1.A Case of 5 alpha-reductase Deficiency in Infancy.

5 alpha-reductase deficiency resulting in male pseudohermaphroditism is a rare disease characterized by clitoral-like phallus, bifid scrotum, urogenital sinus, testis cited in labioscrotal folds. Evaluation of plasma T/DHT ratios in infancy, particularly after hCG stimulation of the testes and elevated urinary tetrahydrocortisol (THF) to 5 alpha-tetrahydrocortisol(5 alpha-THF) ratios provide a valuable dianostic test for 5 alpha-reductase deficiency. We report one case of 5 alpha-reductase deficiency who were presented with ambiguous genitalia and elevated T/DHT ratio before and after hCG stimulation.

3.Mandibular Angle Contouring at the Time of Face Lift.

Many people want to receive facial rejuvenation with surgical treatment, but the post operative result is not always satisfying following classic soft tissue surgery. The reasons for dissatisfaction are many. We were particularly interested in the skeletal disharmony which is frequent in orientals. Since many orientals have flat and quadrangular facial contours, they often don't obtain the harmony of the facial skeleton and soft tissue despite successful surgery. So, to correct facial contour, mandibular angle contouring can be performed at the time of face lift. Seven patients had mandibular angle resection performed during facial rhytidectomy from October of 1996 to October of 1998 at Kangbuk Samsung Hospital, and they were followed from 4 months to 24 months. At the time of face lift, we tried the retromandibular approach for mandibular angle resection when it was indicated, instead of the intraoral approach which does not leave a noticeable scar and does not have the risk of facial nerve injury. Because of a restricted operative field, the intraoral approach has the disadvantages of a long operation time and a high risk of complications. The retromandibular approach with rhytidectomy incision showed neither noticeable scar nor developed a facial nerve injury. In conclusion, we found that mandibular angle contouring by retromandibular approach is easy and the result is more normal with a shorter recovery time. There were no complications and aesthetic results were quite satisfactory.

4.A Morphometric Study on the Changes in the Size of Human Oviduct and Ovary after Immersion in Formalin.

The changes in the size of human oviduct and ovary after immersion in formalin (4%, 10%) were investigated. The results were as follows : 1. After fixation, the length of oviduct was significantly reduced to 93%, but the cross sectional area of oviduct was significantly increased. The length of oviduct showed no significant difference between 4% and 10% formalin. The length of ovarian ligament was also significantly reduced to 84% after fixation. 2. After fixation, the volume of oviduct was increased significantly and showed a difference between the two fixatives. The volume of oviduct in 4% formalin has increased to 107%, as compared to 103% increase in 10% formalin. 3. After fixation, the volume of ovary was increased significantly and showed a difference between the two fixatives. The volume of ovary in 4% formalin has increased to 109%, as compared to 103% increase in 10% formalin.

5.Nesidioblastosis in Neonate with Persistent Hyperinsulinemic Hypoglycemia.

Nesidioblastosis, also known as persistent hyperinsulinemic hypoglycemia of infancy(PHHI) or familial hyperinsulinsm, is the most common cause of recurrent severe hypoglycemia in infancy. It is an autosomal recessive disorder characterized by irregular insulin secretion leading to inappropriately raised plasma insulin concentration compared to blood glucose levels. Recently, mutations in the sulfonylurea receptor(SUR) have been described in association with PHHI. The mainstay of medical treatment is glucose infusion and diazoxide or long acting somatostatin. If medical treatment fails in preventing hypoglycemia, near total pancreatectomy is recommended. We report one case of nesidioblastosis cured by near total pancreatectomy with brief review of literatures.

7.Comparative Study on the Allergy Skin Test and RAST in Allergic Patients.

The allergic skin test and RAST are used commonly to demonstrate the presence of skin sensitizing IgE antibody in the diagnosis of allergic disease. To evaluate the agreement rate and correlation between results of skin test and RAST (radioallergosorbent test) in allergic subject, 83 allergic childhood with positive skin test (bronchial asthma: 26, allergic rhinitis: 27, atopic dermatitis and urticaria: 6 bronchial asthma and allergic rhinitis: 24) and 16 nonallergic childhood were studied. The results were as follows. 1) Peripheral eosinophil counts were significantly elevated in groups with bronchial asthma, allergic rhinitis, bronchial asthma & allergic rhinitis except atopic dermatitis & urticaria group in comparison with normal control group. 2) Serum IgE levels were significantly higher in groups with bronchial asthma, allergic rhinitis, bronchial asthma & allergic rhinitis than in normal control group except atopic dermatitis & urticaria group but there was no significant differences among study groups. 3) RAST units of D.p and D.f were significantly higher in study group except atopic dermatitis & urticaria group. RAST units of cockroach, however, were not significantly increased but in D.f, combined group of bronchial asthma & allergic rhinitis was significantly higher than bronchial asthma or allergic rhinits (p<0.05). 4) Each agreement rate between 3+ skin test and positive rate of RAST in D.p, D.f, cockroach was 37.5%, 63.6% and 12.5% respectively and 4+ skin test and positive rate of RAST was 84.0%, 89.1% and 38.4%. 5) Correlation coefficient between skin test and RAST was 0.71 in D.P, 0.59 in D.f and 0.23 in cockroach. In conclusion, there was difference in agreement rate between positive skin test and RAST to each allergen. Agreement rate between positive skin test and RAST was higher in D.p and D.f. These results suggest that RAST can be used simultaneously with skin test for detecting allergens.

The globus symptom is a condition in which a patient, often middle aged women, complains of a lump andchocking sensation in the throat, Functional disturbance of the cricopharyngeal muscle, rendering it incapable ofrelaxing during swallowing, has long been recognized as a cause of globus symptom and dysphagia. We wanted to findout how often and to what extent distrubed relaxation of the cricopharyngeal muscle can be seen in patients withglobus symptom with routine examination and video esophagogram. The results were as follows: 1. Male : femaleratio was 1:2.4. 2. Globus symptom was most frequent in the age group between 30-39 of female. 3. Organic lesionswere seen in 43.6%(24 Pts) of globus patients. 4. Cricopharyngeal muscle was visualized in 29.1%(16Pts), esophageal diverticulum in 3.6%(2 Pts) and degenerative spondylosis in 3.6%(2 Pts). 5. Incidence of visualizationof cricopharyngeal muscle were higher in male group (50%) than female one (20.5%). 6. Cricopharyngeal muscle wasmost frequently visualized on early swallowing phase(12/16 Pts).

9.A Case of Prurigo Pigmentosa.

Prurigo pigmentosa is a chronic pruritic inflammatory dermaoss characterized by erythematous papules in a reticulated pattern that resolve leaving a reticulated, mottled hyperpigmentation. Most cases have been reported from Japan. Two cases of prurigo pigmentosa have been reported in Korean women. We experienced a casc of prurigo pigmentosa in a Korean man of 20 years of age. Histopathological findings of reddish papule showed exocytosis, sponsiosis, intraepidermal vesicles, hydropic degeneration of basal cells in the epidermis, and hyphohistiocytic infiltration in the upper dermis. Direct irnmunofluorescence was negetive. Therapy with dapsone, 50 mg given daily, resulted in a rerinable regression of the reddish papules. The daily dose of clapsone was reduced to 25mg; however, no new reddish, pruritic papules have appeared.
