Wegener's granulomatosis is a disease of unknown etiology that is characterized by the clinicopathologic complex of necrotixing granulomatous vasculitis of the upper and lower respiratory tract, glomerulonephritis, and variable degrees of small vessel vasculitis. Recently Antineutrophil Cytoplasmic Antibody (ANCA) has been reported to be a highly specific test for the diagnosis of Wegener's granulomatosis. We have experienced a patient of Wegener's granulomatosis in a 11 year old girl who was admitted with complaints f arthralgia, hematuria, convulsion and associated with otitis media and sinusitis. Serologic test of C-ANCA was positive and histologic findings of the kidney showed crescentic glomerulonephritis with sclerosis and surrounding infiltration of multinucleated giant cells. Patient was treated with pulse methylprednisolone without improvement. The clinical course progressed rapidly and expired due to the renal failure, gastrointestinal bleeding and status epilepticus. A brief review of literatures was made.

A 19-year-old man had a good neurologic recovery from a severe hypothermia(19 degrees C) and a prolonged coma following active infernal rewarming. From inhaling hydrocarbons, he was left unconscious on the cold floor for 24 hours. As soon as the patient was brought into the emergency medical center, he was early evaluated arid treated aggressively. ECG showed Osborn(J) wave on all leads. The temperature o( patient was increased by 2-3 degrees C per hour through active external rewarming (by heating blankets and warm bag) and active infernal rewarming (by airway rewarming, warmed IV fluids, gastrointestinal tract irrigation, and bladder irrigation). The temperature reached 36 degrees C after 6 hours. Active infernal rewarming provides rapid core rewarming with the additional benefit of circulatory support during the period of cardiac instability.

Adult intussusception is rare, and the majority of cases has an underlying cause that requires surgical resection. We report a case of a 39 yr-old man with ileo-ileo-colic intussusception caused by ileal lipoma that was successfully managed by a laparoscopic-assisted surgical maneuver. Using a three-cannula technique, ileo-colic intussusception was reduced laparoscopically. Then, through a 4-cm transverse incision in the right lower quadrant abdomen, ileo-ileal intussusception was reduced manually, and a resection of the tumor-bearing ileal segment and end-to-end anastomosis was performed extracorporeally. Although the role of laparoscopy in managing intussusception is not clearly defined, laparoscopy may be an alternative approach to the surgical treatment of adult intussusception in selected cases.

PURPOSE: We evaluated the associations between the expression of bcl-2, CD44, p53 and tumor progression in prostate cancer. MATERIALS AND METHODS: Samples of prostatic tissue from 36 men with histologically confirmed prostate cancer were obtained by biopsy or transurethral prostatectomy, and we examined the expression of bcl-2, CD44, p53 using immunohistochemical staining and related their expression with the stage, grade, disease progression. RESULTS: Ten of 23 tumors were positive for bcl-2, 10 of 20 negative for CD44 and 11 of 16 positive for p53 had progression. Nine of 16 tumors positive for p53 were poorly differentiated compared with 6 of 20 negative for p53(p < 0.05). Also, 13 of 20 tumors negative for CD44 were poorly differentiated compared with 2 of 16 positive for CD44(p < 0.05). A significant number of patients positive for p53 progressed and had shorter time to progression compared to those negative for p53(p < 0.05). Bcl-2 staining was not correlated with grade, clinical stage and tumor progression. p53 staining was not correlated with clinical stage, whereas CD44 was not correlated with clinical stage and tumor progression. CONCLUSIONS: There was an association between aggressive behaviour of prostate cancer and the increasing expression of p53. This study confirmed that the expression of p53 had significant impact on the disease progression regardless of stage in the prostate cancer however, the expression CD44 and bcl-2 had no role in the progression and clinical stage of the prostate cancer.
