2.Successful Treatment of Pyoderma Gangrenosum with Prednisolone, Cyclosporine, and Dapsone.

Pyoderma gangrenosum is an uncommon, chronic, recurrent cutaneous ulcerative disease. It is difficult to diagnosis and is frequently associated with systemic diseases. There is no specific therapy, nor effective uniform therapy. Systemic treatment with corticosteroids or cyclosporine, alone or together, appeared to be effective in many cases. However, prolonged therapy is associated with significant side effects, and in refractory cases, immunomodulatory therapy is needed. We report a case of pyoderma gangrenosum, which developed in a 41-years-old woman, who showed no symptoms of having an underlying systemic disease. The conduction was successfully treated with prednisolone, cyclosporine and dapsone.

4.Clinical Observation on the Renal Tuberculosis.

Renal tuberculosis is one of the most frequent disease in the urological field of Korea for a long period of time. A clinical observation of renal tuberculosis was made on the 50 cases admitted to the Department of Urology, Korea University Hospital from January 1968 to December 1975. The results summarized as follows: 1. The ratio of number of renal tuberculosis cases admitted against total number of urological inpatients is decreasing significantly every years. 2. 31 cases out of 50, belonged to the age group of 21 to 40. 3. Both kidneys were affected in 13 cases, while 21 cases in right side only and 16 cases in left kidney. 4. The important subjective symptoms were as follows: frequency in 54%, painful urination in 40%, flank pain in 40 %, gross hematuria in 38% and asymptomatic in 1 case. 5. 32% of the cases visited to the hospital within 6 months after onset of the symptoms. 6. 56% of the cases were associated with pulmonary tuberculosis. 7. Tuberculous epididymitis were complicated in 3 cases, bone tuberculosis in 1 case and squamous cell carcinoma of the bladder was found in 1 case. 8. Increased ESR were found in 42 cases and 43 cases were within normal range of BUN. 9. Urinary findings revealed proteinuria in 94%, hematuria in 74% and pyuria in 76%. Urine AFB was found in 26 cases (52%). 10. 18 cases of AFB culture was done with drug sensitivity test, which showed that INH was the most sensitive drug, but PAS and streptomycine were sensitive in high concentration of the drugs only. 11. On the urogram, non-visualization of the kidney was found in 20 cases and delayed visualization in 15 cases while abnormal ureteral findings noted in 18 cases. 12. Nephrectomy was performed in 23 cases out of 31 operated cases and nephrostomy was done in 7 cases. 13. Histopathological findings of 23 nephrectomized kidneys disclosed parenchymal tuberculous nodules in all cases and severely atrophic changes in 3 cases.

5.Clinical Observation on Bladder Rupture.

For 10 years and 10 months duration from January 1966 to October 1976. 28 patients with bladder rupture (19 patients had intraperitoneal bladder rupture and 9 patients had extraperitoneal rupture) were observed, among 131 patients hospitalized with urogenital injuries. 1) In this series, the bladder rupture (28 patients) formed 21.4 per cent of the total number of urogenital injuries (131 patients). 2) The 28 patients comprised 22 male and 6 female cases. Three patients were belonged to the age group of 0 to 19 years old, 16 cases were 20 to 39 years old, 4 were 40 to 49 years old, 4 were 50 to 59 years old, and one was 62 years old. 3) Traffic accidents were responsible for the injury in 13 patients, direct blow in 4 patients, industrial accidents in 3 patients, iatrogenic injury in 2 patients, fall down in one patient and in one patient the bladder rupture developed during delivery. Spontaneous bladder ruptures were seen in 4 patients. 4) Of the 28 patients, 15 bad an associated fractured pelvis. Fractured of the both pubic rami were most frequent: 7 cases, unilateral pubic ramus fractures were the next, and acetabular fractures: 2 cases, separation of the symphysis pubis: 2 cases, fracture of wing of the ilium. one case and communized pelvic ring fracture : one case were also noted. 5) Diagnosis was made by retrograde cystography, cystoscopy and laparotomy. Ninety three per cent of the diagnosis was made by retrograde cystography. 6) Ten patients with intraperitoneal rupture and 8 patients with extraperitoneal rupture were treated by operative repair of the defect with suprapubic cystostomy and drainage of the perivesical space. Six patients with intraperitoneal rupture and one patient with extraperitoneal rupture were treated by operative repair of the defect with indwelling urethral catheter and drainage of the perivesical space. The remaining 3 patients died soon after arrival to hospital with heavy injury of several visceral organs and one died of preexisting bladder tumor. 7) There was a 14. 3 per cent mortality rate. Early diagnosis and adequate treatment of bladder rupture is essential to reduce mortality.

6.Clinical Observation on the Cryptorchidisms.

During the last 10 years 8 months clinical observation has been done on 43 cases of cryptorchisms. 1. The cryptorchism has relatively high incidence rate among the anomalies of genitourinary tract (27. 2%). 2. The age group, most frequently seen, was between 6 to 10 and the average age visited the hospital at the first time is 11.4 years. It means 4 t 6 years later than age for the adequate treatment required. 3. Atrophy of the seminiferous tubules, interstitial fibrosis and poor or absent spermatogenesis were noted on the 8 cases of cryptorchid biopsies. 4. Hormonal therapy (Puberogen) was done on 16 cases and 8 cases were responded. But there was no complete descent of testis through this hormonal therapy.

7.A Case of Neurilemmoma in the Nasal Vestibule.

A neurilemmoma is a rare nerve sheath tumor in dermatology. The tumors are usually solitary and affect woman more frequently than men, but with no particular affinity for any age group. It occurs commonly along the main nerve trunks of the extremities, especially the flexor surface of the arms, wrists, and knees. We report a case of neurilemmoma on the left nasal vestibule of 47-years old woman. The tumor was a solitary, pinkish, polypoidal papule which gradually increased in size. A review of the literature revealed extremely rare documented cases of neurilemmoma occurring in the nasal vestibule.

8.A Case of Actinic Keratosis Treated by Topical Photodynamic Therapy with Low Intensity Dye Laser.

Photodynamic therapy (PDT) is a treatment modality using highly reactive oxygen intermediates generated by the interaction of light with a photosensitizer. It has been shown to be an effective treatment for the various cutaneous and noncutaneous malignancies. It is efficient for the curative and palliative treatment of epithelial skin tumors in situ, or early invasive lesions. We treated bowenoid actinic keratosis on the left hand with topical PDT using 5-aminolevulinic acid (ALA) and a low intensity (3.5 J/cm2) dye laser. After three sessions, the lesion had been completely cleared. We report a case of bowenoid actinic keratosis in 74-year-old man, who was treated with low intensity (3.5 J/cm2) dye laser following a topical application of 5-ALA.

9.A Case of Melanoacanthoma.

Melanoacanthoma is a variant of seborrheic keratosis which was first introduced by Mishima and Pinkus in 1960. This is a very rare and benign, epidermal tumor. Clinically, it is usually manifested in a pigmented, verrucous, round or oval plaque and nodular lesion. This is characterized histologically by nonpigmented keratinocytes and numerous large, richly dendritic melanocytes. We describe a 68-year-old man who suffered from a black-colored plaque on his right shin for 5 years. Melanoacanthoma was diagnosed by characteristic histopathologic findings.

10.Clinical Change of Mycoplasma Pneumonia.

PURPOSE: It was noted some changes of clinical manifestations of mycoplasma pneumonia. we reviewed and compared these changes and saught any clues causes for proper dignosis and treatment. METHODS: We divided patients with mycoplasma pneumonia into two groups, Group 1 (from Jan. to Dec. 1996) and Group 2 (from Jan. to Dec. 1994), and analyzed clinical, radiologic, and serologic differences. RESULTS: Mean age of onset lowered markedly from was 8.34 +/- 2.56 years to 6.91 +/- 3.28 years (P<0.05). In clinical symptoms, high fever lasted longer and gastrointestinal symptoms were more frequent noted group 1. Serologically, high titers of mycoplasma-specific antibody (>1 : 1280) were more frequently observed in group 1 and correlated with severity of clinical manifestations. In radiologic findings, alveolar consolidation were significantly prominent findings in Group (P<0.05). The mean period of response to Roxithromycin was not difference between two groups but longer lasting fever (> or = 3 days) in spite of medication were more prevalent in Group 1 (P<0.05), suggesting increased cases of diminished responsiveness to treatment. CONCLUSIONS: Recently, there was some clinical changes of mycoplasma pneumonia, lowering of onset age, severe clinical symptoms, and more decreased responsiveness to antibiotic treatment. We suggest that it is to neccessary to make some efforts to prevent antibiotics abuse and to decrease the occurrence of resistant strains by introducing of new method for early diagnosis, selective identification of micro-organism and minute sensitivity test for antibiotics.
