1.Usefulness of Three - Dimensional CT Angiography as a Confirmatory Diagnostic Test for Scalp Pseudoaneurysms.

OBJECTIVE: We report here on two cases of pseudoaneurysms that were diagnosed using computed tomographic angiography (CTA), with one arising in the superficial temporal artery and the other arising in the occipital artery. METHODS: Case 1: A 29-year-old man presented with a slow growing, pulsatile mass on the left side of his forehead following a blow from a fist 10 days earlier. CTA revealed an 11 x 8 x 11-mm oval pseudoaneurysm arising from the frontal branch of the left superficial temporal artery. Case 2 : A 36-year-old man presented with a pulsatile mass in the right suboccipital region that had been present for the past year. He had no history of direct trauma. CTA revealed a 15 x 8 x 13-mm oval dye-filled sac arising from the right occipital artery. RESULTS: Based on the CTA images, the parent arteries were exposed and ligated proximally and distally to the pseudoaneurysms and then they were completely excised. CONCLUSION: CTA is a minimally invasive technique for making the diagnosis of scalp pseudoaneurysms and CTA is also useful for the preoperative planning.

6.A Rare Case in the Pattern of the Origin of the Celiac Artery.

A rare case in the pattern of the origin of the celiac artery was observed in the cadaver of a 64 year old Korean male and the results are summarized as follows. The superior mesenteric artery originated from the abdominal aorta at the level of the intervertebral dusk between first and second lumbar vertebra. At a point 4cm from the origin of the superior mesenteric arterr, a common trunk appeared and ran 1cm and then divided into the common hepatic artery and the splenic artery. The common hepatic artery divided into the right gastric artery and the proper hepatic artery at a point 4.5cm after the bifurcation. A common trunk formed by the left inferior phrenic artery and the lefts gastric artery arose from the abdominal aorta about 0.8cm about the origin of the celiacomesenteric trunk.

7.The Frequency and Clinical Observation of Ota's Nevus.

The clinical observation was done on 126 patients of Ota's nevus, 113 of whom were seen at Department of Dermatology, Chonnam National University Hospital from January 1973 through July 1983, and 13 of whom were found among 45, 87l male and female students of high school in Kwangju City, Korea. The results obtained were as follows: 1. The incidence of male and female students of Ota's nevus were respectively 0. 013% and 0.043%, with the incidence,as a whole being 0.03%, whereas the frequency of Ota's nevus among the outpatients was 0. 24% with the ratio of male to female being 1: 2. 67. 2. There was only one family of familial occurrence, in which 2 sisters were involved among 5 siblings. 3. Thirty three percent of Ota's nevus showed the onset at birth. 4. According to Tanino's classification, our patients showed that Type II was 41.7% and then Type Ia, III, Ib, and IV being respectively 33. 3%, 16. 7, 4.2, and 4. 2%. 5. Seleral involvement was 54.2%, especially in type III and Type II. 6. The color was increased in 54%, especially Type II by internal and external factors such as fatigue, warm temperature, menstruation and cold temperature. 7. The color intensification was prominent at the age of 10 to 14.

9.Leiomyosarcoma of the Skin: Report of A Case.

Leiomyosarcoma arising in the skin is rare tumor, and diagnosis usually is made microscopically. After local excision, these lesions recur in large proportion of pat ients. The authors herein report a 53-year-old male with leiomyosarcoma appeared in the skin of the right forearm and presenting as a dark reddish colored, 5*6cm in diameter, superficial ulcerated single firm nodule with intermittent pain. Histopathological examination showed poorly circumscribed tumor consisting of interlacing bundles of spindle shaped smooth muscle cells in the middle and lower parts of the dermis. The nuclei were hyperchromatic, large, vacuolated, and irregular in shape. Electron microscopic findings revealed cytoplasmic organelles such as rough endoplasmic reticulum and mitochondria of malignant smooth muscle cells in the paranuclear area, Characteristic subsarcoelmmal caveolae and dense plaque were noted and myofilaments were distributed in the peripheral cytoplasm. The tumor did not recur in 10 months' follow-up.

10.Binding of Lectins to the Zona Pellucida on Sperm-oocytes Interaction in the Pig.

OBJECTIVE: Lectins are cell-agglutinating and sugar specific proteins or glycoproteins of non-immune origin that precipitate glycoconjugates having saccharides of appropriate complementarity. Because of these properties, plant lectins have been used to help characterize the carbohydrate moieties of glycoproteins in the zona pellucida (ZP) of several mammalian species including pigs. Treatment of oocytes with various lectins blocks sperm binding to the ZP in various mammalian species. This study was undertaken to examine the distribution of sugar residues in the ZP of pig oocytes matured in vitro and the ability of spermatozoa to bind to ZP and in vitro penetration in oocytes treated with fluorescein isothiocyanate (FITC)-labelled lectins. MATERIALS AND METHODS: The lectins of Banderiaea simplicifolia (BS-II, bind to beta-D-Nacetylglucosamine), Canavalin ensiformis (Con A, bind to alpha-D-Mannose), Lens culinaris (LCA, bind to alpha-D-Mannose), Ricinus communis (RCA-I, bind to beta-D-Galactose) and Ulex europaeus (UEA-I, bind to alpha-L-Fucose) were examined for spermatozoa penetration, binding capacity to ZP and distribution of lectins. RESULTS: The penetration rates were significantry (p<0.05) higher in control oocytes (63%) than those treated with all lectins, but penetration rates (40~49%) were simililar in group treated with lectins. The incidence of monospermy was similar in oocytes untreated and UEA-I, but it was higher in oocytes treated with BS-II, Con A, RCA-I and LCA. The porcine oocytes cultured for 48 h in TC-199 medium were freed from cumulus cells and treated for 30 min with fluorescein isothiocyanate-labelled lectins. When examined under fluorescein illumination, higher (p<0.001) proportions of oocytes showed fluorescein of zona pellucida after treatment with Con A (93%), LCA (93%) and RCA-I (100%) than BS-II (37%) and UEA-I (50%). All of the oocytes treated with RCA-I exhibited strong fluorescein in the outer region of the zona pellucida while those treated with LCA exhibited strong fluorescein throughout the zona pellucida. BS-II bounded mainly to the outer region and UEA-I bounded mainly to the inner region of the zona pellucida, with either strong or weak fluorescein. At 120 min after insemination in vitro, fewer spermatozoa were bound to the zona pellucida of the oocytes treated with BS-II, Con-A and RCA-I. Of the lectins, Con A most inhibited sperm binding. CONCLUSIONS: These results suggest that beta-D-Galactose residues in the porcine zona pellucida may act as primary sperm receptors and inducers of the sperm acrosome reaction and these sugar residues may be involved in the block to polyspermy.
