2.Two Cases of Extrapelvic endometriosis following Laparoscopy-assisted vaginal hysterectomy and Cesarean section.

Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

Dislocation involving talus is a rare injury comprising less than 1% of all dislocations of the joint. One case of lateral subtalar di location of the talus treated at the Department of Orthopedic Surgery, Kang Nam General Hospital in Decernber 1985. The patient was followed for 6 months and satisfactory healing was obtained.

4.A Comparative Study of the Clinical Features between Negative Appendectomy and Conservative Treatment Groups in Suspected Appendicitis.

This study is a clinical review of two groups of patients. Group I was composed of 97 patients who were submitted to negative appendectomy under the impression of acute appendicitis and, group II was 45 patients who received conservative treatment with the same impression at the department of surgery, Dae Rim Saint Mary's Hospital, from January 1, 1994 to August 31, 1996.The purpose of this study is to identify possible factors for the decision of operation or conservative treatment in the patients with suspected appendicitis and so to decrease unnecessary operation. Clinical characteristics of the patients were compared between two groups by the method of comparison of means(X test). The following results were obtained; 1) The incidences of group I and group II among the patients in the state of suspicious appendicitis were 97 of 1,004 cases(9.7%) and 45 of 1,004 cases(4.5%) respectively. 2) The number of female patients was 1.5 times more than male in both groups(p<0.05). 3) In group I the number of patients under 30 years of age was more than that of patients above 30 years of age(p<0.05) and in group II the highest incidence was shown in the patients in forth decades. 4) Symptoms over 3 days were more frequent in group I(31%) and in group II those within 12 hours were prominent(42.2%). 5) Gastrointestinal symptoms of anorexia, nausea and vomiting were more frequent in group I than group II. 6) Body temperature over 37.5 degrees C appeared in 15% of patients in group I and 11% in group II, but this difference showed no statistical significance and there was no sex difference. 7) RLQ rebound tenderness was manifested in 60% of patients in group I and 37.7% in group II, this difference has statistical significance(p<0.05). Migration pain to RLQ appeared in 12% of patients in group I aand 17.7% in group II(p>0.05) 8) Leukocytosis above 10,000/mm3 appeared in 44% of patients in group I and 53.3% in group II(p>0.05). 9) In admission route, the patients via ER was 2 times more than those of OPD in group I and in group II that was 5 times more than this.

5.Clinical Availability of Topographic Auditory Event Related Potentials in Children with Attention Deficit Hyperactivity Disorder.

OBJECTIVES: This study was carried out to investigate the clinical availability of topographic auditory event related potential P300 as a biological marker in children with attention deficit hyperactivity disorder(ADHD). METHODS: The subjects were composed of children with ADHD(n=22:16 boys, 6 girl s:mean age=89.8+/-17.1months) and normal controls(n=20:12 boys, 8 girls:mean age=90.6+/-16.8months). Topographic auditory event related potential was measured by "oddball paradigm", and the latencies and amplitudes of P300 of both groups were determined by Global Field Power Measurement. After 8 weeks treatment with methylphenidate, P300 of ADHD children were also measured. RESULTS: Both P300 latency and amplitude were not different between ADHD children after treatment and normal controls, but significantly different between ADHD children before treatment and normal controls, and between before and after treatment of ADHD children. CONCLUSIONS: The results implicate that the latency and amplitude of P300 may be considered as state markers of ADHD.

6.An Experimental Model of Isolated Lung Block for Evaluation of Pulmonary Preservation after Ischemia.

During the last 30 years, major organ transplantation has become popular, even in Korea, such as kidney, liver, etc. After the successful clinical cardiac transplantation in Korea, many cases of cardiac transplantation are being performed in some centers. But lung transplantation has a lot of obstacles, especially donor shortage and decreased tolerability of the lung to ischemia-reperfusion injury. Usually it was considered that the maximum safety margin of ischemic time in lung transplantation was about 4 to 6 hours. So, many investigators have tried to develop better preservation methods and experimental model for evaluation of effectiveness in those various methods. But most of those methods had several drawbacks in clinical and experimental settings. So we developed an easily-controllable, reliable, and inexpensive experimental model of isolated rabbit lung block. Using these model, we evaluated its effectiveness and reliability for the experiment of ischemia-reperfusion injury in lung transplantation.

7.Pulmonary Sarcoidosis: CT Findings and Correlation with sACE Level and PFT.

PURPOSE: To assess CT findings of pulmonary sarcoidosis and correlate these with sACE level and PFT. MATERIALS AND METHODS: Between 1989 and 1995, 14 patients (4 men and 10 women, aged between 28 and 55 years) with histologically confirmed pulmonary sarcoidosis were consecutively selected. HRCT scans were performed in 12 patients and conventional CT scans in two. CT findings were reviewed by three radiologists, and were correlated with the index of disease activity based on sACE level and pulmonary function test. RESULTS: Pulmonary parenchymal abnormalities were seen in all patients ; small nodules of less than 3 mm in diameter were seen in eight. Other abnormalities were nodules of more than 3 mm in diameter (n=7), confluent nodules (n=5), ground glass opacity (n=5), patchy areas of consolidation with air bronchogram (n=5), and architectural distortion (n=3). The upper lung zone was more frequently involved than the middle or lower zone. In ten patients, the peripheral interstitum was predominantly involved, while only three patients showed predominant peribronchovascular involvement. Lymphadenopathy was noted in 13. There was no correlation between sACE level, the results of a pulmonary function test and the extent of parenchymal involvement. CONCLUSION: HRCT is valuable for the identification, characterization, and determination of the extent to which parenchymal lung is involved in sarcoidosis. The extent of this involvement does not correlate with sACE level and pulmonary function test results.

8.Localized Single-Voxel Spin-Echo Proton MR Spectroscopy of Normal Hippocampal Area at 1.5 T : Optimal Voxel Volume and Reproducibility of Metabolite Ratios.

PURPOSE: To determine the optimal voxel volume covering the hippocampal area in single-voxel proton magnetic resonance (MR) spectroscopy and to evaluate the reproducibility of metabolite ratios of the spectra. MATERIALS AND METHODS: Localized single-voxel proton MR spectroscopy was applied to the right hippocampal area of five healthy volunteers at 1.5 T (Siemens Vision), using a standard head coil, and we employed the spin-echo or point resolved spectroscopy sequence. Voxel volume was changed from 1ml to 5ml but other operator-dependent measurement parameters were fixed, as follows: repetition time/echo time=1500/135 msec, number of scans=300. Using the same voxel volume, five consecutive measurements were obtained in each subject. Signal to noise ratio (SNR) of N-acetylaspartate (NAA), NAA / Choline containing compounds (Cho) and NAA / Cr (creatine and phosphocreatine) +Cho ratios were calculated for all 25 spectra. RESULTS: The SNR of NAA peaks increased significantly as voxel volume was increased to 3 ml (p < 0.01, Kruskal-Wallis test and Wilcoxon rank sum test). There were, however, no significant differences in values among the spectra obtained with voxel volumes of 3, 4 and 5 ml (p > 0.1) ; in those obtained with voxel volume of 2-4ml, the standard deviations of NAA / Cho (10.6-13.2% of mean values) were similar to those of NAA /Cr+Cho (8.5-13.2% of mean values). CONCLUSION: For spin-echo proton MR spectroscopy of the hippocampal area, the optimal voxel volume may be more than 3 ml in a setting of TR/TE=1500/135 msec and number of scans=300. In this situation, standard deviations of metabolite ratios may reach about 8-13% of mean values.

9.The Effect of Gingko Biloba Extract on Energy Metabolic Status in C3H Mouse Fibrosarcoma: Evaluated by in vivo 31P Magnetic Resonance Spectroscopy.

PURPOSE: Gingko biloba extract (GBE), a natural product extracted from Gingko leaves, is known to increase the radiosensitivity of tumors. This radiosensitization probably arises from the increase in the peripheral blood flow by decreasing the blood viscosity and relaxing the vasospasm. The influence of a GBE on the metabolic status in fibrosarcoma II (FSaII) of a C3H mouse was investigated using 31P magnetic resonance spectroscopy (MRS). MATERIALS AND METHODS: Eighteen C3H mice with fibrosarcoma II (from 100 mm3 to 130 mm3) were prepared for this experiment. The mice were divided into 2 groups; one (9 mice) without a priming dose, and the other (9 mice) with a priming dose of GBE. The GBE priming dose (100 mg/kg) was administered by an intraperitoneal (i.p.) injection 24 hours prior to the measurement. First 31P MRS spectra were measured in the mice from each group as a baseline and test dose of GBE (100 mg/kg) was then administered to each group. One hour later, the 31P MRS spectra were measured again to evaluate the change in the energy metabolic status. RESULTS: In the group without the priming dose, the mean pH, PCr/Pi, PME/ATP, Pi/ATP, PCr/(Pi+ME) values 1 hour after the test dose were not changed significantly compared to the values at the baseline. However, in the group with the priming dose, the mean PCr/Pi, Pi/ATP, PCr/(Pi+PME) values 1 hour after the test dose changed from the baseline values of 0.49, 0.77, 0.17 to 0.74, 0.57, 0.28 respectively. According to the paired t-test, the differences were statistically significant. CONCLUSION: The above findings suggest that the metabolic status is significantly improved after administering GBE if the priming dose is given 24 hours earlier. This shows that the radiosensitizing effect of GBE is based on the increase of tumor blood flow and the improvement in the metabolic status.

The neurosurgical frequency of multiple intracranial meningioma is low. The authors report a case of multiple meningiomas of supratentorial region and posterior fossa. The patient was 57-year-old female and presented with severe headache and vomiting. Computerized tomography and magnetic resonance imaging revealed multiple meningiomas in right parasagittal region. right temporal region and posterior fossa. The patient was treated successfully by stage operation and all of these pathological types was meningotheliomatous meningioma.
