2.Clinical Manifestation and Treatment Outcomes of Nasal Cavity & Ethmoid Sinus Squamous Cell Carcinoma.

BACKGROUND AND OBJECTIVES: We aimed to evaluate the clinical manifestation and treatment outcome of the nasal cavity squamous cell carcinoma (SCC) and compared its survival rate with that of the maxillary sinus (MS) SCC. SUBJECTS AND METHOD: Thirty two patients who were diagnosed as nasal cavity SCC or ethmoid sinus SCC between 1984 and 2005 were selected. These patients were referred to as the intranasal (IN) SCC group. Ten prognostic factors were evaluated in terms of survival rate and recurrence. The survival rate of 61 patients who were diagnosed as MS SCC was compared with that of the IN SCC. RESULTS: Chief complaints of the patients with IN SCC were nasal obstruction (40.4%), epistaxis (25.0%), and rhinorrhea (13.5%). The 5 year survival rate of IN SCC was 44.6% and the 5 year disease free survival rate was 27.7%. The most important prognostic factor was T stage (p<.001). The 5 year survival rate of MS SCC was 55.4%, and it was statistically not different from that of the IN SCC (p=.472). In the T1, T2, T3 stages, the 5-year-survival rate of the IN SCC was similar to that of MS SCC (p=.637). But in the T4 stage, IN SCC tended to have a worse prognosis (p=.074). CONCLUSION: The prognosis of IN SCC was similar to that of the MS SCC. However, MS SCCs were detected in a more advanced stage and T4 stage of the IN SCC had a worse prognosis compared to the same stage of MS SCC.

4.Posterior Fossa Decompression for Hypertensive Cerebellar Encephalopathy: Case Report.

Hypertensive encephalopathy is a medical emergency that is usually presented with manifestating headache, nausea, and visual disturbance. The disease predominantly involves the parieto-occipital white matter. Although cerebellar lesion accompanying hydrocephalus has been occasionally described in the literature, medical management has been sufficient for relieving the posterior fossa hypertension. In the present case, we report a patient who was diagnosed with primary hypertensive cerebellar encephalopathy refractory to medical management and who was successfully treated by surgical decompression of the posterior fossa. A pathologic microscopic inspection of the tissue specimen confirmed hypertensive vascular change.

5.Ten years of experience in the prevention of mother-to-child human immunodeficiency virus transmission in a university teaching hospital.

PURPOSE: Administration of antiretroviral drugs to mothers and infants significantly decreases mother-to-child human immunodeficiency virus (HIV) transmission; cesarean sections and discouraging breastfeeding further decreases this risk. The present study confirmed the HIV status of babies born to mothers infected with HIV and describes the characteristics of babies and mothers who received preventive treatment. METHODS: This study retrospectively analyzed medical records of nine infants and their mothers positive for HIV who gave birth at Korea University Ansan Hospital, between June 1, 2003, and May 31, 2013. Maternal parameters, including HIV diagnosis date, CD4+ count, and HIV ribonucleic acid (RNA) copy number, were analyzed. Infant growth and development, HIV RNA copy number, and HIV antigen/antibody test results were analyzed. RESULTS: Eight HIV-positive mothers delivered nine babies; all the infants received antiretroviral therapy. Three (37.5%) and five mothers (62.5%) were administered single- and multidrug therapy, respectively. Intravenous zidovudine was administered to four infants (50%) at birth. Breastfeeding was discouraged for all the infants. All the infants were negative for HIV, although two were lost to follow-up. Third trimester maternal viral copy numbers were less than 1,000 copies/mL with a median CD4+ count of 325/microL (92-729/microL). Among the nine infants, two were preterm (22.2%) and three had low birth weights (33.3%). CONCLUSION: This study concludes that prophylactic antiretroviral therapy, scheduled cesarean section, and prohibition of breastfeeding considerably decrease mother-to-child HIV transmission. Because the number of infants infected via mother-to-child transmission may be increasing, studies in additional regions using more variables are necessary.

6.Two Cases of Combined Pregnancy Following IVF-ET.

Combined intrauterine and ectopic (heterotopic) pregnancy occurs in approximately 1 in 30,000 spontaneous gestations. Heterotopic gestations are increased in women who have had reconstructive pelvic surgery, Pelvic inflammatory disease and artificial ovarian hyperstimulation. Patients who require assisted reproductive technologies, such as in vitro fertilization / embryo transfer and gamete intrafallopian transfer, also have an increased risk of a heterotopic pregnancy. We experienced two cases of combined pregnancy following IVF-ET. Following is a report of these cases with a brief review of references.

7.Clinical Availability of Topographic Auditory Event Related Potentials in Children with Attention Deficit Hyperactivity Disorder.

OBJECTIVES: This study was carried out to investigate the clinical availability of topographic auditory event related potential P300 as a biological marker in children with attention deficit hyperactivity disorder(ADHD). METHODS: The subjects were composed of children with ADHD(n=22:16 boys, 6 girl s:mean age=89.8+/-17.1months) and normal controls(n=20:12 boys, 8 girls:mean age=90.6+/-16.8months). Topographic auditory event related potential was measured by "oddball paradigm", and the latencies and amplitudes of P300 of both groups were determined by Global Field Power Measurement. After 8 weeks treatment with methylphenidate, P300 of ADHD children were also measured. RESULTS: Both P300 latency and amplitude were not different between ADHD children after treatment and normal controls, but significantly different between ADHD children before treatment and normal controls, and between before and after treatment of ADHD children. CONCLUSIONS: The results implicate that the latency and amplitude of P300 may be considered as state markers of ADHD.

8.Nephropexy Using Percutaneous Nephrostomy (PCN).

For the past two years, nephropexy using PCN was underwent in 9 cases of nephroptosis (8 patients) with variable degrees of flank pain. All were female and 6 on right, 1 on left and 1 on bilateral. All patients had a very mobile kidney and exact distance of movement measured on the film was greater than 6cm (6 to 15 cm). After this operation, 14-20 Fr. nephrostomy catheter was indwelled for about two weeks. All patients were followed at least 3 months(3 to 32, median 17 months). We followed the patients with symptom relief and IVP after 3 months. Nephropexy was regarded as success if she remained asymptomatic for more than 3 months. Five cases were successful and four cases felt recurrent flank pain within 1 month after the surgery. Among four cases of recurrence, one had repeated PCN and got successful result, another had open nephropexy and the others have been followed so far So overall success rate was 67% (6/9). In conclusion, nephropexy using PCN is less invasive, needs shorter period of admission and leaves ignorable scars postoperatively. Furthermore it can be repeated even in the case of failure or recurrence, which would make this new technique available as the first step for the surgical treatment of nephroptosis. And the success rate could be increased if the nephrostomy tract is dilated upto 30 Fr. and the PCN catheter is placed about 4 weeks.
