2.Clinical Efficacy of Dermis-Fat Graft vs. Posterior Tenon's Capsule Suturing in Anophthalmic Orbit.

The anatomic and physiologic changes of the anopthalmic orbit affect cosmetic appearance of the patient and function of the socket and the prosthesis. During the past three years, the authors studied 13 cases of dermis-fat graft and 27 cases of posterior Tenon's capsule suturing with a plastic implant, and compared the post'operative effects of the two procedures. Dermis-fat graft produced good fornix formation with no remarkable fat atrophy after both primary and secondary proceddure. Posterior Tenon's capsule suture technique showes good effect to keep implant from migration or extrusion. Size of the plastic implant and fixation of the extraocular muscles were important factors also for good cosmesis and implant motility. There revealed no significant difference between the two procedures in EOM motility, enophthalmos, deep superior sulcus deformity, implant migration or prolapse etc.
3.A Clinical Study of Ankle Fractures

The authors have reviewed 71 cases (70 patients) of the ankle fractures that were treated through the admission in Orthopedic Department, Paik Foundation Hospital, from June 1972 to April 1978. The average duration of the follow-up waa 15 months. The summary of the results were as follows; 1. Of the 71 cases, male was 54 cases and female 17 cases. The average age of the patients was 32. 2. The main cause of the injuries was traffic accident, and the other causes were sliding down and falling from a height, industrial accident, and sports injury in order. 3. By the classification of Lauge-Hansen, the pronation-external rotation and,supination-adduction types were commoner than the other types. 4. Open reduction was performed in 48 cases, and closed reduction in 23 cases. 5. The average duration of the cast immobilization after the closed reduction was 8 weeks, and that of the open reduction was 6 weeks. 6. The result of the treatment was better in those cases of the open reduction than that of, the closed reduction. 7. The good clinical results could be achieved by obtaining the anatomical position of the talus in the mortise and the reduction was achieved more satisfactorily when the key role of the lateral malleolus in complete reduction was well understood.

4.Tear Drainage through the Upper Can,liculus in Case of Lower Canalicular ObstruCtion.

Symptomatic epiphora has been rarely observed in patients jwith only one canaliculus obstruction. The authors experienced 7 cases of only lower canahculus obstruction without epiphora. Tear drainge of these eyes was assessed using dacryoscintigraphy and compared with the comtrol eyes. The measurement of T1/2 valuse, the duration of which a given amount of 99m-Tc-phytate drops to half in the conjunctival fornix was performed. 5.Clinical Experience of Lazy-T Operation.

Five Lazy-T procedures for medial ectropion of the lower lid were performed in 4 patients(3 males, 1 female). Of these 5 procedures, one was for involutional ectropion and 4 were for cicatrical ectropion which occurred after lid surgery or were due to facial xerosis. Ages ranged from 55 to 68 years. After the follow-up period of 8 to 29 months, all cases showed marked improvement of tearing, ocular injection, and discharge without any recurrence or overcorrection.

6.Retinal Pigment Epithelial Detachment in Posterior Scleritis.

Posterior scleritis must be one of the most underdiagnosed treatable conditions in ophthalmology, partly because its manifestations are so protean and partly because the diagnosis is rarely considered. Although ultrasonography and computer tomogram are ancilliary tests, a careful examination of the posterior segment of the eye including the area of the ora serrata, macula and disc is essential to discover the presence of a posterior scleritis. We experienced prolonged retinal pigment epithelial detachments which had appered at early phase of posterior scleritis.

7.Clinical Evaluations After Cataract Operation.

It is hard to advice for fitting contact lens in the case of monocular lens extraction in rural area because of exposure to dust laden air and elderly people in most cases. Questionares for comfortability, Side effects and the method of visual correction were obtained in 200 cases of cataract operation in rural area. Most of the advanced age group replyed that the unsatisfactory and difficulty for fitting contact lens, therefore it is desirable method to obtain good adaptation and binocualr vision with P-A (Phakic and Aphakic) spectacles by intensive training.

10.Structural Factors Influencing the Quality Management Activities in Nursing Homes.

PURPOSE: Nursing home quality indicators have been focused widely on result outcomes, not for the environment in that quality of service are delivered. This study aimed to examine structural factors influencing quality management activities in nursing homes. METHOD: Sample was 170 nursing homes responded to the survey questionnaire which was distributed to the 543 nursing homes nation-wide. Data were collected on structural characteristics, types of services, and quality management activities. Data were analyzed with the descriptive statistics, Pearson correlations, and multiple regression. RESULT: Most of the nursing homes were operated as free of charge by the social welfare ownership. Average number of residents was 52.1 with severe and mild dementia and bedridden status. In terms of quality management activities, 34% of the sample had CQI committee that focused their activities on services delivery process, performance appraisal, record keeping regularly. 30.6% of quality management activities were accounted for by the number of residents with dementia, the ratio of RN to residents, rehabilitation services, and social wefare services in nursing homes. CONCLUSION: We recommend that more comprehensive quality management activities should be developed as process quality indicators in conjunction with the outcome indicators.
