2.Triple Arthrodesis: Review of 92 feet

Triple arthrodesis is performed to obtain stable stature and walking, to eliminate pain, to correct the deformities of the foot. This is the result of 92 feet triple arthrodesis over a period from 1976 to 1991 at the Department of Orthopaedic Surgery, Yonsei University College of Medicine. The results are as follows 1. Mean age was 17.6 years, ranged from 10 to 51 years. 2. Of the total of 92 feet, sequelae of poliomyelitis was the most common etiology with 58 feet(63%). 3. Most common type of foot deformity was talipes equinovarus with 52 feet(56%). 4. Internal fixation was done with staple or Steinmann pin combined with autogenous bone graft. Among them, using of 2 staples was the most common fixation method, 47 cases(51%). 5. As necessary, triple arthrodesis was combined with tendo-calcaneus lengthening, plantar fasciotomy, posterior capsulotomy, modified Jones operation and tibiotalar arthrodesis. 6. Final results were ideal. in 22 cases(24%), good in 51 cases(56%), fair in 16 cases(17%) and fail in 3 cases(3%). 7. There were 2 cases of pseudoarthrosis and 1 case of residual equinus deformity after operation. In conclusion, triple arthrodesis was the efficient method of treatment for the deformed feet in skeletally matured patients.

4.Balanitis Circumscripta Plasmacellularis.

A 79-year-old man with balanitis circumscripta plasmacellularis(BCP), presenting as an erythematous constricting band of the inner surface of the prepuce encircling the penile shaft is described. The biopsy specimen of the lesion showed, in addition to the typical histologic findings of BCP, increased fibrosis and decreased amount of elastic fibers which correlate well with our clinical observations. Electron microscopic examination revelaed no viral particles or elastic fibers. Immunohistologically, IgG was found to be the major immunoglobulin class in the plasma cellular inf iltrate.

5.Fifteen Cases of Fournier`s Gangrene.

PURPOSE: This study was made to evaluate the clinical characteristics and the treatment of the gangrene of the male genitalia manifested with progressive life threatening disease. MATERIALS AND METHODS: Between June 1992 and December 1998, medical records for patients with Fournier's gangrene were reviewed with respect to age at presentation, associated disease, and clinical course and the treatment. RESULTS: The age of patients ranged from 29 to 92 years. The prodromal sign noted were usually low abdominal and inguinal pain. There were no differences in the causative pathogens compared to those of previous reports. The common underlying diseases were diabetes and chronic liver disease. Hypoalbuminemia and leukocytosis were found in all. Early radical debridement and aggressive antibiotics therapy were performed in all cases. Reconstructive surgery of defected skin was performed 7 to 45 days(mean 33.6) after debridement and period of hospital stay ranged from 14 to 123 days(mean 35.8). The motality rate was 13.3%. CONCLUSIONS: It is thought that the motality rate of Fournier`s gangrene is reduced by early aggresseve debridement of the wound and empirical triple antibiotic therapy, and the comprehension of clinical course is important as well.

10.Cure of severe hypothermia due to loss of consciousness after drug intoxication of inhaled hydrocarbons, Case 1.

A 19-year-old man had a good neurologic recovery from a severe hypothermia(19 degrees C) and a prolonged coma following active infernal rewarming. From inhaling hydrocarbons, he was left unconscious on the cold floor for 24 hours. As soon as the patient was brought into the emergency medical center, he was early evaluated arid treated aggressively. ECG showed Osborn(J) wave on all leads. The temperature o( patient was increased by 2-3 degrees C per hour through active external rewarming (by heating blankets and warm bag) and active infernal rewarming (by airway rewarming, warmed IV fluids, gastrointestinal tract irrigation, and bladder irrigation). The temperature reached 36 degrees C after 6 hours. Active infernal rewarming provides rapid core rewarming with the additional benefit of circulatory support during the period of cardiac instability.
