1.Clinical Observation on Penis Carcinoma.

A clinical observation was made on 16 patients of penis carcinoma admitted to the Department of Urology, Catholic Medical College Hospital during the period from August, 1972 to July, 1982 and the following results were obtained. 1. The incidence of penis carcinoma was 0.4% of total inpatients of urologic department and 3.9% of genitourinary cancer. 2. Age distribution was from 43 to 78, showing highest incidence at 50-59 years. 3. Clinical findings were; mass 56%, Ulcer 44% and inguinal lymphadenopathy 44%. 4. Treatment were partial amputation in 18.8%, partial amputation with lymph node dissection in 43.8% and total amputation with lymph node dissection in 25%. 5. Of 11 cases in whom lymph node dissection were performed, 4 cases were positive for metastasis. Of 7 cases with palpable lymph nodes, 3 cases were positive for metastasis. In 4 cases having lymph node dissection when lymph nodes were not palpable, 1 case was positive for metastasis.

2.Lichtenstein's Tension Free Herniorrhaphy in Adult Inguinal Hernia.

BACKGROUND: Since the herniorrhaphy was performed by Bassini, that method has been the standard for herniorrhaphy for over a hundred year, although it has been modified by other surgeons. During recent decades, biomaterials were introduced to the medical field, and polypropylene mesh was applied to reconstruct the defective abdominal wall of an inguinal hernia in an adult. The Lichtenstein Hernia Institute is regarded as the leading group for the tension-free herniorrhaphy using Marlex mesh under local anesthesia. METHODS: We designed this study prospectively to investigate the characteristics of groin hernias and the results, including complications and recurrence after surgery. The primary method of surgery was a Lichtenstein's tension-free herniorrhaphy using Prolene mesh. We performed 196 hernioplasties in 180 patients having a groin hernia, which included 16 bilateral hernias. RESULTS: 139 indirect hernias, 51 direct hernias, and 3 femoral hernias were included; there were 15 recurrent hernias. A Lichtenstein's tension-free herniorrhaphy was performed in 84.2% of the cases, a preperitoneal mesh graft in 10.2%, and a Bassini's method in 4.6%. There were four recurrences; three were through the femoral canal after the repair of a direct hernia in a female, and one was a recurrent direct hernia in a male patient. CONCLUSIONS: With the use of a mesh prosthesis, a tension-free herniorrhaphy is possible with neither distortion of the normal anatomy nor suture-line tension. However, it should be carefully applied to a direct hernia only after thorough exploration of the groin through the retroinguinal space of Bogros to rule out coexisting intraparietal or femoral hernias. It is necessary to provide enough laxity for the mesh because prolene mesh can shrink up to 20%, and recurrence might be caused by the tension in the applied mesh.

3.Clinical Outcome of Small Incision Lenticule Extraction including Visual Quality Analysis.

PURPOSE: To present the clinical outcomes of small incision lenticule extraction (SMILE) including visual quality analysis in Korean patients with myopia METHODS: The medical records of 228 eyes of 116 patients who underwent SMILE in HanGil Eye Hospital LASIK Center from May 2014 to Feb 2015 and were followed-up for at least 3 months was analyzed retrospectively. The patients were followed up at 1 day, 1 week, 1 month, and 3 months after the operation. Refractive value, visual acuity, intraocular pressure, and visual quality were measured at each visit RESULTS: Preoperatively, uncorrected distant visual acuity was 0.01 ± 0.02 in log MAR, spherical equivalent was -5.03 ± 1.72 diopters, intraocular pressure was 15.85 ± 2.85 mm Hg, and the objective scattering index (OSI) value was 0.68 ± 0.49. The postoperative uncorrected distant visual acuity was 0.13 ± 0.10, 0.05 ± 0.08, 0.04 ± 0.09, and 0.02 ± 0.04 and OSI was 2.16 ± 1.89, 1.25 ± 0.64, 1.14 ± 0.69, and 0.81 ± 0.36 at 1 day, 1 week, 1 month, and 3 months after the operation, respectively. The postoperative intraocular pressure was 12.55 ± 3.74 mm Hg, 13.03 ± 4.35 mm Hg, 11.65 ± 2.49 mm Hg at 1 week, 1 month, and 3 months after the operation. The efficacy of refractive surgery 3 months after the operation was 0.97 ± 0.11, the safety was 0.99 ± 0.10, and predictability was 99.56% and 100.00% at the range of ±0.5 diopters and ± 1.0 diopter, respectively. CONCLUSIONS: The SMILE operation showed comparable clinical outcomes with conventional refractive surgery in terms of efficacy, safety, and predictability. Intraocular pressure and visual quality normalized gradually throughout the 3-month postoperative period.

5.Ultrastructural Study on the Renal Corpuscle after Removal of Ureteral Obstruction in Rabbit.

This study was designed to clarify morphological changes in the renal corpuscles of the experimental rabbit kidney after unilateral ureteral obstruction and release of Unilateral ureteral obstruction through a light and electro microscopy. It was further aimed at obtaining data, esp. of electron microscopy, concerning the morphological repair after relieving ureteral obstruction. A total of 25 white rabbits, weighing 2.5kg, were used. 25 rabbits were divided into three groups-normal control, unilateral ureteral obstruction, release of unilateral obstruction. The normal! control group consisted of 5 rabbits. The remaining 20 rabbits were completely ligated in the left ureter with silk threads. Ten obstructed kidneys were studied by light and electron microscopy, five of them two weeks after obstruction, and the remaining five four weeks after obstruction. A total of ten postobstructed kidneys, five of which were reoperated by Mackinnon method for the relief of ureteral obstruction two weeks after obstruction and the remaining five four weeks after obstruction. They were studied for light and electron microscopy 15 days after relieving ureteral obstruction. Specimens of renal cortex from experimental animals were fixed in 10% neutral formalin or Bouin's solution, embedded in paraffin wax, sectioned at a thickness of 5 um, and stained with hematoxylin-eosin, periodic acid-schiff reaction or Masson's trichome for light microscopy. For electron microscopy, the tissues were fixed in a mixture of 2% paraformaldehyde and 2.5ao glutaraldehyde (phosphate buffer, pH 7.2) prior to fixation in 1% osmium tetroxide (phosphate buffer, pH 7.2) and then embedded in Epon 812. The sections were cut with LKB- III ultramicrotome and contrasted with uranyl acetate and lead citrate and examined with electron microscopy JEM 100B. The results of this study were as follows: 1. The abnormal morphology of the glomerulus in kidneys obstructed for four weeks showed: Slight glomerular congestion, capillary dilatation, no changes in endothelium, podocyte and microstructures. 2. Deformity of Bowman's capsular epithelium such as irregular basal invaginations facing with the basal lamina was observed in the two week obstructed kidneys, and severe degenerative changes such as desquamated capsular epithelial cells in the Bowman's space and long basal cytoplasmic processes embedded in the basal lamina in the four week obstructed kidneys. 3. Among the postobstructed kidney, the two week obstructed kidneys were restored to near normal on the 15th day, but in the four week obstructed kidney, Bowman's capsular epithelium showed partial recovery. 4. The results indicate that glomeruli in the renal corpuscle were preserved until four week weeks after ureteral obstruction but Bowman's epithelium showed change in the 2nd week after obstruction and severe degenerative changes of Bowman's capsular epithelium were noticed in the 4th week after ureteral obstruction. The morphological changes were totally restored in the group where ureteral obstruction was relieved after two weeks, but in the group which was relieved of ureteral obstruction after four weeks the morphological repairs were prolonged until after 15 days.

7.A Case of Renal Cell Carcinoma with Contralateral Renal Hypoplasia.

One patient who underwent radical nephrectomy for renal cell carcinoma participated in a follow up study of the contralateral hypoplastic renal function. Compensatory renal enlargement was demonstrated on follow excretory urogram. The enlargement was 12mm in length, 16mm in width. Immediately, postoperative renal function, GFR, was decreased to minimum value, 8.6 ml/min., but GFR of the remaining kidney was increased to maximal value, 37 ml/min. on post-op 5th day, and then decreased. After post-op 2.6 month the GFR was maintained as the level of 13-14 ml/min. Plasma BUN and creatinine was increased to maximum value, BUN 46.1 mg/dl Cr. 4.1mg/dl, on post-op 9th day and then decreased continuously. But these BUN and creatinine level were maintained as 29.3 mg/dl, 3.4 mg/dl on post-up 6th month. We present a case of this renal cell carcinoma with contralateral renal hypoplasia.

8.Five Year Follow-up Testicular Growth Results after Orchiopexy in Palpable Cryptorchid Testis.

PURPOSE: Postoperative long-term follow-up studies after orchiopexy are rare as compared for other studies such as for histological changes and proper operative age. We analyzed the five-year long-term follow-up results after orchiopexy, expecting a proper prognosis of testicular growth. MATERIALS AND METHODS: A total of 57 patients were selected from a follow-up study in 211 boys who underwent orchiopexy between January 1999 and July 2001. Patients were divided in several categories(operation age, testis consistency, pre-operational position of the testis) and were analyzed by examination of the previous medical records, preoperative and postoperative testicular volume(measured by ultrasonography), follow- up history taking, and from a physical examination. RESULTS: For the 57 patients, 14 patients had bilateral and 43 patients had unilateral cryptorchidism. All of the cryptorchid testes were smaller than a normal testis in the unilateral group of patients as determined by testis ultrasonography. Sorting by operation age, only patients that received orchiopexy within two years from birth showed a significant recovery of testicular volume percentage(operated testis/normal testis x100%: in the unilateral group) at follow-up. Sorting by consistency, the patients with normal testicular consistency showed a significant recovery of testicular volume percentage(operated testis/normal testis x100%: in the unilateral group) at follow-up. Sorting by the preoperative position of the testis, the proximity of the testis to the scrotum was a significant factor for recovery of delayed cryptorchid testicular growth. CONCLUSIONS: Orchiopexy performed at less than two years from birth and the proximity of the testis to the scrotum were significant factors for recovery of delayed cryptorchid testicular growth. Consistency of a cryptorchid testis can be a meaningful factor for expectation of testicular growth by the establishment of an objective measurement for testicular consistency and studies for histological correlation.

10.The Effect of Visual Feedback of Head Angles With Using a Mobile Posture-Aware System on Craniocervical Angle and Neck and Shoulder Muscles Fatigue During Watching the Smartphone.

PURPOSE: To find the effect of visual feedback of head angle with using a mobile posture-aware system on craniocervical angle and neck and shoulder muscles fatigue for preventing or decreasing the forward head posture. METHODS: Twenty-four healthy young adults in Chungbuk to participate in this study. The subjects started to watch a movie clip for 10 minutes with visual feedback in 0°, 30°, and 60° of head angles. During the task, surface electromyography (EMG) was used to collect data from the upper trapezius (UT), sternocleidomasetoid muscle (SCM), cervical erecter spinae (CES) during watching the smartphone. Craniocervical angles were measured using a sagittal-view photograph of the subject in a sitting posture. A one-way repeated analysis of variance with a significant level of 0.05 used for statistical analysis. RESULTS: Craniocervical angle with 0° visual feedback was significantly greater than 30°and 60°. Craniocervical angle with 30° visual feedback was significantly greater than 60°. In addition, MDF of UT muscles in 0° and 30° of visual feedback was significantly greater than 60°. CONCLUSION: We concluded that 0° visual feedback of head angle with using a mobile posture-aware system would be beneficial to prevent or decrease forward head posture during watching a smartphone. We also could recommend using of 30° visual feedback in case of caring UT muscle fatigue primarily.
