1.Clinical Analysis of Metastatic Bone Tumor

A retrospective review of 102 patients with metastatic bone tumor seen at Hanyang University Hospital from 1986 to 1990 was performed. We analyzed the cases in the aspects of primary lesions, age and sex distributions, locations and number of metastatic foci, compared with those in the previous reports in Korea. l. 54.9% of the patients were male and 45.1% were female. The incidence of the metastatic bone tumors in female compared with the previous incidence before 1980's years was relatively increased. The peak age was in the 6th decade. The incidence younger than 50 years was decreased, and the incidence older than the 7th decade was relatively increased. 2. The main primary cancers were lung(40.2%), liver(11.8%), breast(10.8%), unknown(8.8%), and uterine cervix(6.9%) etc. In cases of lung cancer, the frequent histologic types were adenocarci noma, squamous cell carcinoma, and small cell carcinoma. 3. The vertebrae(56.9%), ribs(40.2%), femur(17.6%), pelvis(15.7%) were relatively frequent sites of metastasis which involved multiple metastasis. 4. Single metastasis(62.4%) was more frequent than multiple metastasis(37.6%). 5. In the primary cancers, the incidence in hepatoma of male was increasing, the incidence in lung cancer of female was relatively increasing, but the incidence in stomach cancer was decreasing.

3.Association of Herpes Zoster and Lymphosarcoma: Report of one Case.

Herpes zoster is generally believed to be caused by the activation of varicella-zoster virus present in the body since an original infection with varicella. According to the thesis, the virus remairis in a latent state in the cells of the sensory ganglia until immunity has waned sufficiently to pezmit multiplication of the virus and clinical infection. There are a number of reports that in patient with malignant disease, especially Jymphoma, a frequenry af association of zoster is greater and severity of its symptom is increased to be marked contrast to the benign uneventful course of zoster seen in healthy person. The author observed one case of zoster seen in patient with malignant disorder, and frequency of association of zoster in malignant disease and its etiological factors are reviewed in the literature.

4.Associations of Ultrasonographic Features with Scrotal Pain after Vasectomy.

PURPOSE: Scrotal discomfort is a recognized complication of vasectomy, but the natural history and incidence of this problem are uncertain. The typical ultrasonographic changes after a vasectomy primarily include epididymal thickening and epididymal tubular ectasia with diminished blood flow in the epididymis. We prospectively studied the differences in the ultrasonographic features of the testis and epididymis between patients with and those without scrotal discomfort after vasectomy. MATERIALS AND METHODS: We prospectively assessed pain scores in 178 men who underwent outpatient bilateral no-scalpel vasectomy at our institution between January 2009 and December 2010. At 2 months after vasectomy, we evaluated the postoperative scrotal pain questionnaire and scrotal ultrasonographic features for patients who returned for semen analysis. On the basis of the scrotal information, we investigated the potential relationships between scrotal pain or discomfort and scrotal ultrasonographic features of both testes and epididymides. RESULTS: The average age of the 114 men was 36.3 years (range, 29 to 53 years). group 1 (n=23), which reported scrotal pain or discomfort, showed no significant mean differences in the maximal diameter of the head of the epididymis when compared with group 2 (n=91), who had no scrotal pain or discomfort. Also, the width of the body of the epididymis between the two groups showed no significant differences. CONCLUSIONS: There were no significant differences in ultrasonographic features according to the presence of chronic scrotal discomfort after vasectomy. Therefore, causes of scrotal pain other than obstruction may need to be considered after vasectomy.

6.Retrovirus-Mediated Herpes Simplex Virus Thymidine Kinase Gene Therapy for the Prevention of Stenosis in Rat Carotid Artery Injury Model.

BACKGROUND: Herpes simplex virus thymidine kinase (HSVtk) phosphorylates the prodrug ganciclovir to a nucleoside analog that inhibits DNA synthesis, causing cell death. Neighbouring nontransfected cells may be affected through a 'bystander effect', thereby amplifying the antiproliferative actions. This study was carried out to determine whether retrovirus-mediated HSVtk gene therapy could reduce intimal hyperplasia and prevent stenosis following balloon injury of the rat carotid artery. METHODS: A replication-defective recombinant retroviral vector containing HSVtk cDNA (LtkSN) was constructed. Cultured primary rat smooth muscle cells (SMCs) infected with this vector (SMC/LtkSN) were transplanted to the balloon injured rat right carotid artery. One week after transplantation, HSVtk gene therapy group was administered a 2-week treatment of ganciclovir (30 mg/kg/d). Three weeks after balloon injury and SMC/LtkSN transplantation, carotid arteriography was performed and carotid arteries were perfusion-fixed for histologic examination. RESULTS: Carotid arteriographic evaluation comparing with the uninjured left carotid artery showed that the mean luminal diameter of HSVtk gene therapy group (n=5, 85+/-3%) was significantly larger than that of balloon injury only group (n=5, 65+/-5%). The neointimal mass of HSVtk gene therapy group was less than that of balloon injury only group. SMC/LtkSN transplantation without ganciclovir treatment group (n=3) showed asymmetric intimal proliferation probably because of gravitational pooling of seeding. There were inflammatory cell infiltrations at the gravity dependent portion of HSVtk gene therapy group. CONCLUSION: Retrovirus-mediated HSVtk gene therapy following balloon injury of the rat carotid artery reduced neointimal expansion and arteriographic stenosis.

10.Myoclonus Induced by the Use of Gabapentin.

Myoclonus is a rare side effect of gabapentin (GBP) and has been reported in patients with preexisting myoclonus, mental retardation, chronic static encephalopathy, diffuse brain damage, impaired renal function, or end stage renal disease. We report a case of myoclonus in a patient with normal renal function and no previous disorders. A 69-year-old female underwent diskectomy and foraminotomy at the left L4-L5 level. Postoperatively,she complained of paresthesia in her left leg, which was thought to be due to root manipulation during surgery. To relieve the paresthesia, she was given tramadol, an oral opioid agonist, and GBP. One week after GBP was increased to 900 mg per day, myoclonus developed, which severely impaired her normal activity. Her symptoms resolved 2 days after discontinuation of GBP. The coadministration of tramadol and GBP may mutually enhance the myoclonic potential of each drug. The causal relationship between GBP and myoclonus was suggested by cessation of myoclonus after GBP discontinuation despite continued therapy with tramadol.
