1.A Case of Combined Interstitial and Intrauterine Pregnancy as Fifth Ectopic Pregnancies after In Vitro Fertilization and Embryo Transfer in Bilateral Salpingectomy patient.

Although a higher incidence of ectopic pregnancy has been reported after in vitro fertilization(IVF) and embryo transfer(ET), five ectopic pregnancies in one patient is very rare. We experienced a case of combined interstitial and intrauterine pregnancy as fifth ectopic pregnancies after in vitro fertilization and embryo transfer in one patient with history of bilateral salpingectomy due to previous four tubal pregnancies. Repeated transvaginal ultrasound examinations confirmed heterotopic pregnancy. In IVF-ET, bilateral salpingectomy does not remove the risk of interstitial or cornual pregnancy.

3.A Case of Tuberculosis Verrucosa Cutis.

We report a case of tuberculosis verrucosa cutis in a 33-year-old male patient who presented with mild itching verrucous plaque over the perineum. The Mantoux test revealed a positive reaction. The histophathologic picture consists of slight hyperkeratosis, irregular ac:anthosis overlying an inflammatory iafitrate in the dermis. Tuberculous granulomas with caseation necrosis were seen in the dermis. Tuber clebacillus was demonstrated. The patient was treated with Isoniazid, Rifampicin, and Pyrazine made for 6 months. To date, the skin lesions have markedly improved.

5.A Case of Nodular Amyloidosis.

A 38-year-old male patient visited our clinic complaining of three skin lesions on the scalp. There were yellowish to brownish, waxy, non-tender, walnut-sized nodules. Hematoxylin and eosin staining revealed amorphous pinkish material deposits in the dermis. The Congo red stain and Dylon stain under polarizing microscopy showed yellow-green birefringence and the immunoglobulin-lambda light chain stain showed a positive reaction. An electron microscopic examination revealed filaments with uniform diameter(6 to 10nm) that were straight and neither branched nor anastomosed. Based on the clinical, histopathological, immunohistochemical and electron microscopical findings, the skin lesions were diagnosed as nodular amyloidosis.

6.Two cases of Marjolin's ulcer.

Marjolins ulcer is a special type of skin malignancy arising from previous scar site. We report two cases of Marjolins ulcer arising from chronic deg enerating burn scar. A 65-year-old male had a 9 x 15cm sized ulcer on his right popliteal fossa. He had a history of burn when he was seventeen years old and an ulcerati 1g tumor developed at the site of burn scar about 1 year ago. The ulcer became progressively penlarged in size and also painful. Skin biopsy revealed squamous cell carcinoma and,subsequently Above-Knee amputation was done. Another 54-year-old male had a painful coin sized ulcer on his right popliteal fossa where he had burn at the age of thirty-four. The ulcer was developed at the site of burn scar about 2months ago. The pathologic diagnosis was squamous cell carcinoma. He was recommended to get his leg amputated, but it was refused.

7.Cutaneous Manifestations in Sepsis Caused by Methicillin-Resistant Staphylococcus aureus.

Sepsis refers to the systemic response to serious infection. Patients with sepsis usually manifest fever, tachycardia, tachypnea, leukocytosis, and a localized site of infection. Methicillin-resistant Staphylococcus aureus(MRSA) is a gram-positive, nonmotile, aerobic, catalase- positive coccus, which is resistant to all the B -lactam antibiotics. Cutaneous manifestations in sepsis are maculopapules, nodules, petechiae, ecchymoses, purpurae, pustules, vesiculobullae, hemorrhagic bullae and ulcers. When MRSA is identified in blood cultures and skin tissue cultures, the skin lesions can be considered as cutaneous manifestations in sepsis caused by MRSA. We report two cases with erythematous pustules, petechiae, hemorrhagic bullae and maculopapules caused by MRSA sepsis. MRSA grew in blood cultures and skin tissue cultures.

9.Anterior Discectomy and Fusion with Plate Fixation for Cervical Disc Herniation.

The anterior discectomy is widely accepted method for removal of the herniated disc material of cervical spine due to direct visualization of the lesion site. In addition, it is believed to prevent recurrence of herniation, progression of degenerative change, and to preserve normal cervical lordosis after the fusion. For more effective anterior fusion and early postoperative ambulation without prolonged external support, internal fixation technique was introducsuspected to influence the result of operation, the type of bone graft and age of the patient may be the effective factors to the late result. ed. Authors reviewed 36 patients with cervical disc herniation and evaluated the result of anteior discectomy and fusion with plate fixation. With a mean follow up of 34 months, there were remarkable symptomatic improvement at most instances and bony union in situ within 3 months in all cases after the operation. Disclosed complications were 2 cases of hoarseness which was improved spontaneously, and 1 case of dysphagia. At the comparison of the effect of the factors which were suspected to influence the result of operation , the type of bone graft and age of the patient may be the effective factors to the late result.
