Apocrine gland carcinorna is a very rare cutaneous malignancy, By 1987, only about 35 cases are found in the literature. This lesion occurs predominantly in axillae of elderly individuals. It is a mostly asymtomatic nodule or cystic mass. We report herein a case of apocrine gland carcinoma in the right mammary area of 68-year-old male. Histopathological findings were characterized by a glandular arrangement of large cells with abundant. eosinophilic cytcplasm and evidence of decapitation secrtion. The cytoplasm of the tumor cells contained PAS-positive, diastase-resistant granules and lysozy ne. The patient was treated with wide superficial excision includirig t he enough marginal skin. After 20 months, large mass was cleveloped at. the same site and right axi lary lymph nodes were palpabled. Then we treated him by modified radical mastectomy with regional lymph node dissection.

3.Hypersensitivity myocarditi caused by ceused by cafadroxil.

Hypersensitivity myocarditis is an inflammatory disease of the myocardium usually related to drug allergy. The clinical manifestation may be nonspecific, and the diagnosis is seldom suspected or established during the life. So in most of the reported patients, the diagnosis was made at autopsy. Although, retrospectively, more than 90% of the patients described have had clinically recognizable cardiac involvement before death, tole clinical suspicion of hypersensitivity myocarditis is rare. The presence of nonspecific cardiac finding in a patient receiving any drug associated with hypersensitivity should alert the clinician to the possibility of drug related myocarditis. We experienced a case of hypersensitivity myocarditis accompanying allergic skin lesion in a 58 years old man with a history of taking cefadroxil. Fortunately, the outcome was favorable, therefore we do not have histologic confirmation of the diagnosis.

4.Restriction Endonuclease Analysis of Plasmids and Antimicrobial Resistance Pattern of Staphylococcus Aureus and S. Epidermidis Isolated from Clinical Specimens.

BACKGROUND: Methicillin-resistant Staphylococcus aureus is a major etiologic agent of hospital acquired infection. Coagulase negative staphylococci (CNS) species are previously regarded as contaminants. However nowadays CNS were regarded as an important cause of bacteremia. So in this study we wanted to analyze the patterns of plasmids and antimicrobial susceptibility test of Staphylococcus species isolated from clinical specimens. METHOD: Plasmid DNA was extracted and then processed through restriction enzyme digestion for plasmid analysis of S. aureus and antimicrobial susceptibility, which was done by agar dilution method. For S. epidermidis plasmid analysis was done without enzyme digestion. RESULTS: All of MRSA have 1 to 5 plasmids. There exists 6 patterns of S. aureus plasmid without enzyme digestion. With EcoRI and HindIII digestion pattern were more distinct and clear. For S. epidermidis enzyme digestion is not needed. Antimicrobial susceptibility patterns of S. aureus are simple whereas S. epidermidis showed variable patterns. CONCLUSIONS: For the plasmid analysis of S. aureus restriction enzyme digestion is required and for the S. epidermidis, the pattern of plasmids are variable so without restriction enzyme analysis we can obtain several patterns. Plasmid analysis will be used as a good epidemilogical tool for Staphylococcus.

5.Determinants of North Korean Refugees' Trust in South Korea Health Service

BACKGROUND: The purpose of this study was to investigate the determinants of North Korean refugees' trust in South Korea health service. METHODS: This study surveyed 168 North Korean refugees aged 19 and over living in Seoul, Gyeonggi, Incheon, and Gangwon. The collected data were analyzed by frequency analysis, t-test, analysis of variance, and hierarchical regression analysis to analyze the effect on the trust of refugees. RESULTS: The difference analysis showed statistically significant difference according to duration of residence and employment status. The determinants of North Korean refugees' trust in South Korea health services were duration of residence, basic communication skills, health communication skills, and experience of health service. CONCLUSION: In order to increase the trust of North Korean defectors in South Korea medical services, it is necessary to resolve communication problems and improve the satisfaction of health services. For this, the interpreter service should be activated. And specific education should be given to the North Korean refugees about the health care system and the utilization methods of health service in Korea.

7.Clinical Investigation of 11 Cases of Chronic Eosinophilic Pneumonia Reported in Korea.

BACKGROUND: Chronic eosinophilic pneumonia(CEP) presents with profound systemic symptoms, including fever, malaise, night sweats, weight loss, and anorexia together with localized pulmonary manifestations such as cough, wheeze, and sputum. It is an illness occurring predominantly in women. The chest radiogragh shows fluffy opacities that often have a characteristic peripheral configuration. The hallmark of CEP is the peripheral blood eosinophilia and a prompt response to oral corticosteroid therapy. We investigated characteristics of eleven patients of chronic eosinophilic pneumonia, reported in Korea. METHOD: There were eleven reports of CEP from 1980 to 1996, including three cases experienced in our hospital. The journals were analysed in respects of clinical history, laboratory, and radiographic findings. RESULTS: 1) Male vs. female ratio is 3 : 8. The peak incidence occurred in forty and fifty decades. The atopic diseases were present in 6 cases. Asthma was the commonest manifestation. 2) The presenting symptoms were as follows: cough, dyspnea, sputum, weight loss, fever, general weakness, night sweats, urticaria with the descending incidence. 3) Peripheral blood eosinophilia was present in all patients(mean; 38.4%) and serum Ig E level was elevated in nine patients(mean; 880IU/ml). CONCLUSION: The diagnosis of chronic eosinophilic pneumonia is based on classic symptoms, including fever, night sweats, weight loss with a typical roentgenogram of peripheral pulmonary infiltrates and peripheral blood eosinophilia, and that is confimed by lung biopsy and/or bronchoalveolar lavage. Chronic eosinophilic pneumonia is responsive to corticosteroid promptly and recommended at least 6 months of therapy to prevent relapse.

8.Changes in Plasma and Urine Endothelin Levels During Acute Exacerbation of Asthma.

BACKGROUND: Recent studies have documented increased release of endothelin(ET) during acute attack of asthma. The purpose of this study is to observe the link between plasma level and urinary excretion of each and changes during acute exacerbation. METHOD: Plasma and 24 hour urine were collected from sixteen asthmatics during acute exacerbation, twice ; first day of symptomatic exacerbation and two weeks after treatment. Controls were ten healthy normal subjects. All patients were treated with corticosteroid and beta-2 adrenergic agonist on admission. ET was determined by radioimmmunoassay and had 100% cross reactivity with ET-1, 67% with ET-2, 84% with ET-3, and 8% with Big-ET. RESULTS: Plasma ETs were significantly elevated during acute attack of asthma compared with those in remission and controls. However, there was no significant changes in urine ET concentrations or total ET amounts in 24 hour urine during exacerbation upto two weeks. Those levels of urine ET in asthmatics were still higher than controls. ET concentrations in plasma or urine were not correlated with pulmonary functional parameters and hypoxemia. CONCLUSION: The findings suggests that increased plasma ETs are related with exaggerated release during acute asthma. Urinary ET excretion is increased in asthma. However, urine ET changes during exacerbation should be observed in a larger and longer scale.

9.Descriptive Epidemiology of Cholangiocarcinoma and Clonorchiasis in Korea.

In 2009, infection with the liver fluke Clonorchis sinensis (C. sinensis) was classified as "carcinogenic to humans" (Group 1) based on its involvement in the etiology of cholangiocarcinoma by the International Agency for Research on Cancer. However, little is known about the descriptive epidemiology of cholangiocarcinoma in Korea. We examined incidence trends of intrahepatic and extrahepatic cholangiocarcinomas, using data from the Korea National Cancer Incidence database for 1999-2005. The prevalence of C. sinensis infection was estimated from a recent population-based survey in rural endemic areas. Cholangiocarcinoma incidence rates are currently rising, even while primary liver cancer incidence rates are decreasing. Annual percent changes in cholangiocarcinoma incidence rates were 8% for males and 11% in females. Known areas of C. sinensis endemicity showed high incidence rates of cholangiocarcinoma. The positivity of C. sinensis eggs in stool samples from endemic areas was more than 25% of adults tested during 2005-2008. From a meta-analysis, the summary odds ratio for cholangiocarcinoma due to C. sinensis infection was 4.7 (95% confidence interval: 2.2-9.8). Approximately 10% of cholangiocarcinomas in Korea were caused by chronic C. sinensis infections. More specific policies, including health education and an extensive effort for early detection in endemic areas, are needed.

10.Change of Hepatic Volume after Selective Bile Duct Ligation: An Experimental Study in the Rabbit.

PURPOSE: To evaluate the role of bile duct obstuction in the development of atrophy of the liver, using ananimal model. MATERIALS AND METHODS: Seven rabbits were divided into two groups : group 1(n=5), in which therewas selective bile duct ligation, and group 2(n=2), which underwent a sham operation. Each group was evaluated using CT for changes in hepatic volume after selective bile duct ligation or a sham operation. In group I, the diameter of dilated bile duct was measured 2, 4, 8, 12 and 16 weeks after bile duct ligation, while gross andhistologic change were evaluated in all cases. RESULTS: In group 1, bile duct dilatation was seen on CT two weeks after selective bile duct ligation, and did not change significantly during follow-up. In four of five cases, CT revealed no evidence of significant atrophy of the involved segment. Pathologic specimens, however, revealed dilatation of the bile duct, periductal fibrosis, infiltration of chronic inflammatory cells, and periportalfibrosis. One of five cases showed segmental liver atrophy after selective bile duct ligation. In addion to the above pathologic findings, there was obstruction of the portal vein by foreign body reaction. In group 2, no evidence of dilated bile duct or liver atrophy was revealed by CT or pathologic specimen after a sham operation. CONCLUSION: During long-term follow-up of 16 weeks, obstruction of the bile duct did not play a major role in the development of lobar atrophy in the rabbit.
