1.Histopathologic Features of Gastric Mucosa Colonized by Helicobacter pylori.

TO evaluate the histopathologic features of gastric mucosa by Helicobacter pylori (HP), we reviewed 70 endoscopically biopsied chronic gastritis and peptic ulcer. The results are as fonows. l. HP was detected in 923% (48/52) of active gastritis, 8l8% (9/l l) of peptic ulcer and l43% (1/7) of chronic gastritis. The prevalence of HP infection was significantly higher in active gastritis and peptic ulcer than chronic gastritis (P<0005). 2. The infiltration of intraepithelial neutrophil of the 58 HP-positive cases was significantly more than of the l2 HP-negative cases (P<0005). 3. The infiltration of neutrophil and chronic inflammatory cells in the lamina propria of the 58 HP positive cases were significantly more than of 12 HP-negative cases (P<0.005, P<0.01) 4. The gastric epithelium of 58 HP positive case showed characteristic degenerative change, such as epithelial pits (93.l%), irregular surface (84.4%), individua1 cell drop-out (46.5%) and microerosion (27.6%). The similiar changes were not seen in l2 HP-negative cases. In summary, HP is significantly correlated with inflammatory reaction of the gastric mucosa. It is also significantly correlated with the epithelial degenerative changes that is considered to the precursor of peptic ulcer.
2.A Case of Thromboembolism Associated with Central Venous Catheter.

The central venous catheter(Quinton) was replaced in the right atrium for chemotherapy and blood sampling to a eight-year old girl with acute lymphoblastic leukemia. The catheter was flushed with heparin two times daily and the chemotherapeutic drug regimens by CCSG-105 protocol were vincristine, prednisone, L-asparaginase, daunomycin, methotrexate, cyclophosphamide, cytosine arabinoside, 6-mercaptopurine and adriamycin. On day 31 of catheterization, the lumen for blood sampling was blocked. From day 60 of catheterization, anorexia, nausea, vomiting, abdominal distension and tachypnea were developed. Echocardiogram, lung scan, pulmonary function test(PFT), arterial blood gas analysis (ABGA) were done. Two cemtimeters in diameter of thrombosis was found in the right atrium on echocardiogram. The lung scan showed slight decrease in uptake of Tc-99m on the whole lung fields. Restrictive ventratory impairment on PFT and decreased PaO2(48 mmHg) on ABGA were found. To lysis of thromboembolism, urokinase(4,400 IU/kg for initial 10 minutes and the 4,400 IU/kg for 12 hours) was injected intravenously and aspirin(30 mg/kg/d, po) was given. The thrombosis was disappeared from the atrium on echocardiogram and PaO2 was increased up to 97 mmHg temporary. The catheter was removed but total haziness on the right middle and lower lobes were developed after 5 days. The same dose of urokinase was injected just infront of the embolism through Pitfall catheter for 8 days intermittently as results of pulmonary angiography. Symptoms and PaO2 were alleviated and the lung was expanded with mild atelectasis on day 55 of the first fibrinolytic enzyme therapy. Although the frequent complications of central venous catheter are catheter occlusion and infection, we have to check the development of thromboembolism with echocardiogram periodically or in case of unexplained respiratory symptoms.

4.A Case of Cavernous Hemangiomatosis involving all Tendon Sheath of Flexors and Median Nerve of the Right Forearm

A case of eavernous hemangiomatosis involving alI tendon sheath of flexors and median nerve of the right forearm is reported. This case was a 22 year old Korean male who complained of painful swelling of the flexor side of right wrist region, limited motion of wrist joint, loss of pinching and opposition with tingling sensation on the admission. Roentgenographic examination revealed fusiform swelling of the soft tissues in flexor aspect of the right wrist region, with phlebolith, 3mm. in diameter. The tumor mass looked like strawberry in shape, and was scattered into all tendon sheath of flexors of the forearm, median nerve, the distal half of pronator quadrtus and other surrounding soft tissues. The specimen was confirmed as cavernous hemangioma in microscopic examination. The right long arm splint was applied for 3 weeks, and then the patient was treated with physical therapy for 4 months. Pinching, opposition, and motion of the right wrist joint recovered almost completly, and no evidence of the recurrence of hemangiomatosis was seen until 8 months after surgery.

5.Rupture of Peroneus Brevis Combined with Bimaileolar Fracture: A case Report

Combined injury of peroneus brevis rupture and bimalleolar fracture is very rare. The avulsion of peroneus brevis tendon from insertion maybe neglected and may prevent anatomic reduction of lateral malleolar fracture. We have treated open bimalleolar fracture with avulsion of peroneus brevis tendon from its insertion.

6.The Results of Treatment in Femoral Neck Fractures

We clinically analyzed 53 patients of fresh femoral neck fractures who were treated in Daegu Catholic Hospital from Aug. 1980 to Apr. 1987: The end results of the surgical methods, i.e. internal fixation, endoprosthesis and total hip replacement, were compared in the 45 patients who were followed for more than 2 years. The results were as follows. 1. The most frequent age occurrence was different in sexes ; male was in the 8th decade (29%) and female was in the 6th decade(31%). 2. The most frequent cause of fracture was “fell from height” in male(43%) and slip down in female(47%).3. Mid cervical fracture was the most frequent in anatomical classification(49%) and 83% of fractures were displaced(Garden stage III & IV). 4. The results were evaluated by the modified Lunceford criteria which included the degree of patient's own satisfaction. The combined result of “excellent” and “good” were obtained in 75% of 20 internal fixation cases, 80% of 15 endoprosthesis cases and 71% of total hip replacement cases. It was concluded that we could get similar results by 3 surgical methods if reasonable criteria of indication were applied for each method: rehabilitation of patient can more important than osteosynthesis itself in the determination of indication of operative methods.

7.Stem Cell Supportive Care in Chemotherapy of Acute Leukemia.

OBJECT: Peripheral blood stem cells(PBSC) were collected after remission induction chemotherapy and reinfused after intensified chemotherapy in order to increase the chemotherapeutic efficacy. METHOD: Collection of mononuclear cells(MNC) was started when CD34+ cells above 1% and WBC above 1,000/ul with mononuclear cell percentage above 30%, the collection procedures were done 2-6 times in each patient of total 6 cases. The collected MNCs were stored in -75 degrees C and reinfused after thawing in the water bath at 37 degrees C. RESULTS: Twenty three collection procedures from the 6 cases of acute leukemia had made effective MNC collection in 4 cases and mean collection efficiency was 49.8%(from 39.0% to 74.5%). Hematological changes after the procedure were mean reduction of hemoglobin 1.34g/dl and mean 20.25% of decreased platelet count. There were noticed transient symptoms of discomfort, coughing, chest tightness and dyspnea in association with the infusion of stored cells. CONCLUSION: The basic process for the peripheral blood stem cell transplantation(PBSCT) was accomplished with this supportive care for the consolidation chemotherapy of acute leukemia. Effective PBSCT would be warrented with more qualified process and high dose chemotherapy.

8.Evisceration Using Scleral Capping Technique for Severe Phthisis Bulbi.

PURPOSE: To introduce a new technique, involving the creation of a circumferential 360 degrees scleral relaxing incision, using an implant as large as 20 mm in diameter, and report on the surgical results in the treatment of severe phthisis bulbi, which are not indicated for classic evisceration. METHODS: The medical records of 20 patients with 20 phthisis bulbi who had undergone evisceration and placement, using the scleral capping technique, of porous polyethylene orbital implants as large as 20 mm in diameter between September 2002 and February 2006 were reviewed. For each patient, after evisceration, a circumferential 360 degrees equatorial scleral relaxing incision was made posterior to insertion of 4-recti muscles, and a porous polyethylene orbital implant was implanted. Each circumferential scleral relaxing incision wound was loosely sutured in the mattress fashion, using 5-0 braided polyester (Ethibond). RESULTS: The average of preoperative axial length, obtained using A scan and orbital computed tomography scan, was 16.8 mm. Orbital implants of 20 mm in diameter were implanted in all patients. The average of postoperative follow-up interval was 29.1 months. Orbital implant showed good motility to the satisfaction of all of patients. And the scleral capping technique could prevent complications, such as exposure of orbital implant, enophthalmos, and superior sulcus deformity. CONCLUSIONS: Scleral capping technique is safe and useful for evisceration in patients with severe phthisis bulbi. This technique can provide adequate space for the placement of and implant as large as 20 mm in diameter without generating any tension on wound. Consequently, this technique provided cosmesis and prosthesis motiliy, even without the use of a Medor coupling peg, with which patients were satisfied.

9.A seroepidemiologic survey for human sparganosis in Gangweon-do.

Gangweon-do is known to be highly endemic area of sparganosis more than other provinces in Korea. A seroepidmiologic examination for the detection of anti-Spirometra erinacei plerocercoid IgG in serum was carried out in normal inhabitants in Hongcheon-gun, Gangweon-do. Sere were tested by enzyme-linked immunosrobent assay (ELISA) for the anti-sparganum antibodies. Positive rate for anti-sparganum antibody in 719 adults was 3.3%. Data of the questionnaire for 24 ELISA positive inhabitants revealed that 20 had a history of eating raw meat of snakes, 24 had a history of eating frogs, and 24 had a history of drinking stream water. Two positive cases had a past history of sparganosis. Two positive cases showed current symptoms of sparganosis. The data revealed that ELISA would be useful to find infected cases among normal inhabitants at sparganosis-endemic areas.

10.The Clinical Study on 28 Patients with the Pericardiac Effusion.

The clinical study was performed to 28 patients with the pericardiac effusion who were admitted in Busan university Hospital and Paik Hospital, In-Je Medical College, Busan, korea during March 1976 and July 1981. The results were as following; 1) 12 cases(42.9%) of total 28 pericardiac effusion occured in 3rd decade, 8(28.5%) in 2nd decade and 5 cases(17.9%) in 4th decade. Sex distribution showed male to female 2.5:1. 2) With etiological distribution there were 15 cases(53.6%) tuberculous, 5(17.9%) nonspecific, 3(10.7%) malignant, 2(7.1%) pyogenic, 2 traumatic and 1(3.6%) rheumatic. 3) On admission 28 cases(100%) complained of dyspnea, 24(85.7%) distant heart sound, 20(71.4%) each engorged jugular vein and edema, 18(64.3%) each cough and abdominal fullness, 17(60.7%) each sputum and chest discomfort and pain, 12(42.9%) fever, 11(39.1%) orthopnea, 10(35.7%) chilling, 7(25%) oliguria, 4(14.6%) palpitation and 3(17.2%) headache. Increased cardiac dullness was observed in 26 cases(71.4%), hepatomegaly 18(64.3%), pulmonary rales 17(60.7%), ascites 13(46.1%), fever 12(42.9%), pericardiac friction rub 10(35.7%), splenomegaly 7(25%), paradoxical pulse 6(21.4%) and jaundice 2(7.1%). 4) On amission SGOT and SGPT level was increased each in 15(53.6%) and 13(46.7%), TTT and CCF abnormal in 10(35.7%), and 8(28.5%), serum NPN, BUN, creatinine increased in each 10(35.7%), 11(39.1%), 8(28.5%), WBC increased in 6(21.4%), RBC decreased in 8(28.5%) and ESR increased in 16(57.1%). 5) Characteristics of pericardiac effusion showed hemorrhagic 17(60.7%), serosangeous 6(21.4%), sangeous 3(17.2%) and pyogenic 2 cases(7.1%). 6) The ECG on admission revealed low voltage in 25(89.1%), depressed ST segment in 13(46.1%) and inverted T wave in 12(42.9%). 7) Chest X-ray showing cardiomegaly 28(100%), pleural effusion in 20(71.4%), and pulmonary congestion in 16(57.1%). 8) The m mode and 2-D Echocardiogram showed echo free space in 28 cases and after pericardiocentesis the space was reduced or disappeared. 9) 19 cases of 28(67.5%) were improved clinically, while 5 cases(17.9%) were not improved and 4 cases(14.6%) died.
