1.A Case of Glomus Tumor.

A case of solitary painful glomus tumor occurring in a 36-year-old Korean rnale is reported. This tumor was a slightly elevated, bluish subcutaneous nodule about 1cm. in diameter on the left antecubital fossa. We performed a complete surgical excision of the nodule, which was histopathologically cnnfirmed as a typical solitary glomus tumor.

2.A Case of Epidermodysplasia Verruciformis.

We reported a case of epidermodysplasia verruciformis which occurred in a 15 year-old girl. She had an extensive eruption of plane warts on the face, neck, upper trunk, and extremities, especially on the dorsa of the hands and feet, with symmetrical diatribution. The skin lesions appeared initially on the forehead and gradually generalized. all over the body for 10 years duration. Skin biopsy showed hyperkeratosis with basket-weave pattern, thickened granular layer, vacuolization of many celIs in the upper stratum malpighii and the granular layer and acanthosis.

3.Hyperbaric Oxygen Therapy in Pyoderma Gangrenosum.

Pyoderma gangrenosum, as described by Brunsting and his aaaociates in 1930, is characterized by the rapid appearance of one or more bluish-black, boggy, undermined ulcers, most frequently on the legs of peraons with ulcerative colitis or rheumatoid arthritis. Its pathogenesis is often obscure. Present management of pyoderma gangrenosum consists of local treatment of the wound, systemic cortieosteroid therapy, and treatment of any underlying disease. In our 21 year-old male with pyoderma gangrenosum, hyperbaric oxygen therapy was performed. Treatment was given in the hyperbaric oxygen one-man chamber at 3.0 ATA for 30 minutes daily for 49 days. By 36 days, the ulcers on the face and neck was healed. Approximately one month after the patient was discharged, complete resolution of the 1esions on the back occurred.

4.A Case of Reiter's Syndrome.

We presented a case of Reiters syndrome consisting of conjunctivitis, arthritis and mucocutaneous lesions, occurring in 49 year-old Korean male. He had an pisode of dysentery 18 months before the onset of the syndrome and later followed by transient conjunctivitis but no history of venereal disease was revealed. He had the relatively well demarcated erythematous scaly patches on the scalp, face and back, and also the solitary or confluent thickened hyperkeratotic plaques on the dorsum of the feet and hands and lower extremities. Balanitis circinata as .a lesion of the mucous membrane, was seen but there were no lesions on the oral mucosa and palate. Arthritis of the knee and elbow joints were very scvere, accompanied by severe pain and limited motion, and severe wasting and weakness of the muscles of both lower extremities were noticed. He was treated with penicillin and corticosteroids systemically combined witb physiotherapy with some improvement of both skin lesions and joint pain but he died 3 months later probably of the far advanced pulmonary tuberculosis, gastrointestinal bleeding and cachexia.

5.Angiographic Diagnosis for Various Causes of Gastrointestinal Bleeding.

PURPOSE: To evaluate the specific findings of angiography for various causes of gastrointestinal bleeding. MATERIALS AND METHODS: We retrospectively analyzed 35 patients of gastrointestinal bleeding detected by angiography, between April 1987 and July 1993. There were 28 men and 7 women, 19 to 83 years old (mean, 47 years). Of these 35 cases of gastrointestinal bleedings, there were 12 gastric bleedings, 4 duodenal bleedings, 9 small intestinal bleeings, 6 ileocecal bleedings, and 4 large intestinal bleedings. RESULTS: Of 35 cases, we could diagnose the specific cause of bleeing in 13 patients (37%) by angiography. Of these 13 cases, there were 4 cases of gastric ulcers showing contrast pooling in ulcer crater, 5 cases of vascular malformations showing anomalous vascular mass with early dilated venous drainage, 2 cases of direct invasion of hepatocellular carcinoma showing invasion of hypervascular mass into small bowel, one case of pseudoaneurysm at gastroduodenal artery in a patient of acute pancreatitis, and one case of small intestinal lymphoma showing contrast pooling in a irregular dilated small intestinal lumen. CONCLUSION: in gastrointestinal bleeding, angiography is useful not only in detecting the bleeding site, but also for evaluating the specific causes such as gastric ulcers or vascular malformations.

6.A Case of Multiple Endocrine Neoplasia type 3 With Multiple Mucosal Neuroma.

Multiple endocrine neoplasi~a type 3 is a rare, heritable or sporadic, multiple neoplastic disorder that is charracterized by thyroid medullary careinoma, pheochromocytoma, multiple mucosal neuroma, and marfanoid habitus. The most important disorder of the syndrome is mucosal neuroma that is an early diagnostic sign of multiple endocrine neoplasia type 3. Early diagnosis of MEN type 3 determines prognosis of the disease. We present a case of multiple endocrine neoplasia type 3 of the sporadic pattern in a 27-year-old man who had typical medullary thyroid carcinoma, mucosal neuroma, marfanoid habitus, and megacolon.

7.A Study on the Influence of Serum IgE Level on T cell Function in Atopic Dermatitis.

The influence of serum IgE level on T cell function and number in atopic dermatitis was performed. For the study we selected 2 groups of atopic derrnatitis patients which were 10 cases of atopic dermatitis patients with more than 400 IU/ml of serum 1gE level and 1(l cases with less than 400 IU/ml, The lymphocyte transformation test with phytohernagglutinin and E-rosette test with AET treated sheep cell were perforrned in each group and compare the result of each group with each other and al'o with those of control group. The reults are as follows; l. The mean serum level of IgE in AD(538. 8 407.42 IU/ml) was higher than in normal control(126. 5 133, R8 IU/ml) (p<0.01). 2 The mean LTT S.I. in AD(8.83 -3.85) was lower than in nomnal control (13.99 5.55) (p<0.01). 3. The mean percentage of E-rosette in AD(68,65+6.6g) was lower than 4pp IU/ml(mean+2x SD of serum IgE of control groups), 1) The LTT S, I. was decreased in both groups according to increment of serum IgE, The higher IgE group(8.64+3,9p) had statistical significance compared to normal control(13.gg+5.55) (p<0.01) but not in low IgE group(9.08:4.11).2) The percentage of E-rosette was decreased in both groups according to increment of serum IgE. The both groups of low IgE(71.08.-6,24) and high IgE(66. 22 6. R7) had statistical significance compared to normal control(77.65+4.39 %) (p 0.01).

8.A Case of Cutaneous Metastasis of Male Breast Cancer.

Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema.

9.Leiomyosarcoma of the Skin: Report of A Case.

Leiomyosarcoma arising in the skin is rare tumor, and diagnosis usually is made microscopically. After local excision, these lesions recur in large proportion of pat ients. The authors herein report a 53-year-old male with leiomyosarcoma appeared in the skin of the right forearm and presenting as a dark reddish colored, 5*6cm in diameter, superficial ulcerated single firm nodule with intermittent pain. Histopathological examination showed poorly circumscribed tumor consisting of interlacing bundles of spindle shaped smooth muscle cells in the middle and lower parts of the dermis. The nuclei were hyperchromatic, large, vacuolated, and irregular in shape. Electron microscopic findings revealed cytoplasmic organelles such as rough endoplasmic reticulum and mitochondria of malignant smooth muscle cells in the paranuclear area, Characteristic subsarcoelmmal caveolae and dense plaque were noted and myofilaments were distributed in the peripheral cytoplasm. The tumor did not recur in 10 months' follow-up.

10.Clinical Study of Traumatic Hyphema.

We reviewed 114 patients who were admitted and treated at the Chonju Presbyterian Medical Center between 1986 and 1990 for nonperforating traumatic hyphema. Of these 114 cases, 103 (90.4%) were male and 11 (9.6%) were female. Mostly, they were under 20 years of age. Grade I hyphema (bleeding less than one third of anterior chamber) was 72 cases (63.2%). The causes of injuries were blows to the eye (56 cases, 49%) and projectile objects (45 cases, 39.5%). Associated injuries to the eye and periocualr structures occurred in 78 patients (68.4%), which were angle recession (23.7%), eyelid laceration (20.2%), increased intraocular pressure (16.7%), traumatic cataract (14.0%). Secondary hemorrhage occurred in 15 patients (13.2%) and was significantly more frequent among patients whose initial hyphema filled more than one therd of the anterior chamber. The final visual acuity were worse in eye with associated ocular injuries such as vitreo-retinal disorders, traumatic cataracts, etc.
