4.Alopecia Associated with Occult Syringoma.

Occult syringoma means a clinically inapparent tumor of the swea gland that are histologically similar but not identical with the common syringoma. They may occur in a variety of inflammatory skin lesions and cutaneous neoplaams as a reactive process. We report herein a case of diffuse alopecia areata associated with occult syringoma in the scalp of a 57-year-old man. Our patient developed progressive, fairly well circ amscribed patches of alopecia and diffuse hair loss over a 1 month period. Histopathological examinaiion revealed non-scaring alopecia and a proliferation of eccrine ductal structures in the upper dermis. These ductal structures resembled syringoma. Only minimal focal fibrosis was occasionally observed arcund the hair follicles. We believe that occult syringona is a reactive process and is secondary to active inflarnmation or dermal fibrosis. It has been most commonly observed in scarring alobiecias but plays no significant role in the etiology of hair loss.

6.Intradural Extramedullary Tuberculoma Mimicking En Plaque Meningioma.

A 24-year-old man with tuberculosis meningitis developed acute paraplegia and sensory disturbances 5 weeks after receiving conventional antituberculous therapy. Magnetic resonance imaging revealed an intradural extramedullary long segmental mass mimicking en plaque meningioma at the T2-T6 vertebrae levels. Prompt surgical decompression was performed. A histology examination of the mass revealed a tuberculoma. After surgery, the patient showed improved motor power and a normal bladder function. Intradural extramedullary tuberculoma of the spinal cord is rare complication of tuberculosis meningitis, which can occur as a response to conventional antituberculous therapy.

7.A Case of the Malformation of Azygos and Hemiazygos System.

A malformation of azygos and hemiazygos system were observed from the cadaver for anatomy practice in College of Medicine, Chungnam National University. The observing results were as followings ; Hemiazygos vein was connected with left superior vena cava and drained into coronary sinus, which engorged enough to fill up with blood about 20cc to 30cc. Communicating pathway from hemiazygos vein to azygos veln wasn't found the diameter of left brachiocephalic vein was very small, about 2mm to 3mm. And, the diameter of left internal jugular vein was smaller than that of right infernal jugular vein. In this case, the azygos and hemiazygos system ceased its development at early embryologic stage(around 7 week embryo) and didn't progress into next stage.

8.CHANGE OF PULSE RATES DURING SURGICAL EXTRACTION OF MANDIBULAR IMPACTED 3rd MOLAR.

Changes of human pulse rates are influenced by psychological factor as well as systemic disease. Pulse rates tend to increase in the state of fear, pain, anxiety, and excitement. We performed this study for the evaluation of psychological state during the surgical extraction of mandibular impacted 3rd molar. Total 42 patients without systemic compromised condition were selected and pulse rates were checked with pulse oximeter(Nellcor, N-180) during the initial sitting state at dental unit chair, local anesthesia, draping and operation(5-minute interval). Male patients were 22, female 20. Age ranged from 13 to 57, with a mean of 26.9. Operative time(from incision to end of suture) ranged from 4 minutes to 32 minutes, with a mean of 11.1 minutes. The most significant elevation of pulse rates were observed during draping, the next at 10 minutes after incision(P< 0.05). Elevation of pulse rates during the anesthesia was not more than preoperative state and variable(P> 0.05).

10.Massive Postoperative Bleeding: A Case Report.

While oral and maxillofacial surgery such as orthognathic surgery, TMJ surgery is in progress, there always exists a risk of bleeding from maxillary artery, and this perplexes the surgeon in operation. In case of massive bleeding, it is mostly very difficult or even impossible to take an immediate action in order to stop bleeding. Even when hemostasis is possible by applying such methods as the use of local hemostatics, pressing, electrocoagulation or direct ligation, there is a high risk of secondary bleeding. Therefore, in case there is bleeding from maxillary artery, it is the best to restrain bleeding completely either by the ligation of the artery in bleeding in operation fields or by the ligation of the external carotid artery.
