OBJECTIVE: Our purpose was to investigate the relationship between the levels of IL-6 and tumor necrosis factor-alpha in the peritoneal fluid(PF) of women with and without endometriosis and infertile women. Design: Prospective and case-control study. Setting: University hospital. Patients: Twenty-nine women with laparotomy or laparoscopic findings of minimal to severe endometriosis, and twenty-eight women with no visual evidence of pelvic endometriosis and with benign gynecologic disease. Main Outcome Measures: PF IL-6 and tumor necrosis factor-alpha levels were determined using commercial ELISA. IL-6 and tumor necrosis factor-alpha concentrations were compared among women with and without endometriosis, and with infertile and fertile women, and then also compared according the revised American Fertility Society classification. RESULTS: IL-6 and tumor necrosis factor-alpha concentrations were higher than in the PF of women with endometriosis than in matched normal controls. Cyclic variations in IL-6 concentrations were seen in PF from patients with endometriosis: the concentrations in the secretory phase were significantly higher than those in the proliferative phase. IL-6 concentrations were higher than in the PF among of infertile women than in fertile women. A significant correlation between PF IL-6 and tumor necrosis factor-alpha concentrations and endometriosis stage III and IV was noted. CONCLUSIONS: Increased PF levels of IL-6 and tumor necrosis factor-alpha in patients with endometriosis may be relate to endometriosis-associated infertility and to the pathogenesis of endometriosis suggesting that partially contribute to the disturbed immune regulation observed in patients with endometriosis.

Percutaneous transluminal angioplasty(PTA) were performed in three patients with atherosclerotic stenosis of subclavian arteries. The arteries were successfully dilated without complications during the procedure. All patients were asymptomatic during follow-up periods ranging from eight months to fifteen months after PTA.

Of 14,682 samples of feces examined, 81.7% were positive for intestinal helminths. Trichocephalus trichiurus was observed most frequently, in 73.2%. Eight other species of helminths found were Ascaris lumbricoides, Hookworm, Trichostrongylus orientalis, Clonorchis sinensis, Tapeworm, Hymenolepis nana, Paragoniumus westermani and Enterobius vermicularis, listed in decreasing order. Double infections were found more frequently than single infections, and mixed infection of five or more species of helminths occurred in 0.2% Of 10,320 samples of feces examined, 22.3% were positive for intestinal protozoa, Entamoeba coli was observed most frequently, in 11.1%. Six other species, Endolimax nana, Giardia lamblia, Entamoeba histolytica, Iodamoeba butschlii, Chilomastix mesnill and lsospora hominis were found, in the order. It was noticed that E. histolytica was found more frequently in trichurial infection than with other helminths. Of 1,050 samples of sputum from the T. B. Clinic, 7.0% were positive for paragonimus eggs. Trichomonas vaginalis were found in 35.8%of 1,146 vaginal swabs examined. Seven cases of Plasmodium vivax infection were found among 140 suspected blood smears.

OBJECTIVE: The purpose of this study is to investigate changes of vasoreactivity after STA-MCA anastomosis in patients who develop hemodynamic cerebral ischemia associated with ICA or MCA occlusion. METHODS: Twenty-five patients with hemodyamic infarction due to symptomatic ICA or MCA occlusion were treated with extracranial-intracranial bypass surgery (EIAB). Of these, we enrolled 17 patients who underwent follow-up SPECT and follow-up angiogram. To test vasomotor reactivity, we measured relative regional cerebral blood flow (rrCBF) with SPECT both at rest and after diamox infusion. rrCBF were calculated using the following equation: (lesional radioactivity/contralateral radioactivity) x100. We performed EIAB in patients with impaired vasomotor reactivity. To evaluate changes in vasomotor reactivity after surgery, follow-up brain SPECT was performed 1-6 months after surgery and compared rrCBF before and after surgery. RESULTS: Base line radioactivity ratios did not change after surgery. These ratios after diamox challenge were 59.8+/-4.2%, however, 1-6 months after surgery, ratios increased to 77.7+/-.6% (p<0.05, Paired T-test). Preoperative degree of angiographic collateral circulation did not influence the restoration of vasoreactivity. All but two patients showed good collateral circulation through the bypass. There were no cerebral accidents in any these patients at 6 months to 2 years follow-up. CONCLUSION: These results demonstrate the restoration of vasomotor reactivity distal to the occluded ICA or MCA after EIAB, and also show that measurement of rrCBF using SPECT are useful indicators of the clinical and hemodynamic improvement.

The main goal of the surgical treatment of maxillofacial deformity is improved esthetic appearance and masticatory function. Recently, the purpose of orthognathic surgery has been focused on esthetic improvement of the patients. The purpose of this study was to investigate the changes in gonial angle to orthognathic surgery. For this study 30 patients were randomly selected. All patients with mandibular prognathism were operated upon with bilateral sagittal split ramus osteotomy and mandibular setback. The patients were cephalmetric lateral radiographs for 1 hear. Lateral cephalmetric radiographs taken preoperatively, over 6-months and 1 year follow up were traced and analysed. The following conclusions were reached. : 1. During following up period for 1 year, the change of gonial angle after orthognathic surgery was decreased, except 3 cases. The mean gonial angle change was decreased from 128.6 degrees(+/-4.83) to 123.4 degrees(+/-5.52) in female(p<0.05). 2. During follow up period for 1 year, the mean gonial angle was decreased after orthognathic surgery from 129.7 degrees(+/-6.47) to 123.3 degrees(+/-7.52), in all male patients(p<0.05). 3. The mean gonial angle changes after orthognathic surgery was about 5.2 degrees in female and 5.4 degrees in male. There was less statistical significance between male and female. 4. The ratio of mandibular setback amount from 6mm to 15mm was 83.3%, and from 1mm to 5mm was 10%.
