BACKGROUND: The relation between clinical characteristics and a change of thymid functions in patients with hyperemesis gavidarum(HO) has not been well described. In order to clarify this characteristics, we evaluated thyroid function in patients with HG and compared the relationship to clinical characteristics of gestational thyrotoxicosis and hyperemesis. METHODS: From March, 1991, to April, 1997, 20 patients were admitted to the department of obstetrics in Chonbuk National University Hospital because of HG. We examined clinical characteristics of patients with HG(age, marrital age, levels of educations, obstetrical history, gestational duration, vomiting duration, degree of weight loss, infant's birth weights, duration of pregnancy). Thyroid functions, liver functions and serum electrolytes was measured. Also, we compared to the clinical characteristics of patients with HG according to thyroxine concentration. RESULTS: An increased free thyroxine concentration was observed m 65% of 20 pregnancies complicated by severe HG. The level of serum ALT was greater in thyrotoxicosis patients(91.2+-76.6 IU/L, mean +-SD) than normothyroxemic patients(20.3+-12.7 IU/L, P=0.027). Also, increased level of serum AST was observed in patients with HG. In patients with thyrotoxicosis, the long durations of vomiting was observed. CONCLUSIONS: Increased free thyroxine concentration is found in severe HG. Because the relatively higher risk for thyrotoxicosis, patients with severe hyperemesis, they must be evaluated about thyroid functions.

The clinicolaboratory findings, cardiac catheterization, and outcome of operation were analyzed in 78 patients with tetralogy of Fallot, which were diagnosed by echocardiography, cardiac catheterization, and a cardiac angiography and confirmed by operation at Dong San Hospital, Keimyung University, during a 4(1/2)-year period from January 1984 to June 1988. The following results were obtained : 1) Out of the 78 cases, 45 were male and 64 were female. 2) The electrocardiogram showed right axis deviation in 71 cases(91%) and right ventricular hypertrophy in 66 cases(84.6%). 3) Chest X-ray revealed that, the cardiothoracic ratio was normal or decreased in 68 cases(87.3%), and cardiac apex elevation was noticed in 57 cases(73%). 4) Pulmonary stenosis were chiefly valvular and infundibular type(53.8%). 5) The associated heart diseases with TOF were patent foramen ovale(64.1%), right sided aortic arch(19.2%), and secundum ASD(10.3%), in that order. 6) There was an intimate correlation between secondary polycythemia and thrombocytopenia. 7) Among the 78 Cases, 73 cases had total correction, 1 case had shunt operation, and 4cases had total correction after shunt operation. The highest mortality rate occurred in the cases of total correction after shunt operation(25%). The cases with a main pulmonary artery size of 1/3 to 2/3 against, the aorta had a higher mortality than the other group, and no cases expired in the group with a ratio of more than 2/3. The overall surgical mortality was 8.9%.

Infectious process involves infrequently the posterior compartment of the vertebra. Three cases of such condition are experienced at the Dept. of Orthopedic Surgery, Seoul National University during the period of August 1962 to June 1971 Two of three cases were from pyogenic infection invoIving the spinous process of third and fourth lumbar vertebra respectively, one of which was complicated by formation of paraspinal abscess and both were treated with total laminectomy of the involved vertebrae. Thee third case was tuberculosis on the transverse process of first thoracic vertebra which was accompanied by irritating sign of left ulnar nerve, and was treated with transversectomy and partial laminectomy. Excellent results were obtained without recurrence and no deformity nor instability of the spine developed postoperativeIy.

A child, aged 4-year-old girl was admitted to our hospital because of impaired vision and right hemiparesis. Brain MRI showed a lesion in the left parietal white matter, and visual evoked potentials and brainstem evoked potentials gave abnormal findings showing lesions also of the optic nerves and brainstem. Her symptoms were resolved with steroid therapy. Afterwards, she had 3 episodes of spastic paraparesis, cerebellar syndrome, downbeat nystagmus, and clinical seizures for a year. With a history of remissions and exacerbations and brain MRls that revealed new scattered lesions each time, the diagnosis of multiple sclerosis was able to be made. This is the one of the youngest of known patients with MS, yet described, in Korea, whose first attack occurred at 4 years old of age.

Diabetes mellitus is the most common cause of the peripheral neuropathy and an important risk factor of the cerebrovascular disease. There are also several case reports of the diabetic myelopathy, and it has been known that the incidence of cord involvement is not rare on autopsy and somatosensory evoked potential studies. To determine the functional derangement of central descending motor pathway in diabetes, 59 cases of diabetes were studied with the magnetic motor evoked potential (MMEP) study. The diabetic patients were divided into two groups according to the nerve conduction study (23 with normal and 36 with abnormal nerve conduction studies) and compared with the results of 31 normal control cases. The results are as follows. 1. At each stimulated point, there are statistically significant differences in latencies of MMEPs between normal control and diabetic groups. 2. Compared to the control group, the peripheral conduction time is also significantly prolonged in diabetic patients. 3. However, there is no difference in the calculated central motor conduction time among three groups. In conclusion, this result means that prolonged latencies of MMEPs in diabetes may be due to peripheral neuropathy rather than dysfunction of central motor pathway. Therefore the clinical significance of MMEP has to be decided segmentally rather than by simple determination of the vertex latency.

Here we report a case of the classical inclusion body myositis. The muscle pathology in a 61-year-old male patient with slowly progressive proximal muscle weakness and atrophy revealed basophilic rimmed vacuoles on light microscope and intracytoplasmic filamentous inclusions with membranous whorls through electron microscope. He did not respond to steroid therapy.
