1.Pathologic Study on Carcinomas of Extrahepatic Biliary Tract.

The authors reviewed surgical materials from 20 patients with carcinoma of the extrahepatic biliary system, and a correlation between macroscopic appearance of the tumors with various clinical features and histopathologic findings was made. Microscopically, the tumors were classified into four types; Four (21%) patients had polypoid tumors, six (32%) had nodular growths, five (26%) were scirrhous constricting in type, and four (21%) had diffusely infiltrating type. Histologically all the differentiation in two cases. The degree of differentiation of the tumors was classified into 3 types: 11 (55%) patients were well differentiated, 3(15%) were moderately well and 6(30%) were poorly differentiated. All polypoid tumors were well differentiated and had low stage. No correlation in the degree of differentiation of the tumor with the stage was present. No correlation in clinical symptoms, duration of symptoms, laboratory findings with morphologic findings of the tumors was noted.
2.A Study for Culture Condition of Chlamydia pneumoniae.

BACKGROUND: Chlamydia pneumoniae, a new species of the obligate intracellular Chlamydia, has been recognized as a significant pathogen that causes infection of the human respiratory tract and has recently been associated with coronary atherosclerosis. Diagnosis of infections with C. pneumoniae is problematic, because the syndrome usually presents few distinguishing features and culture of the organism is far more difficult than other Chlamydia species. To further improve the cell culture isolation and passage of C. pneumoniae organisms. we have studied several chemical and physical factors that might affect their viability and growth. METHODS: C. pneumoniae strain (TW-183) was obtained from the Centers for Disease Control, Atlanta. Ga. First we compared McCoy HeLa-229, and HEp-2 cells in the search for a more efficient and practical cell culture system. The growth rate of C. pneumoniae was assessed by the effects of diethylaminoethyl-dextrin, by the adequate centrifugation force and time, by the growth promoting effect of cycloheximide, and by the optimal incubation time. All of the results were evaluated by the indirect immunofluorescent stain using the genus-specific monoclonal antibody(HYMo 1-1) to Chlamydia. RESULTS: The HEp-2 cell was the most efficient for culturing C. pneumoniae and the inclusion bodies in monolayer were increased with DEAE-dextran pretreatment at 30microgram/ml. Also application of a centrifugal force of 1.500 xg for at least 15 minute during inoculation enhanced the growth of C. pneumoniae. The best concentration of cycloheximide in the culture medium for host cell cytostasis was 1microgram/ml. The yields of organisms were greater when the cultures were harvested at 48 hours. CONCLUSIONS: We suggest that this system may make it more practical for laboratories to culture for C. pneumoniae.

6.Pathologic Correlations of Various Indications for Large Loop Excision of Transformation Zone(LLETZ) in Patients with Cervical Lesion.

OBJECTIVE: This study was performed to evaluate the effects of mutiple indications and age on the likelihood of finding cervical lesion in large loop excision of transformation zone(LLETZ). METHOD: We reviewed the medical records of 471 evaluable LLETZ of cervix for cervical neoplasia at the Yonsei University College of Medicine, from April 1996 to August l998. The indications for the LLETZ were grouped as being for treatment(biopsy-proven disease)(indication A), significant discrepancy among cytologic and histologic results(indication B), unsatisfactory colposcopic finding(transformation zone not fully visualized)(indication C), possible microinva- sion(indication D), and combinations of the above. RESULTS: The prevalence rates of cervical lesion in performed LLETZ for indication A alone, for indication B alone, for a combination of indication A and C, for a combination B and C, and for indication D alone were 85.7%, 56.5%, 82.3%, 42.6%, 95%, respectively. The data were then reanalyzed to determine the likelihood of finding high-grade disease on LLETZ specimen. Overall, those with preoperative high-grade cytologic or histologic characteristics were much more likely to have high grade disease(78.1%) than were those with preoperative low-grade cytologic or histologic characteristics(27.2%)(p<0.001). The influence of age on the likelihood of identifying diasease on the LLETZ is undertermined. CONCLUSION: Neither age nor the preoperative grade of disease are good discriminators of the likelihood that disease will be found on a LLETZ specimen, However, patients with preoperative high-grade disease are much more likely than those with preoperative low-grade disease to have high-grade dysplasia or invasive cancer on LLETZ.

7.Distribution of Human Papillomavirus 52 and 58 Genotypes, and Their Expression of p16 and p53 in Cervical Neoplasia.

BACKGROUND: This study investigates the prevalence of human papillomavirus (HPV) 52 and 58 genotypes among women residing in Busan, and the expression of p16 and p53 proteins in cervical neoplasia with HPV 52 and 58 infections. METHODS: A total of three hundred fifteen cases were analyzed using the HPV DNA chip test for HPV genotypes, and of these, we retrospectively examined p16 and p53 expression in 62 cases of cervical tissues infected with HPV 52 and 58 using immunohistochemistry. RESULTS: HPV 52 and 58 genotypes were identified in 62 (54.9%) out of 113 high-risk, HPV-infected cases. Of the cases examined, there were 19 single HPV 52 infections (16.8%), 23 single HPV 58 infections (20.4%), 4 multiple HPV 52 infections (3.5%), and 16 multiple HPV-58 infections (14.2%). Immunoreactivity of p16 and p53 was observed in 41 (66.1%) and 23 (37.1%) of the 62 cases of cervical neoplasia infected with HPV 52 and 58 genotypes, respectively. CONCLUSIONS: This study demonstrates a high prevalence of HPV 52 and 58 genotypes, in addition to HPV 16, among high-risk strains of cervical neoplasia in Korea. These findings suggest that development of more vaccines would be beneficial for the prevention of the various HPV genotypes.

8.Clinical Characteristics of Late Adult-Onset Elderly Atopic Dermatitis

Background:
Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by typically distributed eczematous skin lesions and is classified according to the period of life. Recently, the number of elderly patients newly diagnosed with AD have been increased in industrialized countries.
Objective:
This study aimed to analyze the clinical characteristics of late adult-onset elderly AD.
Methods:
Medical charts and photographs of patients aged 60 years or older (n=36) who visited the Department of Dermatology with chronic and recurrent dermatitis were analyzed retrospectively. The anatomical distribution of AD lesions was evaluated by categorizing them into head and neck, trunk, and upper and lower extremities. The results of laboratory tests related to AD were also analyzed.
Results:
Thirty-six patients were diagnosed with elderly AD according to the diagnostic criteria established by Hanifin and Rajka, with a male predominance. All patients had late adult-onset AD, with the primary onset of AD occurring after the age of 40 years. Eczematous erythroderma, lichenification, dryness, and pruritic papules were observed on the trunk and extremities. The proportion of patients with extrinsic AD was 54.8%, and that of patients with one or more allergic diseases was 25.0%. Patients with late adult-onset elderly AD had lesions in the sagging areas, such as the back or extensor surface of the extremities.
Conclusion
Male predominance, reverse sign, and involvement of the extensor areas are characteristic features of AD in the elderly. These results indicate that late adult-onset AD in elderly patients is a distinct subgroup of patients with AD.
9.The Study on the Relationships Between Inhalation Volume and Exposure Duration and Biological Indices of Mercury among Workers Exposed to Mercury.

This study was conducted to evaluate the relationships between the environmental exposure and biological monitoring among workers exposed to metallic mercury. We interviewed each workers to get the medical history including previous hazardous occupational history we measured the respiration rate and tidal volume of each worker in order to calculate the 8-hour inhaled mercury of workers. And we wanted to evaluate the effect of exposure duration to mercury concentrations in blood and urine as biologic exposure indices of metallic mercury. The regression and correlation analysis were done to the relationships of 8-hour inhaled mercury are mercury in blood and urine. The results were as follows; 1. The subjects were 35 fluorescent lamp manufacturing workers. The mean age of subjects was 24.8 years old, and the mean work careers of workers was 1.19 years. 89% of the total was consisted man. 2. The correlation coefficients between 8-hour inhaled mercury and mercury in blood and urine were higher than that of only considered air mercury concentration. 3. The correlation coefficients of 8-hour inhaled mercury and mercury in blood and urine were above 0.9 in workers who had exposed to mercury more than 1 year. 4. The R-square value and -value of regression analysis between the 8-hour inhaled mercury and mercury inn blood and urine was also higher in workers who had exposed to mercury over 1 year than in workers who had less than 1 year working experience. The important results or this study were that relationships between the 8hr-inhaled mercury and mercury in blood and urine was very high than that with air mercury concentration only. And the results were very apparent when considering workers 1 year or more. Therefore we concluded that the work career and respiratory volume of each individuals should be considered in evaluation the results of biological monitoring of workers exposed to metallic mercury.
