1.Mammographic findings of breast cancer: Emphasis on the tumor and microcalcifications.

Most of the breast cancer revealed mass and/or microcalcifications on mammography. We analyzed morphologic characteristics of the masses and microcalcifications on mammography which confirmed as breast cancer pathologically. Of all 54 cases, 4 patients (7%) showed microcalcification only, 27 patients (50%), mass only, and the other 23 patients (43%), both microcalcification and mass on the mammography. The margin of the breast mass were ill-defined in 37 cases, well-defined in 8 cases, and well-defined with surrounding infiltration in 5 cases. The morphologic characteristics of the microcalcification were punctate-linear-V shape in 11 patients, punctate-linear shape in 9 patients, and punctate shape in 7 patients.

2.Existance of cholinergic and purinergic receptor on the detrusor muscle of rat urinary bladder.

This study was aimed at investigation of the stimulatory innervations on the rat urinary bladder. Detrusor muscle strips of 15 mm long were suspended in isolated muscle chambers containing 1 ml of PSS maintained at 37℃ and aerated with 95% O²/5% Co². Isometric myography was performed, and the results were as followings: Muscle strips showed “on-contraction” by electric field stimulation (EFS) frequency-dependently. The EFS-induced contraction was not affected by hexamethonium, a ganglion blocker, but abolished by tetrodotoxin, a nerve conduction blocker. Physostigmine, a cholinesterase inhibitor enhanced the EFS-induced contraction which was inhibited by hemicholinium, an inhibitor of choline uptake at the cholinergic nerve ending. Such an EFS-induced contraction was antagonized by atropine only partially, and the atropine-resistant portion was completely abolished by the desensitization of purinergic receptors by prolonged incubating of the strips in the presence of high concentration of ATP. Bethanechol, a cholinergic agonist, elicited concentration-dependent contraction. Adenosine triphosphate (ATP), a purinergic agonist, induced a weak but concentration-dependent contraction of short duration. Bethanechol-induced contraction was not affected by ATP-desensitization, and ATP-induced contraction was not affected by tetrodotoxin. These results suggest that there are at least two main stimulatory components of innervations in the detrusor muscle, cholinergic muscarinic and purinergic; and those receptors are independent each other.

3.Two Cases of Fetal Bilateral Renal Agenesis.

Fetal bilateral renal agenesis is a lethal congenital anomaly. An early and reliable prenatal diagnosis is extremely important as it may offer options for pregnancy termination as early as possible. The criteria for the ultrasonographic diagnosis of bilateral renal agenesis are severe oligohydramnios, nonvisualization of the bladder, and the empty renal fossa. However, severe oligohydramnios makes it difficult to diagnose the disease because of poor sonographic resolution. We present two cases of bilateral renal agenesis, one is diagnosed by ultrasonography after amnioinfusion at 24 weeks gestation, the other is diagnosed postnatally after term delivery.

5.Temporal and Spatial Expression of Bone Morphogenetic Protein -2 and -4 mRNA in Distraction Osteogenesis and Fracture Healing.

Temporal and spatial expression of bmp-2 and bmp-4 was investigated in distraction osteogenesis and fracture healing models in order to delineate their roles in these new hone formation processes. Distraction osteogenesis was performed on the left tihia diaphyses of Sprague-Dawley rats, and was confirmed by serial radiographs and histologicaJ examination. Total RNA was isolated from the distraction gaps and fracture sites sequentially until the postoperative eighth week, and mRNA expression was quantitated hy competitive reverse transcription-polymerase chain reaction(RT-PCR) using specific primers for bmp-2 and bmp-4. In-situ hybridization was performed on the undemineralized tissue section of distraction osteogenesis group. Both bum-2 and bmp-4 mRNA expression increased during distraction osteogenesis and fracture healing. Increase in bmp-2 mRNA expression was more marked and prolonged in distraction osteogenesis compared with fracture healing, whereas bmp-4 mRNA expression was persistently increased in both groups. In-situ hybridization study revealed that hoth bmp-2 and bmp-4 were expressed at eariy osteohlasts producing osteoid, trabeculae-lining osteoblasts and osteocytes of regenerate bone. Enhanced new bone formation in distraction osteogenesis is associated with prolonged and increased expression of BMPs, especially BMP-2. lnspite of their structural similarity, BMP-2 and BMP-4 appear to have their distinct roles in new hone formation.

7.Recanalization of Chronic Total Occlusion of Coronary Arteries by Prolonged Intracoronary Urokinase Infusion.

BACKGROUND: PTCA is often unsuccessful in a patient with chronic total occlusion of coronary arteries with success rates varying from 60 to 70%. Success rates are related to the duration of total occlusion, longer occlusions being associated with lower success rates. Chronic total occlusion may be associated with thrombi superimposed on the stenotic lesion. We used an intra-coronary bolus of urokinase followed by a prolonged urokinase infusion in an attempt to lyse the lesion and allow for passage of the PTCA wire during subsequent angioplasty. The purpose of prolonged durokinase infusion was to reduce the clot sufficiently to recanalize the coronary artery and make it more amenable to PTCA. METHODS: Study patients: We were included six patients who developed total occlusion for more than 3 weeks and good collateral channels of Grade 2 or more and previous attempts at angioplaty had failed. Procedures: All patients underwent dual catheter system and incremental dose protocol of intracoronary urokinase infusion. RESULTS: The mean duration of occlusion was calculated to be 65.3+/-2.7 weeks and urokinase dose ranged from 130,000 to 200,000U/hr and treatment lasted 21.7+/-1.4 hours in our study. The prolonged urokinase infusion resulted in reperfusion of the occluded dvessel in 5 of the 6 patients(83%), with or without the complementary balloon inflation. One patient failed to recanalize the occluded vessel because cardiac tamponade was developed during the prolonged urokinase infusion. CONCLUSION: We concluded that the prolonged urokinase infusion in occluded coronary artery appeared to increase the likelihood of successful PTCA in patients with chronic total occlusion of coronary arteries.Also, in carefully selected patients, prolonged urokinase infusion in occluded coronary artery was relatively safe and well tolerated.

9.A Clinical Analysis of 52 Cases of Spinal Cord Tumors.

The authors analyzed 52 cases of spinal cord tumor who had been operated upon at the department of neurosurgery of the Koryo General Hospital from 1983 to 1992. The results were summerized as followings: 1) The spinal cord tumor was most common in the 6th decade of age(52 cases, 28.8%) and the ratio of male to female was 1.26:1. 2) The pathologic diagnosis was neurogenic tumor in 25 cases(48.1%), metastatic tumor in 12 cases(23.1%), glial tumor in 6 cases(11.5%), and meningeal tumor in 3 case(5.7%) in order frequency. 3) The tumors were located most frequently in the thoracic area(19 cases, 36.5%) and in the intradural extramedullary space(28 cases, 53.9%). 4) The most common initial presentation was pain(27 cases, 51.9%) and on admission 14 cases(26.9%) showed paraparesis. 5) The rate of abnormality of the plain radiologic study was 59.6% and the most common positive finding was the pedicle erosion(15 cases, 28.8%). 6) The computed tomographic scanning with water soluble contrast media was useful in planning of operation to provide the exact relationship between the tumor, spinal cord and location of dura. 7) The total removal was possible in 27 cases(51.9%). 8) The follow-up results after 19.4 months of the mean follow-up duration in 46 cases were recovery in 29 cases(63.0%), improvement in 12 cases(26.0%), stationary state in 3 cases(6.5%), worse in 2 cases(4.3%). It was excellent particulary in cases of intradural-extramedullary tumor. 9) The incidence of metastatic tumor was 23.1%(12 cases) and the lung was most common primary focus. The follow-up result of metastatic tumor was poor than the other tumors.
