1.The Efficacy of Cervicography Combined with Pap Smear in Cervical Cancer Screening.
Korean Journal of Gynecologic Oncology and Colposcopy 1999;10(1):13-23
Cervicography is used commonly in cervical cancer screening with a Pap smear. Many reports shows that the combination of these two methods is more accurate and helpful in cervical cancer screening. OBJECTIVE: The purpose of this investigation was to evaluate the efficacy of conjoined methods in cervical cancer screening and to evaluate any difference between health care center and out-patient treatment. METHODS: Pap smear and cervicogram data were obtained for 699 patients who visited the Department of Obstetrics and Gynecology, Chungnam National University Hospital from January 1997 to December 1997. Of these patients, 551 patients visited the health care center and 148 were out-patients. We exmined all pertinent information, including biopsy results, for patients with detected abnormalities. A total of 62 patients received a biopsy. RESULTS: 1. Results for Papanicolaou smear were: sensitivity, 81.8%, specificity, 98.0%, positive predictive value, 83.7%, negative predictive value, 98.8%, false negative rate, 18.2%, and false positive rate, 2.0%. 2. Results for cervicography were: sensitivity, 86.4%, specificity, 97.8%, positive predictive value, 73.1%, negative predictive value, 99,1%, false negative rate, 13.6%, and false positive rate, 2.2%. 3. Results for the conjoined method were: sensitivity, 97.7%, specificity, 97.2%, positive predictive value, 70.5%, negative predictive value, 99.8%, false negative rate, 2,3%, and false positive rate, 2.8%. 4. When cervicography and Papanicolaou smear were used conjointly The sensitivity, specificity, negative predictive value, false negative rate, and false positive rate were statistically significant for Papanicolaou smear (P<0.05). 5. There was no statistical significance difference between cervicography and the conjoined method. 6. Compared with out-patients, Health care center patients showed increased diagnostic error and failure to follow-up. CONCLUSION: When cervicography and Papanicolaou smear were used conjointly, cervical cancer detection rates were increased.

The Journal of the Korean Orthopaedic Association 1988;23(2):542-548
Transient synovitis of the hip in children is a non-specific inflammatory and self-limited condition. It is also the most common cause of painful hip in children under ten years of age. Despite of the benign prognosis, there are many difficulties in distinguishing it from other diseases of the hip joint. It remains a common diagnostic problem for clinician because the clinical symptoms, physical findings, and conventional radiography is not pathognomonic of the condition. The authors paid attention to the increase of effusion in the affected hip and studied the value of the hip ultrasonography in 24 cases of transient synovitis from August 1985 to July 1987. The results are summerized as follows : 1. The ratio of male to female was 7 to 1, average age was 7.9 yrs, average hospitalization period was 4.5 days. 2. In simple X-ray studies, no bony change was detectable except for soft tissue signs in 68% of the cases. 3. Capsule-to-bone distance in sagittal ultrasonographic section revealed abnormal increase in 87.5% of the affected hip. 4. Average capsule-to-bone distance of affected hip joint was 7.50 mm, while that of the normal hip was 4.26 mm. 5. As shown in the above studies, ultrasonography can be considered good noninvasive technique in detection and follow-up of hip effusion. So, it is thought to be a valuable method in the diagnosis of transient synovitis of the hip in children.

Sleep Medicine and Psychophysiology 2014;21(1):5-13
OBJECTIVES: Depression, sleep complaints and cognitive impairments are commonly observed in the elderly. Elderly subjects with depressive symptoms have been found to show both poor cognitive performances and sleep disturbances. However, the relationship between sleep complaints and cognitive dysfunction in elderly depression is not clear. The aim of this study is to identify the association between sleep disturbances and cognitive decline in late-life depression. METHODS: A total of 282 elderly people who underwent nocturnal polysomnography in a sleep laboratory were enrolled in the study. The Korean version of the Neuropsychological Assessment Battery developed by the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) was applied to evaluate cognitive function. Depressive symptoms were assessed with the geriatric depression scale (GDS) and subjective sleep quality was measured using the Pittsburg sleep quality index (PSQI). RESULTS: The control group (GDS< or =9) when compared with mild (10< or =GDS< or =16) and severe (17< or =GDS) depression groups, had significantly different scores in the Trail making test part B (TMT-B), Benton visual retention test part A (BVRT-A), and Stroop color and word test (SCWT)(all tests p<0.05). The PSQI score, REM sleep duration, apnea-hypopnea index and oxygen desaturation index were significantly different across the three groups (all indices, p<0.05). A stepwise multiple regression model showed that educational level, age and GDS score were predictive for both TMT-B time (adjusted R2=35.6%, p<0.001) and BVRT-A score (adjusted R2=28.3%, p<0.001). SCWT score was predicted by educational level, age, apnea-hypopnea index (AHI) and GDS score (adjusted R2=20.6%, p<0.001). Poor sleep quality and sleep structure alterations observed in depression did not have any significant effects on cognitive deterioration. CONCLUSION: Older adults with depressive symptoms showed mild sleep alterations and poor cognitive performances. However, we found no association between sleep disturbances (except sleep apnea) and cognitive difficulties in elderly subjects with depressive symptoms. It is possible that the impact of sleep disruptions on cognitive abilities was hindered by the confounding effect of age, education and depressive symptoms.

The Journal of the Korean Orthopaedic Association 1984;19(5):899-908
Acetabular fractures are usually caused by violent external force such as car clash and commonly accompany injuries of the other parts of the body. And there has been long controversy between conservative and operative treatments. A clinical analysis was performed on the 29 patients with acetabular fractures, who had been treated at seoul National University Hospital from 1973 to 1982, mainly focussed on the result of treatment. Followings are the summary of the result. 1. Conservative treatment was done in 52% and operative treatment in 48%. 2. Mean follow-up period was 1 year and 8 months. 3. The result of treatment was good or fair in 53% of conservative treatment group comparedto 71 in operative treatment group. 4. Primary total hip replacement was done in one case of bursting fracture combined with fracture of femur neck and the result was good. 5. Superior and bursting fractures showed the worst result(poor in 83%). 6. Degenerative arthritis with or without avascular necrosis was the most common complication (45 %) and it was more common in conservative treatment group (60 vs. 29%). 7. From above results it could be said that operative treatment in acetabular fractures, if carefully done with reasonable indications, would improve the result and decrease the rate of degenerative arthritis.

Yeungnam University Journal of Medicine 1986;3(1):179-183
The frozen section technique is a means of intraoperative pathological diagnosis, and a procedure of great value to the surgeon. This method should be accurate, rapid and reliable. This method serves useful purposes, such as determining the presence of tumor, its type (especially whether it is benign or malignant), the adequacy of a biopsy of a suspected lesion, and the conditions of the surgical margins. But, it bears many disadvantages, the most of which is the danger of incorrect diagnosis. We studied the indications, the limitations, and the accuracy of the frozen section method and the materials studies was total of frozen section diagnosis of 809 cases was 98.1% with 0.5% of false negative, 0% of false positive 0.5% of incorrect histological diagnosis or grading errors, and 0.9% of deferred cases. The tissues submitted were lymph node, gastrointestinal tract, skin subcutaneous tissues in decreasing oder of frequency. The false positive case is not present, while the false negative cases were 4.

Journal of the Korean Radiological Society 1983;19(4):882-890
The accurate diagnosis of spine trauma is essential to its proper management, since therapeutic decisionsdepend on radiography and clinical data. Failure to recognize significant injury to the spine can lead to severeneurological deficit in the previously neulogically intact patient. The developmemt of CT has open a new demensionin evaluation of spinal column. In our experience CT not only offer the accurate and thorough evaluation of spinalinjury, but does so in a rapid and more efficient manner when compared with conventional radiolgraphy. CT hasbecome the diagnostic procedure of choice when screening plain film and clinical examination indecate that acomprehensive radiographic evaluation is necessary. Eighteen patients with thoracic and lumber spinal fracturewere studied with CT. Four had multiple level injuries. The resuls are summerized as follow; 1. Among the 18patients, 4 had multiple level injuries and other 14 patients had single spinal injury. 2. 8 patients (11 spines)had simple compression fracture and 12 patients (13 spines) had burst fracture of vertebral body. 3. 15 spinesamong the 24 involved spines are located at T12 and L1 level. 4. Spineal canal narrowing and bony fragment in thecanal are defiend only 7 of 13 spines (53.8%) of burst fracture in conventional radiography. However CT showed inall spines of burst fracture. 5. Spinal posterior element involvement is suggested only one of 12 spines of burstfracture, but correctly interpretated by CT in 7 spines(11 anatomical position).

Journal of the Korean Radiological Society 1983;19(4):767-775
Conventional radiology of the pancreas are too often unsatisfactory. It is well known that the whole body CTis very useful in identifying retroperitoneal pathology. The authors intended to present normal pancreaticmorphology and data for preparation of basis for interpretation o abnormalities. We results were as follows; 1.There were 36 male and 24 female patients, and their ages ranged fro 7 to 78 years. 2. 1) The organs adjacentpancreas were stomach, inferior vena cava, duodenum, caudate lobe of the liver left kidney, left adrenal gland, superior mesenteric vessels, spleen. 2) In 19 patients, pancreatic tail at the level of left kidney in thetransverse plane, it was either ventral in 13(68%), ventromedial in 2(19%), ventrolateral in 4(21%) to leftkidney, in the other ventral in 13(68%), ventromedial in 2(19%), ventrolateral in 4(21%) ot left kidney, in theother 41 patients, it was cranial to the upper pole of left kidney, ventral in 15(61%), ventromedial in 1(2%),ventrolateral in 15(37%). 3) Pancreatic tail was cranial to the pancreatic body, 3cm cranial in 2(4%), 2-3cm in5(8%), 1-2cm in 6(10%), less than 1cm in 11(18%). In the other, caudal in 3(5%). 3. Pancreatic shape was uniformtapering form in 37(62%), lobulated form in 23(38%). 4. Pancreatic orientation was horizontal in 13(22%), vertical56(76%), S-shaped in 1(2%). 5. Pancreatic margin was smooth in 22(37%), lobulated in 38(63%). 6. In most patients, pancrease was uniform in density. 7. Pancreatic size was 0.5+-0.1 in measurement ratio of the head in 48(80%),0.4+-0.1 of the body in 49(88%), 0.5+-0.1 of the tail in 47(78%).
