2.The Relationship between Febrile Convulsion and Temporal Lobe Epilepsy: Is Febrile Convulsion a Preferential Association with Temporal Lobe Epilepsy?.

BACKGROUND: Although a history of febrile convulsion (FC) is common in epilepsy patients, the preferential associa-tion of febrile convulsion with temporal lobe epilepsy (TLE) is not clear. METHODS:We obtained the FC data from "Paik and Bongsang hospital" epilepsy clinic. We classified epilepsy syndromes into generalized epilepsy (GE), temporal lobe epilepsy (TLE), extratemporal epilepsy (ETLE), unclassified partial and undetermined epilepsy by standardized criteria. The incidence of antecedent FC was evaluated in relation to the epilepsy classifications. We calculated kappa values for inter and intra observer reliability for the classifications of epilepsy syndromes. RESULTS: The agreement of epilepsy classifications were reliable (intra-observer kappa value=0.78, inter-observer kappa value=0.77). Thirteen percent of the studied patients (72/537) had a history of FC and 38% of FC (27/72) were complex types of FC. TLE was more likely to be preceded by FC 25% (42/166) than ETLE 6% (12/189), p<0.05 or GE 13% (12/93), p<0.05 and 85% of complex FC (23/27) preceded TLE. GE however, was more likely to have non-complex FC 100% (12/12) than partial epilepsy 55% (32/58), p<0.05. CONCLUSIONS We therefore conclude and agree that FC should be preferentially associated with TLE.

Ovarian granulosa cell tumors are uncommon low-grade malignancies. They are characterized by their long history and their tendency to recur years after an apparent clinical cure. Only a small percentage of granulosa cell tumors metastasize, and they rarely do so to the liver. In this paper, we present a case of hepatic metastasis treated by surgery and BEP combination chemotherapy, with a brief review of corresponding literatures.

5.Effects of Multiple Cyclic Episodes with Short Ischemia and Reperfusion on the Distribution of NF-kappa B, AP-1, Bcl-2, and Bax in Rectus Femoris Muscles of Rats.

The present study was designed to observe the expression patterns of NF-kappa B and AP-1, redox-sensitive transcription factors, and Bcl-2 and Bax, apoptosis repressing and promoting factors, respectively, upon repetitive cycles of short ischemia and reperfusion. Nine and thirty five weeks old Sprague-Dawley rats were subjected to the 3, 6, and 10 cycles of the ischemic process for 5 minutes followed by reperfusion for 5 minutes. The rats were divided by 5 groups, according to the time after treatment, such as 0, 3, 6, 24 and 72 hours. For short ischemia and reperfusion, left common iliac artery was occluded 3, 6, and 10 times for 5 minutes of ischemia followed by 5 minutes of reperfusion using rodent vascular clamps and left rectus femoris muscles were removed. The expression profiles and distribution of NF-kappa B, AP-1, Bcl-2, and Bax which were observed using immunohistochemical staining methods with 6 microgram thick paraffin sections of the rectus femoris tissue were as follows: The distribution of NF-kappa B was increased as the cycles of ischemia and reperfusion increased up to 3 hours after treatment. This phenomenon was prominent in 35 weeks-old rats. The distribution of AP-1 was increased as the cycles of ischemia and reperfusion increased up to 3 hours after treatment. This phenomenon was prominent in 9 weeks-old rats. The distribution of Bcl-2 was decreased as the cycles of ischemia and reperfusion increased up to 3 hours after treatment. The extent of such reduction was more prominent in 35 weeks-old rats than 9 weeks-old rats. The distribution of Bax was increased as the cycles of ischemia and reperfusion increased up to 3 hours after treatment. After 3 hours of treatment, Bax positivity was gradually decreased in 9 weeks-old rats, but increased in 35 weeks-old rats to reach a peak at 24 hour after reperfusion. The extent of enhancement in 9 weeks-old rats was higher than that in 35 weeks-old rats. In summary, multiple episodes of short ischemia and reperfusion altered the expression profiles of NF-kappa B, AP-1, Bcl-2, and Bax in the rectus femoris muscle at the similar extents in 9 and 35 weeks-old rats. Such alterations were more more increased when the episodes were more repeated.

6.Weight-related Factors Associated with Active Attitude Toward Weight Loss in Overweight Children and Parents.

Background: The purpose of this study was to find the weight-related factors related to active attitude towards weight loss in children who are overweight or obese and their parents. Methods: A total of 252 elementary school children who were overweight or obese and their parents were surveyed on weight-related attitudes for one month in May 2003 in Kimhae. The definition of overweight and obesity was based on age and sex-specific Korean children's BMI references established in 1998. Results: Among the total, 64.5% of children and 76.4% of parents thought frequently about losing weight in children. Among the total, 52.8% of children and 47.6% of parents tried losing weight in children more than three times. In children, the factors related to frequent thoughts of losing weight were gender, body mass index (BMI), body dissatisfaction index, and the score of eating attitude (P<0.05). Parents' thought about their children's losing weight and the efforts to persuade their children to lose weight were related to children's frequent thought about losing weight. The children's factors related to trying to lose weight by themselves were age, BMI, and the score of eating attitude. The parent's factors related to children trying to lose weight were body dissatisfaction index of their children by parents, parent's eating attitude score, parent's thought about children's losing weight and making efforts to persuade children to lose weight by parents. The children's eating attitude was related to frequent thoughts about losing weight (odds ratio 1.05, 95% CI 1.03~1.07) and trying to lose weight more than 3 times (odds ratio 1.02, 95% CI 1.003~1.04) after adjusting for other factors. Likewise, parents' effort to persuade their childrento lose weight was related to children's effort to lose weight (odds ratio 2.62, 95% CI 1.34~5.1). Conclusion: Active attitude towards losing weight in overweight or obese children was related to parent's attitude to wards losing weight.

8.Morphometrics of the Medial Longitudinal Arch of Foot in Korean.

In recent times the modification and injury of foot are shown due to various activity. It is essential to have detailed knowledges about the anatomical structure of medial longitudinal arch of foot for doing orthodigita or making shoes to prevent variation of medial longitudinal arch of foot. This study aimed to measure the constitution and to suggest the index of medial longitudinal arch of foot. Fifty four feet (30 M/24 F) obtained from the collection of the Department of Anatomy and Cell Biology of College of Medicine, Hanyang University were studied. We measured the length of foot, the height of foot, and the first ray angle. Also, we found the index of medial longitudinal arch and compared this results with various races anthropologically. Each items was analyzed using SPSS win 13.0. The length of foot was 211.12+/-13.02 mm and the height of foot was 39.03+/-6.27 mm. There was statistically significant difference between the values of male and female and among the age groups (p>0.001). The first ray angle was 24.98+/-2.16 degrees. There was statistically significant difference between the values of male and female (p>0.001). The medial longitudinal arch index was 18.43+/-2.33. The date shows clear distinction between the value of index in this study and the American's value of index as a result of comparing races. The results of this study would be useful to clarify the characteristics of the medial longitudinal arch.

9.The Clinical Significance of Follow Up SCC Levels in Patients with Recurrent Squamous Cell Carcinoma of the Cervix.

PURPOSE: To investigate the clinical usefulness of a follow-up examination using serum squamous cell carcinoma antigen (SCC) for the early detection of recurrence in patients treated for cervical squamous cell carcinoma. MATERIALS AND METHODS: 20 patients who were treated for recurrent cervical squamous cell carcinoma between 1997 and 1998, who had experienced a complete remission after radiotherapy and who underwent an SCC test around the time when recurrence was detected, were included in this study. The levels of SCC were measured from the serum of the patients by immunoassay and values less than 2 ng/mL were regarded as normal. The sensitivity of the SCC test for use in the detection of recurrence, the association between the SCC values and the recurrence patterns and the tumor size and stage, and the temporal relation between the SCC increment and recurrence detection were evaluated. RESULTS: The SCC values were above normal in 17 out of 20 patients, so the sensitivity of the SCC test for the detection of recurrence was 85%, and the mean and median of the SCC values were 15.2 and 9.5 ng/mL, respectively. No differences were observed in the SCC values according to the recurrence sites. For 11 patients, the SCC values were measured over a period of 6 months before recurrence was detected, and the mean and median values were 13.6 and 3.6 ng/mL, respectively. The SCC values of 7 patients were higher than the normal range, and the SCC values of the other 4 patients were normal but 3 among them were above 1.5 ng/mL. At the time of diagnosis, the SCC valuess were measured for 16 of the 20 recurrent patients, and the SCC values of the patients with a bulky tumor (> or =4 cm) or who were in stage IIb or III were higher than those of the patients with a non-bulky tumor or who were in stage Ib or IIa. CONCLUSION: The SCC test is thought to be useful for the early detection of recurrence during the follow up period in patients treated for cervical squamous cell carcinoma. When an effective salvage treatment is developed in the future, the benefit of this follow-up SCC test will be increased.
