1.One Case of Poikiodermatomyositis.

The authers observed one case of typical poikilodermatomyositis who was 47 year old female. The poikiIodermatomyositis is a rare disease which is a variant of dermatomyositis and has never been reported in Korea. This patient complained of a weakness of left lower leg and tingling sensation of both fingers since one year ago, And she has a difficulty of both finger flexion since 20 days ago. The skin lesions revealed mottled dark brownish pigmentation, hardness, scattered atrophic patches and telangiectasia in the lesion on the left lower leg since one year ago. The same skin changes appeared on the thigh, lower abdomen and upper chest on 4 months ago. Histopathologically, a biopsy was performed from the skin of lower leg, the skin shows typical poikiloderma, i.e. the epidermis shows moderate atrophy of the stratum malpighii, flattened rete ridges and hydropic degeneration of the hasal cells. The dermis is edematous and a band like dense lymphocytic cellular infiltration. And the muscIe biopsy from gastrocnemius muscle shows dermatomyositis finding which the muscle bundles are edematous and degenerative. Many inflammatory cells present between muscle bundles but there is no suppurative change. Diagnosis was confirmed by clinical appearance, histopathologically and other laboratory studies. The patient was markedly improved by the prednisolone 80 mg and vasculat (vascular dilatator) 100 mg daily.

3.Magnetic Resonance Imaging Findings of Intraspinal Neurenteric Cyst: Case Report.

Intraspinal neurenteric cysts are rare congenital lesions that results from abnormal separation of germ layers in the third week of embryonic development, which may cause spinal compression. Although, the diagnosis of neurenteric cyst was very difficult prior to operation, MRI has proven to be a useful imaging modality in detection, localization and characterization of intraspinal neurenteric cysts. We recently experienced intraspinal neurenteric cyst in two patients who presented with progerssive quadriparesis. Myelography, CT myelography and MRI were taken and complete excision was performed. The MRI findings are presented and the literature is reviewed.

4.Malignanr Acanthosis Nigricans: Report of a Case.

6.A Study on the Influence of Serum IgE Level on T cell Function in Atopic Dermatitis.

The influence of serum IgE level on T cell function and number in atopic dermatitis was performed. For the study we selected 2 groups of atopic derrnatitis patients which were 10 cases of atopic dermatitis patients with more than 400 IU/ml of serum 1gE level and 1(l cases with less than 400 IU/ml, The lymphocyte transformation test with phytohernagglutinin and E-rosette test with AET treated sheep cell were perforrned in each group and compare the result of each group with each other and al'o with those of control group. The reults are as follows; l. The mean serum level of IgE in AD(538. 8 407.42 IU/ml) was higher than in normal control(126. 5 133, R8 IU/ml) (p<0.01). 2 The mean LTT S.I. in AD(8.83 -3.85) was lower than in nomnal control (13.99 5.55) (p<0.01). 3. The mean percentage of E-rosette in AD(68,65+6.6g) was lower than 4pp IU/ml(mean+2x SD of serum IgE of control groups), 1) The LTT S, I. was decreased in both groups according to increment of serum IgE, The higher IgE group(8.64+3,9p) had statistical significance compared to normal control(13.gg+5.55) (p<0.01) but not in low IgE group(9.08:4.11).2) The percentage of E-rosette was decreased in both groups according to increment of serum IgE. The both groups of low IgE(71.08.-6,24) and high IgE(66. 22 6. R7) had statistical significance compared to normal control(77.65+4.39 %) (p 0.01).

8.A Case of Cutaneous Metastasis of Male Breast Cancer.

Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema.

9.Malignant Acanthosis Nigricans Associated with Severe Oral Florid Papilloma.

One case of malignant acanthosis nigricans associated with internal malignany and oral florid papilloma has been presented and reviewed the subject of malignant acanthosis nigricans. The patient who was 34 year old man had been affected generalixed dark brownish hyperpigmentation with papillomatoses especially on forehead, neck, axillae, umbilical area, anogenital area, webs of fingers, palms and soles. And characteristically it was associated with severe florid papilloma on mucous membrane such as buccal mucosa, lip and glans penis. Histopathologically, internal malignancy was confirmed metastatic adenocarcinoma of stomach after autopsy, although previous biopsy finding was squamous cell carcinoma when explorative laparatomy was done. The response to Bleomycin, new antibiotic anticancer drug, followed by intravenous injection is good and satisfying. But the patient died due to sudden pulmonary hemorrhage which are serious side effect of Bleomycin.

10.Applicability of Genes of Cancer-associated Testis Antigens in Diagnosis of Cancer.

Genes of cancer-associated testis antigens (CTAs) are expressed in various cancer tissues. In order to use CTAs as cancer diagnosis marker, we developed molecular method for detection of CTAs transcripts in tissue. In order to know the applicability of DNA of cancer-associated testis antigens (CTAs) on cancer diagnosis, molecular diagnostic methods for detection of gene expression of melanoma antigen gene (MAGE), GAGE, and B melanoma antigen (BAGE) was studied. After comparing DNA sequences of CTAs, S1/AS1 and S2/AS2, GAGE-S/ GAGE-AS, and BAGE-S/BAGE-AS primers were designed for the detection of MAGEs, GAGEs and BAGEs, respectively. The gene expression of CTAs in cancer cell lines, head and neck cancer tissues, ovary cancer tissue, and peritoneal cells of gastric cancer patients were investigated by reverse transcription-polymerase chain reaction (RT-PCR) using these primers. The MAGEs, GAGEs and BAGE genes were expressed in 8/8 (100%), 5/8 (62.5%) and 1/8 (12.5%) of head and neck cancer tissues, respectively. The gene expression of MAGEs were also detected in 8/10 (80%) of ovary cancer tissues and in 9/10 (90%) of peritoneal cells of gastric cancer patients in RT-PCR test using S1/AS1 primers. The results of this study suggest that molecular diagnosis method using CTAs genes, especially RT-PCR using S1/AS1 primer combination, is useful for diagnosis of cancer and it will be used for the prediction of cancer progression or regression and metastasis in future.
