain control using IV-PCA was delayed by 2 and decreased 60% in group 2, respectively. CONCLUSIONS: Isoconcentration nomogram was useful tool to control the expected concentration of fentanyl during TIVA and postoperative pain control using fentanyl IV-PCA.
