3.A Case of Type IV Hyperlipoproteinemia Associated with Xanthoma Tuberosum and Diabetes Mellitus.

We report a case of type IV hyperlipoproteinemia associated with xanthoma tuberosum and diabets mellitus in a 48 year old female patient. She had several yellowish-brown papules on the knees and palms. The blood glucose level was elevated and serum lipid and lipoprotein analysis revealed patterns of type IV hyperlipoproteinemia. Histopathologic findings of yellowish-brown papules on the knee showed the aggregation of foam cells in the dermis. She has been treated with diet control and hypolipidemic drug(lovastatin and procetofen) and is under our continuing supervision.

4.Ambulatory Blood Pressure and Heart Rate of the Workers Exposed to Industrial Noise.

OBJECTIVES: This study was carried out to evaluate the effects of industrial noise on blood pressure and heart rate. METHODS: Resting blood pressure, hearing loss, and general characteristics of the 102 subjects who were engaged in a factory in Pusan were measured from March to June for two years, in 1998 and 1999. With noise dosimeter, noise exposure level was measured from 8 A.M. to 4 P.M. Ambulatory blood pressure and heart rate were also measured every 30 minutes from 8 A.M. to 10 P. M. RESULTS: Controlling for age, smoking, and Quetelet's index, in subjects of under 40 years old exposed to higher than 85dBA, noise exposure and systolic blood pressure had a statistically significant correlation, and the same result was obtained in all subjects. The daily variability of ambulatory blood pressure and heart rate were observed in older than 40 years old group, but only heart rate in under 40 years old. CONCLUSIONS: The blood pressure and heart rate would be elevated when the workers exposed to noise. And in under 40 years old, the systolic blood pressure was elevated to the workers exposed to higher than 85dBA.

6.Cerebral Aneurysms in Childhood and Adolescence.

The incidence of intracranal aneurysm is rare in childhood and adolescence. During the period from January 1982 through December 1989, 793 aneurysms were operated on the our department. Of these 8 aneurysms occurred 8 patients under age of 18 years. The average age was 11.6 years(range 4 to 18 years). There were 6 male and 2 females. Presenting sings and symptoms included SAH in 6 aneurysms and focal neurologic deficits in 2 aneurysms. All aneurysms were surgically treated : direct clipping was performed in 6, aneurysmorraphy in 1, trapping alone in 1. The postoperative result were excellent in 5, good in 2, and fair in 1. The rarity of such lesions in childhood and adolescence and their pathogenesis are reviewed briefly.

7.A Case of Brainstem Encephalitis.

We experienced a case of brainstem encephalitis that is possibly caused by herpes simplex virus infection in a 3 year-old girl. The diagnosis was made by clinical symptoms, neurologic exam, serologic test, brain CT, brain MRI and brainstem biopsy. Herpes simplex virus specific antibody was detected from cerebrospinal fluid and the titer was 11.78 by enzyme immunoassay. Stereotaxic brainstem biopsy was done for differenciate from brainstem glioma at 24th hospital day. Patient was treated with acyclovir and improved in association with neurologic sequallae at 3 months later. Brief review of literatures was made.

8.A Case of Traumatic Giant Aneurysm.

The authors are reporting traumatic giant aneurysm arising from supraclinoid portion of internal carotid artery. The patients was a 22-years old man who had suffered a major head injury in a traffic accident. On admission he was comatous and there were multiple abrasion over the body and severe hemorrhage comming from the left ear with open fracture in left parietal. 4 weeks later, computed tomographic scan demonstrated large well enhanced mass in the suprasellar region. Carotid angiography showed giant aneurysm fed by supraclinoid portion of internal carotid artery. After frontotemporal craniectomy, direct approach was performed using surgical microscope.

9.Clinical Features in Relation to Discordance of Sleep and Waking EEG in Epileptic Children.

PURPOSE: Some patients showing epileptiform discharges were noted only in sleep or only in waking electroencephalogram(EEG). But children are difficult to take both sleep and waking EEG's due to poor cooperation. We carried out this study to determine the discordance between sleep and waking EEG's in epileptic children and its correlation with clinical features. METHODS: This study included 432 epileptic children who had been admitted or visited out patient clinic of the department of pediatrics, Pusan National University Hospital from Jan. 1994 to July 2001. The medical records and EEG records of 432 epileptic children were reviewed. Both sleep and waking EEG were recorded in 142 recordings of 97 epileptic children. Retrospective analysis of medical records and EEG results were performed. The analysis included the discordance of EEG findings, age at EEG recording, age at first seizure, sex, seizure types, underlying causes, and number of EEG recordings. RESULTS: Among 142 EEG recording cases, 46 cases showed normal recording in both sleep and waking states, 41 cases abnormal only in sleep, 8 cases abnormal only in waking, 47 cases abnormal in both sleep and waking. 49 cases(34.3%) showed discordance of sleep and waking EEG's. The discordance was correlated with age at diagnosis, especially the highest discordance rate in the age between 3 and 10 year(P<0.05). There was no significant difference in sleep and waking EEG's for age at first seizure, sex, seizure types, underlying causes, and number of EEG recordings. CONCLUSION: Despite of the difficulty in taking both sleep and waking EEG recordings, both sleep and waking EEG's should be carried out in the patients between 3 and 10 years of age.

10.Subgenus classification of Acanthamoeba by riboprinting.

Subgenus classification of Acanthamoeba remains uncertain. Twenty-three reference strains of Acanthamoeba including 18 (neo)type-strains were subjected for classification at the subgenus level by riboprinting. PCR/RFLP analysis of 18S rRNA gene (rDNA). On the dendrogram reconstructed on the basis of riboprint analyses, two type-strains (A. astronyxis and A. tubiashi) of morphological group 1 diverged early from the other strains and were quite distinct from each other. Four type-strains of morphological group 3, A. culbertsoni, A. palestinensis, A. healyi were considered taxonomically valid, but A. pustulosa was regarded as an invalid synonym of A. palestinensis. Strains of morphological group 2 were classified into 6 subgroups. Among them, A. griffini which has an intron in its 18S rDNA was the most divergent from the remaining strains. Acanthamoeba castellanii Castellani, A. quina Vil3, A. lugdunensis L3a, A. polyphaga Jones, A. triangularis SH621, and A. castellanii Ma strains belonged to a subgroup, A. castellanii complex. However, A. quina and A. lugdunensis were regarded as synonyms of A. castellanii. The Chang strain could be regarded as A. hatchetti. Acanthamoeba mauritaniensis, A. divionensis, A. paradivionensis could be considered as synonyms of A. rhysodes. Neff strain was regarded as A. polyphaga rather than as A. castellanii. It is likely that riboprinting can be applied for rapid identification of Acanthamoeba isolated from the clinical specimens and environments.
