3.A Clinical Study on 35 Cases of Tuberculous Osteomyelitis in Long Bones

Tuberculosis of the bone and joints is one of the genralized manifestation or solitary localized lesion. Although the incidence of the tuberculous osteomyelitis of long bone is decreasing with good neutrition, development of preventive medicine and improvment of treatment regimen, it is still one of the common diseases in Korea and must be considered in the differentil diagnosis of common orthopaedic complaints. Thirty five cases of tuberculous osteomyelitis of long bone were studied in our department from Junuary 1973 to December 1984. The results are as follows: 1. Long bone tuberculous osteomyelitis were 35 cases (6.6%) from the overall tuberculous lesion of the bone and joints. (534 cases) 2. Childrean and young adults were common age group. 3. Concomitant pulmonary tuberculosis was found in 11 cases. 4. Radiologic findings showed variable lesions including osteolytic, cystic, sclerotic and destructive forms. 5. Confirmative diagnosis could be made by smear, culture of the lesion and pathologic findings. 6. The overall results treated with curettage and bone graft as well as proper antituberculous medication were satisfactory.

4.Effect of Superoxide Dismutase on Apoptosis in Ischemia-Reperfused Myocardium of Rabbit.

BACKGROUND: Recently involvenment of apoptosis, or programmed cell death, has been suggested in myocardial reperfusion injury. Free radicals are one of the inducers of apoptosis, and superoxide dismutase(SOD), a oxygen free radical scavenger, inhibits apoptotic cell death of neurons. Reperfusion of ischemic myocardium results in a burst of oxygen free radical production, however, it has not been defined that oxygen free radicals mediate apoptosis in myocardial reperfusion injury. This study was undertaken to investigate the role of oxygen free radicals by examining the inhibition of apoptosis by SOD. METHOD: New Zealand white rabbits (n=16) weighing 1.8-20kg underwent 30 minutes of left anterior descending coronary artery occlusion followed by reperfusion for 1 or 4 hours. In control group, bovine serum albumin(5mg/kg) was administered continuously via the left atrial appendage starting 10 minutes before reperfusion and ending simultaneously with reperfusion for 1 hour(n=4) or 4 hours(n=4). In SOD group, bovine erythrocyte SOD(15,000u/kg) was administered starting 10 minutes before reprefusion and ending simultaneously with reperfusion for 1 hour(n=5) or 4 hours(n=3). Ventricles were excised immediately after intervention. Tissues were fixed in 10% buffered formalin and 2.5% glutaraldehyde. Apoptosis was examined by hematoxylin and eosin(H&E) staining, in situ nick end labeling, and transmission electron microscopy. Number of apoptotic cells was evaluated semi-quantitatively on H&E stained section. RESULTS: Evidence of apoptosis was detected in every ischmia-reperfused myocardium, and apoptotic cells were found in the non-necrotic myocardium near areas of contraction band necrosis. In control group, the average number of apoptotic cells was 1.7(range 1.5-2.0)for 1 hour reperfused myocardium and 1.4(range 0.3-2.5) for 4 hours reperfused myocardium per high power field(x400). In SOD group, the average number of apoptotic cells was 0.2(range 0.2 -0.3) for 1 hour reperfused myocardium and 0.3(range 0.2-0.4) for 4 hours reperfused myocardium. There was a significant difference in the number of apoptotic cells between conrol and SOD groups (as a whole group 1.5 +/- 0.2 vs 0.3 +/- 0.1,p<0.01). CONCLUSION: SOD partially, however, singificantly inhibits apoptosis, which suggests that oxygen free radicals may induce apoptosis in ischemia-reperfused myocardium of rabbit.

5.Usefulness of Three - Dimensional CT Angiography as a Confirmatory Diagnostic Test for Scalp Pseudoaneurysms.

OBJECTIVE: We report here on two cases of pseudoaneurysms that were diagnosed using computed tomographic angiography (CTA), with one arising in the superficial temporal artery and the other arising in the occipital artery. METHODS: Case 1: A 29-year-old man presented with a slow growing, pulsatile mass on the left side of his forehead following a blow from a fist 10 days earlier. CTA revealed an 11 x 8 x 11-mm oval pseudoaneurysm arising from the frontal branch of the left superficial temporal artery. Case 2 : A 36-year-old man presented with a pulsatile mass in the right suboccipital region that had been present for the past year. He had no history of direct trauma. CTA revealed a 15 x 8 x 13-mm oval dye-filled sac arising from the right occipital artery. RESULTS: Based on the CTA images, the parent arteries were exposed and ligated proximally and distally to the pseudoaneurysms and then they were completely excised. CONCLUSION: CTA is a minimally invasive technique for making the diagnosis of scalp pseudoaneurysms and CTA is also useful for the preoperative planning.

6.A Case of Gout.

A 56-year- old man showed millet seed to egg sized, yellowish tender papules, subcutaneous mass on the both elbows, dorsa of both hands & feet, and lateral malleoli and helix of right ear for 6 years. the significant laboratory findings were leukocytosis, increased erythrocyte sedimentation rate, glycosuria, hyperuricemia and increased blood glucose level. X-ray findings of both hands & feet demonstrated gouty arthritic patterns. Histopathologic findings of the left lateral malleolar lesion revealed the typical patterns. Histopathologic findings of the left lateral malleolar lesion revealed the typical pattern of gout with needle-shaped urate crystal. Therapy was begun with colchicine & allopurinol but the patient died of renal failure three weeks after the start of therapy.

9.A Case of Granuloma Annulare.

Granuloma, annulare is a chronic dermatosis characterized by skin colored, firm intradermal papules or subutaneous nodules, which may be discrete or arranged in an annular configuration. Histologically the dermis shows well define8 areas of necrobiosis of collagen tissue surrounded. by palisading histiocytes and lymphocytes. There may be lobules of epithelioid cells ani multinucleated giant ce.1s surrounding the necrobiotic areas. Many factors have been implicated in the genesis of this reaction. Recent studies have indicated that delayed hypersensitivity reactions rnay be involved in the pathogenesis of granuloma annulare. We present a typical granulorna annulare on a 16 year-old boy who had flesh colored, hyperkeratotic small nodules arranged in ring fashion on the dorsum of hands and right ear auricle.

10.Congenital Subglottic Stenosis of the Larynx Associated with Tracheoesophageal Fistula: 1 autopsy case.

Congenital subglottic stenosis of the larynx is one of the most common cause of chronic airway obstruction im infancy and childhood. It is defined as narrowing of the space bounded inferiorly by the inferior margin of the cricoid cartilage amd superiorly by the insertion of the fibers of the conus elasticus into the true vocal cords. In case we experienced was a female full-term baby delivered by Cesarean section. The stenosis was believed by hypertrophy of stromal soft tissue and cricoid cartilage in the subglottic area. The lesion was associated with tracheoesophageal fistula of H1 type. A brief review of the literature was done.
