4.Treatment of Segmental Tibial Fracture

In addition to general charceteristics of tibial fracture, segmental tibial fracture is commonly combined with extensive soft tissue injury, comminution and displacement with poor blood supply in its middle segment. According to recent reports, intramedullary nailing was regarded as the excellent method in the management of this kind of fracture. Twenty-one cases were treated at the department of Orthopaedic Surgery, National Medical Center from January 1980 to December 1987 and following results were obtained; 1. Most common type of fracture was Type I (38.1%). Almost all the fractures were accompanied by open wounds(85.7%) and GIIIB open wounds were 12 cases(57.1%). 2. A verage union time was 31.8 weeks(union rate, 76.2%) and showed marked difference between closed fracture(20.2 weeks) and GIIIB open one(38.6 weeks). 3. Better results were seen in 8 cases of intramedullary nailing(average union time, 24.2 weeks), while all the plating method showed non-union in 3 cases of open wound.

6.Significance of Microsurgical Varicocelectomy for Non- obstructive Azoospermic Patients.

PURPOSE: This study attempted to assess the treatment outcomes after microsurgical varicocelectomy in non-obstructive azoospermic patients and correlate the surgical outcomes with the histopathological patterns of their testes. MATERIALS AND METHODS: Fourteen non-obstructive azoospermic men who underwent testicular biopsy and varicocelectomy, between September 2001 and December 2002, were reviewed. Varicocelectomy was unilaterally and bilaterally performed in 12 and 2 patients, respectively, using an inguinal approach with a microsurgical technique. Postoperative semen analyses were performed in each patient four months after the varicocelectomy. RESULTS: In the testicular histology, hypospermatogenesis was identified in three, maturation arrest in five and Sertoli-cell-only syndrome in six of the subjects. After a mean follow-up of 7.4 months, motile sperm in the ejaculate was identified in 6(43%) of the non-obstructive azoospermic patients. These included 2 in hypospermatogenesis, 3 in maturation arrest and 1 in Sertoli-cell-only syndrome, in terms of their histopathology. These six had improvements in their sperm concentration and motility, which were 0.45x10(6)/ml and 51.3%, respectively. However, of these six, with motile sperm after varicocelectomy, 1 with maturation arrest and 1 with Sertoli-cell-only syndrome returned to their previous azoospermic state on their second postoperative semen analyses. A pregnancy was achieved by natural intercourse for 1 of the men(7.1%) with hypospermatogenesis. CONCLUSIONS: Microsurgical varicocelectomy offers non-obstructive azoospermic patients the chance to have sperm in their ejaculate, and therefore, the possibility of natural pregnancy. Microsurgical varicocelectomy may be considered with the initial treatment modality in selective non- obstructive azoospermic patients with varicocele prior to intervention with assisted reproductive technology.

7.Effects of Locally Applicated Safflower Seeds Extract on Bone Regeneration of Rat Calvarial Defects.

The ultimate goal of periodontal treatment is the regeneration of periodontal tissues which have been lost due to periodontal disease. Recently, many natural medicines have been studied for their potential of anti-bacterial, anti-inflammatory and regenerative effects in periodontal tissues. Safflower seeds have been traditionally used as a drug for treatment of fracture and blood stasis in oriental medicine. The objective of the present study is to examine the biologic effects of safflower seeds extract on bone formation and regeneration of rat calvarial defects. The calvarial defects were made with 8mm trephine bur and extract of safflower seeds were placed directly at these defects. 24 rats were divided into control and experimental groups, and each group was sacrificed at 1 week, 4 weeks and 8 weeks. To study a histopathology related to bone regeneration, Goldner's Masson Trichrome stain and histomorphologic measuring was done at each weeks. In the early phase of bone healing, less inflammatory infiltration and capillary proliferation was found in experimental group compared to control. Dense bony tissues and matured bone structures in defect areas were found in experimental groups. And area of new bone formation was significantly increased at 8 weeks in experimental group. These results indicate that direct local application of safflower seeds extract reduces the early inflammatory response and promotes the regeneration of new bone in calvarial defects of rats.

8.Comparative Study of the Tension-Free Vaginal Tape (TVT) Procedure and the Suprapubic Arc Sling (SPARC) Procedure for Treating Female Stress Urinary Incontinence: a 1-Year Follow-Up.

PURPOSE: We wanted to compare the efficacy and outcomes of two retropubic mid-urethral sling procedures, tension-free vaginal tape (TVT) and suprapubic arc sling (SPARC), in the women suffering with stress urinary incontinence (SUI). MATERIALS AND METHODS: A total of 124 women with SUI were assigned to either the TVT group (n=62) or the SPARC group (n=62) within a same period. Only the patients with a follow-up of at least 12 months were included and those patients who underwent conjoined pelvic reconstructive surgeries for coexisting pelvic organ prolapse were excluded from this study. Finally, 90 patients (TVT: 42, SPARC: 48) remained in the study. The objective cure rate was evaluated by clinical and urodynamic examinations; the satisfaction rate was determined by using a questionnaire via the telephone or a self-addressed, stamped envelope. The mean follow-up period was 16.3 months in the TVT group and 16.3 months in the SPARC group. RESULTS: Two groups were similar in their preoperative characteristics and the perioperative parameters. There was no significant difference between the 2 groups in terms of the cure rate: cure (91.7% vs. 100%, p=0.056), improvement (6.2% vs. 0%, p=0.099), and failure (2.1% vs. 0%, p=0.347) for SPARC and TVT, respectively. In addition, the patient satisfaction rate was not different significantly between 2 groups. The main complication was urinary retention, and this was diagnosed in 6 patients; 3 (7.1%) in the TVT group and 3 (6.3%) in the SPARC group. De novo urge symptoms were observed in 3 patients (1 in the TVT group and 2 in the SPARC group). CONCLUSIONS: SPARC sling and TVT appears to be equally effective and safe for the surgical treatment of female SUI at the 1-year follow-up, although further studies are needed to establish the long-term efficacy and safety of these procedures.
