BACKGROUND: The relation between clinical characteristics and a change of thymid functions in patients with hyperemesis gavidarum(HO) has not been well described. In order to clarify this characteristics, we evaluated thyroid function in patients with HG and compared the relationship to clinical characteristics of gestational thyrotoxicosis and hyperemesis. METHODS: From March, 1991, to April, 1997, 20 patients were admitted to the department of obstetrics in Chonbuk National University Hospital because of HG. We examined clinical characteristics of patients with HG(age, marrital age, levels of educations, obstetrical history, gestational duration, vomiting duration, degree of weight loss, infant's birth weights, duration of pregnancy). Thyroid functions, liver functions and serum electrolytes was measured. Also, we compared to the clinical characteristics of patients with HG according to thyroxine concentration. RESULTS: An increased free thyroxine concentration was observed m 65% of 20 pregnancies complicated by severe HG. The level of serum ALT was greater in thyrotoxicosis patients(91.2+-76.6 IU/L, mean +-SD) than normothyroxemic patients(20.3+-12.7 IU/L, P=0.027). Also, increased level of serum AST was observed in patients with HG. In patients with thyrotoxicosis, the long durations of vomiting was observed. CONCLUSIONS: Increased free thyroxine concentration is found in severe HG. Because the relatively higher risk for thyrotoxicosis, patients with severe hyperemesis, they must be evaluated about thyroid functions.

Apert syndrome, or acrocephalosyndactyly is a complex of associated malformations, which are craniosynostosis and symmetric complex syndactyly of the hands and feet. The syndrome was first described by Apert in 1906. Review of the worlds literature disclosed about 200 similar cases and 9 Cases in Korea. Authors experienced one case of Apert syndrome and treated surgically for acro-syndactyly and polydactyly of both hands and feet. The case was compared with the other previously reported cases in Korea.

The following case of solitary bone cyst of the proximal femur was misdiagnosed as fibrous dysplasia because of similarity of X-ray findings and was confirmed by histological findings. The cyst was treated by curettage, packing with autogenous iliac bone and kiel bone graft and internal fixation with Ender nail. The most recent evaluation one year after surgery showed no pain and recurrence.

This study was performed to investigate the distribution of endotoxin in various organs after intraperitoneal injection of E. coli homogenator(0111:B4, 3X10(9)cells/200g of body weight). Sprague-Dawley rats were intraperitoneally injected with E. coli homogenator and sacrificed 1 and 3 hours after injection. The lung, liver, and kidney were immunohistochemically stained with avidin-biotin complex method and observed by light and electron microscopy. On the light microscopy, granular deposits of reaction products of immunohistochemical stain were found on the cytoplasmic membrane of endothelial cells and some of parenchymal cells of all organs observed. Electron microscopic study revealed finely granular reaction products on the surface of endothelial cells and some of parenchymal cells. The pinocytotic vesicles of endothelial cells demonstrated reaction products in the early phase of experiment. The distribution of reaction products were prominent in the liver among three organs. The Kupffer cells showed the most sensitive and strongest positive reaction. The hepatocytes and endothelial cells revealed weak positive reaction 3 hours later. The alveolar macrophages of the lung were also positive from the early phase of endotoxemia, while the pneumocytes and alveolar septa demonstrated weakly positive reaction in the later phase. The capillary endothelium of the kidney revealed positive reaction from the early phase. According to above results, it is concluded that the endotoxin entered into the systemic circulation was captured in the liver and lung. And both mononuclear phagocytic system and endothelial cells could be activated or damaged by endotoxin.

Ureteroscopic removal of stone is now an established technique for the management of ureteral stones and can be done under direct vision using flexible grasping forceps or stone baskets with increased safety and efficacy. A clinical study of our 35 cases of ureteroscopic removal of ureteral calculi was performed. Only one among 4 cases of mid and upper ureteral calculi and 27 among 31 cases of lower ureteral calculi were removed successfully. Therefore the overall success rate was 80 %(28 among 35 cases). Complications occurred in 6 cases : a case of ureteral avulsion, a case of ureteral separation, 2 cases of uncomplicated urinary tract infection and 2 cases of ureteral perforations. We conclude that ureteroscopic removal of ureteral stone under direct vision can be done safely and be the first choice of management for the lower ureteral surgical calculi, but the antegrade removal of stone through percutaneous nephrostomy tract may be preferable for the upper and mid ureteral calculi.

We performed this study to observe the effects of chronic administration of leuprolide acetate(potent synthetic LHRH analog) to the testicular histopathology of prostatic cancer patient and to the spermatogenesis in cryptochism treatment. Ten out of fifty healthy male rats whose body weight ranged 250-300gm were selected as a control group and the other forty as a study group. Two hundred micro-gram of leuprolide acetate was injected daily into the peritoneal cavity of study group, and ten rats were sacrificed at each first, second, third and sixth week, respectively. Then the changes of testicular weight and histopathological readings under light microscope of both control and study group were recorded and analyzed. The results of experimental data can be summarized as follows : 1. The average testicular weight of control group increased progressively until sixth week to 1. 1.43gm from the initial value of 1.26gm, while the weight of study group decreased to 0.96gm which was 67.1 % of age-matched control group. 2. There were almost no remarkable changes between both groups in terms of number, size and cellularity of seminiferous tubules. We observed the degenerative and regenerative changes simultaneously in spermatogonia and spermatocytes. During the first three weeks, degenerative changes took place while the regenerative changes started from the second week and have fully recovered at the sixth week. And, the number of spermatids and spermatozoa slightly decreased at the first week, but have recovered to normal value from the second week. 3. There were noticeable changes neither in the interstitium of testes except the minimal edema nor in terms of size and number of Leydig cell. The results using rats showed the discordance with those of human study of prostatic cancer patient. This conflict can be attributed to the functional differences of leuprolide action between two species, age of sacrificed rats and insufficiency of dose and time of administration. However, we can draw another conclusion that the leuprolide acetate is expected to have no significant adverse effects to the spermatogenesis in cryptorchism treatment.

The onset of systemic scleroderma in the first. decade of lif occurs in less than 1.5% of instances. Systemic sclerodrma in childhood have been characterized by less severe visceral involvement and a benign course We report a boy, aged 7 years, with a history of Raynaid; phenomenon and dysphagia. He had a mask-like face, flexion contracture of hands and knee joints, digital pitting scars, ichthyosiform skin change on leg and hyperpigrnented induration on all extremities and trunk. Antinuclear antibodies were positive in nucleolar type and Scl-70 antiocy was also present. On radiologie study, the pattern of reflux esophagitis and resorption of the listal ends of all phalanges were nated. Histopathological findings were compatible with sclerodma. He was treated with Penicillamine-D and oral corticosteroic with good improvernent.

The globus symptom is a condition in which a patient, often middle aged women, complains of a lump andchocking sensation in the throat, Functional disturbance of the cricopharyngeal muscle, rendering it incapable ofrelaxing during swallowing, has long been recognized as a cause of globus symptom and dysphagia. We wanted to findout how often and to what extent distrubed relaxation of the cricopharyngeal muscle can be seen in patients withglobus symptom with routine examination and video esophagogram. The results were as follows: 1. Male : femaleratio was 1:2.4. 2. Globus symptom was most frequent in the age group between 30-39 of female. 3. Organic lesionswere seen in 43.6%(24 Pts) of globus patients. 4. Cricopharyngeal muscle was visualized in 29.1%(16Pts), esophageal diverticulum in 3.6%(2 Pts) and degenerative spondylosis in 3.6%(2 Pts). 5. Incidence of visualizationof cricopharyngeal muscle were higher in male group (50%) than female one (20.5%). 6. Cricopharyngeal muscle wasmost frequently visualized on early swallowing phase(12/16 Pts).
