As cost pressures have escalated, policy makers, politicians, health care providers and families have tried to devise ways to reduce health care costs. While originally developed to enhance patient control and to provide better care at the end of life, hospice care has recently received significant attention as a mean of reducing health care costs. As a program providing care for patients who are dying at their homes, hospice has expanded slowly since the opening of the first hospice in Korea in 1963. Therefore, a variety of services that responds to the needs and concerns of many dying people and their families is limited The purpose of this study was to determine the potential cost savings at the end of life among patients who used home hospice compared with the patients who received institutional care in Korea. This study used a retrospective, descriptive design. The sample for this study included 46 patients who died of lung cancer: 25 patients who received home hospice care and 21 patients who received institutional care. Data on patient characteristics, kinds and frequencies of provided treatment and nursing services, and hospice and hospital charges during the last month before death were collected. Cost of care was measured by the average cost per patient per day in the last month of life. The results of the study indicated that there were significant differences in average cost of care between home hospice sample and institutional care sample (t=9.956, p<.001; home hospice sample: M=18,102 won, institutional care sample: M=317,578 won). The cost of the home hospice sample was approximately 6% of the cost of institutional care. The majority of the home hospice nursing services were education (35.7%) and supportive counseling (25.2%), followed by medication management (13.6%), assessment (12.1%), basic nursing (7.2%), treatment (5.5%) and others. In institutional care sample, basic nursing and treatment were more emphasized than education or supportive counseling among the nursing services provided. The results of this study showed the potential for hospice to reduce costs and implications for policymakers and clinicians to incorporate hospice program into the formal health care delivery system in Korea.

Purpose: The purpose of this study was to identify changes in physical activity, sleep quality, and quality of life (QOL) during COVID-19 in adults by age group. Methods: An online survey was conducted on a total of 160 participants who were divided into three groups by age; young adults, middle-aged adults, and old adults. The participants responded to the self-reported assessment of the impact of social distancing during COVID-19, International Physical Activity Questionnaire (IPAQ), Pittsburgh Sleep Quality Index (PSQI), and Short Form-36 Health Survey (SF-36). Descriptive statistics, analysis of variance, and Pearson’s correlation were used for data analysis. Results: There were significant differences between the three age groups in physical activity (p < 0.05) and PSQI (p < 0.01) and a post hoc analysis showed that the young adults’ group had significantly higher physical activity than the old adults’ group, while the old adults’ group had significantly lower scores of PSQI (better sleep quality) than the others. The young adults’ group showed a significant negative correlation between the stress from social distancing and QOL (r = -0.27, p < 0.05) and between PSQI and QOL (r = -0.48, p < 0.05). For the middle-aged adults’ group, there was a significant negative correlation between PSQI and QOL (r = -0.53, p < 0.05). The old adults’ group showed a significant negative correlation between the stress from social distancing and physical activity (r = -0.35, p < 0.05) and PSQI (r = -0.50, p < 0.05), while there was a significant positive correlation between physical activity and PSQI (r = 0.30, p < 0.05) and QOL (r = 0.30, p < 0.05). Conclusion The results of this study could be used as basic data for the promotion of physical and mental health in the post-COVID-19 era

The objective of this investigation was to evaluate the influence of gestational age at exposure on the prenatal effects of gamma-radiation. Pregnant ICR mice were exposed to a single dose of 2.0 Gy gamma-radiation at a gestational 2.5 to 15.5 days post-coitus (p.c.). The animals were sacrificed on day 18 of gestation and the fetuses were examined for mortality, growth retardation, change in head size and any other morphological abnormalities. The only demonstrable effect of irradiation during the preimplantation period was an increase in prenatal mortality. Resorptions were maximal on post-exposure day 2.5 after conception. The pre-implantation irradiated embryos which survived did not show any major fetal abnormalities. Small head, growth retardation, cleft palate, dilatation of the cerebral ventricle, dilatation of the renal pelvis and abnormalities of the extremities and tail were prominent after exposure during the organogenesis period, especially on day 11.5 of gestation. Our results indicate that the late period of organogenesis in the mouse is a particularly sensitive phase in terms of the development of the brain, skull and extremities.

Hyalinizing trabecular adenoma is an uncommon benign thyroid tumor that is recently described in the literature. This tumor is easily confused with medullary carcinoma on surgical specimens and with papillay carcinoma on cytologic specimens. Herein we report the cytologic characteristics of a case of histologically proven hyalinizing trabecular adenoma of the thyroid gland. Cytologically, the aspirate showed trabecular or individually dispersed polygonal cells with finely stippled chromatin pattern, nuclear grooves, and eosinophilic nuclear pseudoinclusions. No colloid materials were noted in the background.

Left ventricular free wall rupture following acute myocardial infarction (AMI) is the second most common cause of death and has been reported to be responsible for 4 to 24% of all infarction deaths. The rupture occurs anywhere from a few hours to several days after AMI. The common findings of ventricular rupture are persistent chest pain bradycardia and shock. This may be often mistaken for the ruptured dissection of the ascending aorta. The different points from dissection are 1) persistent chest pain 2)persistent ST segment elevation and 3) only intramural hematoma in ascending aorta. We have sucessfully managed two patients with postinfarction myocardial rupture. Surgical management consisted of infarctectomy repairi of the ventricular rupture and coronay artery bypass grafting. We conclude that successful surgical management of ventricular free wall rupture should require prompt diagnosis and emergency operation.

The cytologic findings in fine needle aspiration of a case of myxoid liposarcoma of the mediastinum are described. The smear and cell block of the aspirate revealed solid clusters with background of amorphous material and scattered single tumor cells. The clusters were moderately cellular and consisted of atypical lipoblasts in varying stages of differentiation and delicate plexiform capillaries. Good correlation was found between the histologic and cytologic findings in the fine needle aspirates. The methods of hysterectomy for intrapelvic benign disease include total, subtotal, la-paroscopic-assisted transvaginal, and SACSIH operation. There have been 298 cases of SACSIH operation for benign uterine or pelvic diseases at the Chonbuk National University Hospital from November 16, 1992 to March 31, 1997. Patients were 41.8 years old on the average ranging from 29 to 50. Their indications cover uterine myoma(211), adenomyosis(37), uterine myoma with pelvic adhesion(16), uterine my- oma with endometriosis(11), and others(23), the uterine myoma being the most common. Both CIN and CIS is excluded after the full evaluation of the cervical disease through Pap. smear, cervicography, ECC biopsy, and HPV typing etc, and the consent of patient is secu- red. The modified SACSIH operation, when compared with other simple hysterectomy oper- ation, has the following advantages;1) minimal surgery, 2) easy adaptation, 3) avoidance of stump cancer, 4) rapid recovery and 5) less postoperative care as far as psychosomatic posthysterectomy problems are concerned. One of the new equipment for new modified technique is the Electro-Surgical Unit system with Electrode for endocervical canal resection. This is developed at this hospital as a part of the G-7 project supported by the Ministry of Health and Welfare. It is made of electrode and rod from electrosurgical unit system and operated by high frequency of current. Thus, we introduce, herein, the modified SACSIH operation and propose the postopera- tional scoring system to estimate postoperative sequelae and to help carry on personal well- being in terms of sexuality and normal life.

Purpose: This study examined the effects of social distancing in COVID-19 on musculoskeletal problems and the quality of life. Methods: One hundred and thirty-five subjects participated in this study and responded to an online survey on stress from the social distancing in COVID-19, computer and smartphone usages, musculoskeletal pain, and quality of life. The stress from social distancing was assessed by a Visual Analogue Scale (VAS). The Neck Disability Index (NDI), Oswestry Disability Index (ODI), and Boston Carpal Tunnel Questionnaire (BCTQ) were provided to assess the musculoskeletal pain, and Short Form-36 (SF-36) was provided to assess the quality of life. Frequency analysis, descriptive statistics, Pearson’s correlation coefficient, and independent t-test were used for data analysis. Results: Statistically significant, but low positive correlation stress from the social distancing (VAS) and NDI (r=0.19, p<0.05) and ODI (r=0.18, p<0.05), and negative correlation between stress from the social distancing (VAS) and SF-36 (r=-0.17, p<0.05). The NDI score was significantly greater in the group with high stress than in the group with low stress from social distancing (p<0.05). Conclusion This study is meaningful in that it evaluated, through the physical therapy approaches, the potential side-effects on physical and mental health in various social changes caused by COVID-19. The results of this study may be used as basic data in future studies related to COVID-19.
