6.A Clinical Study on Ligamentous Injuries of the Knee

The knee joint is one of the most susceptable joint to ligamentous injury. For good results, it is very important to make an early diagnosis and to give prompt management. The aim of treatment is to restore the ligaments to their previous anatomic position and tension. The authors reviewed 420 cases of ligamentous injuries in 400 patients who were admitted and managed for 6 years from March, 1981 to February, 1987. The results were as follows : 1. The athletic injury was the most commcn cause of injury(55%), and fall-down was next(26%). 2. The most common ruptured individual ligament was the medial collateral ligament. Combined ligament rupture was more common in medial collateral ligament and anterior cruciate ligament. 3. The ruptured sites of the individual ligament were found to be the following ; a) the medial collateral ligament was ruptured most frequently at the femoral attachment(43%), with the superficial layer ruptured in the midsubstance and the deep layer at the femoral attachment; b) the lateral collateral ligament torn at the fibular attachment(51%); c) the anterior cruciate ligament torn at the femoral attachment(39%); d) the posterior cruciate ligament torn at the femoral attachment(64%). 4. Many ligamentous injuries were associated with fracture(44%) and tear of meniscus- (42%). 5. In stress-radiogram, the significance of ligament injuries was present in more than 10 and 10 mm. 6. Excellent and good results were obtained in 94% by non-surgical treatment and in 92% by surgical treatment. 7. Early surgical repair of the ligament within 2 weeks after injury, gave much better result than later repairs.

7.A Clinical Study of the Ankle Fracture

The ankle is a complex structure supporting the entire musculoskeletal system during standing and walking. The injuries to the ankle joint result in a severe functional disturbance because of complex anatomical characteristics around the ankle joint. 284 patients of the ankle fracture that were treated in orthopedic Dept. Capital Armed Forced General Hospital from march 1985. to march 1988. were analized in clinical and radiological aspects. The following results were obtained: 1. The main cause of the injuries was falling from a height and the other were sports injury and slipping down in order. 2. The most common type by Lauge-Hansen classification was supination-external rotation type(122 case, 42.9%). 3. 211 case(74.3%) were treated with open reduction and internal fixation, and 73 case(25.7%) were treated with closed reduction. Poor outcome seemed closely related to the severity of the injury and degree of fracture displacement. 4. Accurate reduction and rigid internal fixation of the lateral malleolus with shortening was important factor. 5. Classification of Lauge-Hansen was useful in the diagnosis and treatment of the ankle fractures.

8.Distribution of S-100 Protein Positive Dendritic Cell and its Correlation with Degree of Malignancy in Gastric Carcinoma.

The present study was performed in order to investigate the correlation among the number of T zone histiocytes, the clinicopathologic parameters and the patient's survival. The subjects in this study were 131 advanced gastric carcinomas including 86 follow-up cases and 41 early gastric carcinomas. The infiltration of T zone histiocytes into tumor tissues was investigated using the immunohistochemical method with andti s-100 protein, nd the data obtained were evaluated statistically. T zone histiocytes were scattered among the cancer cells and in areas formed clusters within the cancer stroma. These cells were abundantly present in tumor tissues and around lymphoid follicles but were rarely seen in surrounding normal stomach tissue. Analysis of the number of T zone histiocytes showed no correlation between the number of T zone histiocytes and the clinicopathologic features except the degree of lymphocytic infiltration within the advanced and the early gastric carcinomas. Survival of the patients with a marked infiltration of T zone histiocytes in stage III was longer than that with mild infiltration of T zone histiocytes, but there was no statistical significance(P value=0.084). Multivariate regressio analysis revealed that the depth of invasion(P value=0.0074) and the lymphocytic infiltration(P value=0.0152) were the important prognostic factors. The results indicate that good prognosis is expected in cases with less deep invasion and high lymphocytic infiltration, and that the number of T zone histiocytes is in proportio to the degree of lymphocyte infiltration within the tumor, although not directly correlated with the patient's survival.
9.A Statistical Study of New Outpatients During the Rescent 5 Years (1976~1980).

The statistical study of new outpatients of skin problems seen in dermatologic department of Seoul Red Cross Hospital from l976 to 1980 was done and the 10 most common dermatoses were analysed in detail, The results were summarized as follows: 1) Of 19,679 outpatients, males were 10,530(53.5%) and females were 9,149(46.5%). The sex ratio of male to female was 1. 15: l. 2) The most common dermatoses were atopic dermatitis/eczema(16. 9%), fungal infection(16. 2%), contact dermatitis(15. 3%), scabies(10. 7%), urticaria(10. 6%), acne(9. 5%), pyoderma(8. 3%), insect bite(5. 6%), syphilis(4. 1%) and psoriasia (2. 8%). 3) Of 10 most common dermatoses, contact dermatitis, acne, scabies and urticaria were revealed to have a tendency to increase year by year, syphilis and psoriasis were constant pattern and other common dermatoses showed irregular pattern in their incidence. 4) The sex ratio of male to female of l0 most common dermatoses was l. 23: l. Fungal infection, scabies, pyoderma, insect bite, syphilis and psoriasis wereoutnumbered in male and atopic dermatitis/eczema, contact dermatitis, urticaria and acne were outnumbered in female. 5) Atopic dermatitis/eczema & scabies were prevalent in winter, acne, fungal infection, urticaria and pyoderma in summer, and contact dermatitis in spring and summer. 6) The most frequent age groups of atopic dermatitis/eczema were 1 to 10 years, acne and fungal infection, 11 to 30 years, and contact dermatitis and fungal infection, 31 to 50 years.
