BACKGROUND: Methicillin-resistant Staphylococcus aureus(MRSA) is an increasingly common cause of nosocomial infections worldwide. Epidemiologic investigation of MRSA outbreaks and identification of pathways of nosocomial MRSA spread require the ability to distinguish individual MRSA strains. We applied molecular tap ing of the methicillin-resistant determinant (mec) and coagulase typing in the investigation of a nosocomial MRSA infections. METHODS: We randomly selected 79 strains of mecA positive MRSA isolated from patients who visited Dong-A university Hospital from Dec. 1995 to Oct. 1996. Molecular typing of MRSA was performed by comparing the size of the mac-associated hypervariable region amplified by the polymerase chain reaction (PCR). Coagulase typing with type I-VIII antisera was also used for classification of MRSA based on its phenotype. Each isolates were classified by the combination of molecular analyses and coagulase type. RESULTS: The 79 MRSA isolates were grouped Into sin hypervariable legion (HVR) genotypes on the basis of the size of the PGR products. In coagulase typing, the most predominant type was II(46.8%) and type V was not found. Nine strains were not typable. The combination of HVR genotypes and coagulase types showed 23 different types in 79 MRSA Isolates. The strains which were repeatedly isolated from the same patients showed the same HYR genotypes and coagulate types. CONCLUSION: The combination of HVR genotypes and coagulase types is thought to be useful in epidemiolgical Investigation of nosocomial infections caused by MRSA ,because of its simplicity and reproducibility.

4.Detection of Point Mutations in the rpoB gene Related to Drug Susceptibility in Mycobacterium Tuberculosis using an Oligonucleotide Chip.

BACKGROUND: The appearance of multiple-drug-resistant Mycobacterium tuberculosis strains has been seriously compromising successful control of tuberculosis. Rifampin-resistance, caused by mutations in the rpoB gene, can be indicative of multiple-drug-resistance, and its detection is of great importance. The present study aimed to develop an oligonucleotide chip for accurate and convenient screening of drug-resistance. METHODS: In order to detect point mutations in the rpoB gene, an oligonucleotide chip was prepared by immobilizing specific probe DNA to a microscopic slide glass by a chemical reaction. The probe DNA that was selected from the 81 bp core region of the rpoB gene was designed to have mutation sites at the center. A total of 17 mutant probes related to rifampin-resistance including 8 rifabutin-sensitive mutant probes were used in this study. For accurate determination, wild type probes were prepared for each mutation position with an equal length, which enabled a direct comparison of the hybridization intensities between the mutant and wild type. RESULTS: Mycobacterial genomic DNA from clinical samples was tested with the oligonucleotide chip and the results were compared with those of the drug-susceptibility test in addition to sequencing and INNO-LiPA Rif. TB kit test in some cases. Out of 15 samples, the oligonucleotide chip results of 13 samples showed good agreement with the rifabutin-sensitivity results. The two samples with conflicting result also showed a discrepancy between the other tests, suggesting such possibilities as existence of mixed strains and difference in drug-sensitivity. Further verification of these samples in addition to more case studies are required before the final evaluation of the oligonucleotide chip can be made. CONCLUSION: An oligonucleotide chip was developed for the detection of rpoB gene mutations related to drug-susceptibility. The results to date show the potential for using the oligonucleotide chip for accurate and convenient screening of drug-resistance to provide useful information in antituberculosis drug therapy.

5.Ground-Glass Opacity in Lung Metastasis from Adenocarcinoma of the Stomach: A Case Report.

Ground-glass opacity is a frequent but nonspecific finding seen on high-resolution CT scans of lung parenchyma. Histologically, this appearance is observed when thickening of the alveolar wall and septal interstitium is minimal or the alveolar lumen is partially filled with fluid, macrophage, neutrophils, or amorphous material. It has been shown that ground-glass opacity may be caused not only by an active inflammatory process but also by fibrotic processes. When a focal area of ground-glass opacity persists or increases in size, the possibility of neoplasm-bronchioloalveolar carcinoma or adenoma, or lymphoma, for example- should be considered. Diffuse nonsegmental ground-glass opacity in both lung fields was incidentally found on follow up abdominal CT in a stomach cancer patient and signet-ring cell-type metastatic lung cancer was confirmed by transbronchial lung biopsy. We report a case of diffuse ground-glass opacity seen in metastatic lung cancer from adenocarcinoma of the stomach.

6.Meniscus Tear

Classification of meniscal tear is necessary only for partial resection. In western countries many reports said that medial meniscal tear is commoner than that of lateral meniscus. Actually the reverse is true in Korea and in some other oriental countries, However there is no plausible explanation concerning this matter. In this report 250 knees of 240 patients were involved. We performed arthroscopic meniscectomy in all cases and we investigated the relationship between the pattern of tear and many possibly related factors. The follow-up period was 1–8 years, the ratio of male-female was 163:77, and the range of age was varied between 7 and 71 tears with the young and middle age groups being predominant. Right and left side ratio was 117:113 and the involvement of both knees was noted in 20 knees of 10 patients. The results were as follows: 1. The medial menisci were torn in 100 knees and was less than 142 lateral meniscal tear, with the ratio being 1:1.4. The tear of lateral meniscus was commoner in the age group of less than 30 years and the tear of medial meniscus was commoner in the age group older than 50 years whereas there was no difference in the age group of inbetween. 2. The commonest pattern of tear was longitudinal one including peripheral tear, that is 135 knees (54.0%), followed by complex tear, 50 knees (20.0%), oblique tear, 40 knees (16.0%), horizontal tear, 21 knees (8.4%) and lastly superior and inferior flap tear, 5 knees (2.0%). 3. The longitudinal tear was most prevalent in the age group of 20–40, whereas the complex tear was most prevalent in the age group of beyond 50 years (p<0.05). 4. The longitudinal tear was significantly prevalent in lateral menisci, the complex tear in medial menisci (p<0.05), whereas the transverse and oblique tear were not significantly prevalent in either meniscus (p>0.05). 5. In the longitudinal tear group, the tear occured most frequently in the midsubstance of posterior portion in the medial menisci without statistical significance (p>0.05) wereas the tear occurred most frequently in the periphery of the anterior portion in case of lateral menisci (p<0.05). 6. There were 37 discoid lateral menisci, with the longitudinal tear being the commonest. 7. Ground sports injuries occurred only in 34 knees (13.6%), with the longitudinal tear being significantly commoner than in the other injuries. From these results it would be suggested that although the discoid lateral meniscus has to be given much weight in the total meniscal injuries another explanation should be given concerning the prevalence of lateral meniscus tear compared to the medial meniscus tear and the prevalence of longitudinal tear.

8.Assessment of Neurobehavioral Performance among Welders Exposed to Manganese.

OBJECTIVES: A cross sectional study was carried out in the welders exposed to manganese and control group. The aim of this study was to assess the adverse effects of manganese on central nervous system of welders. METHODS: The study group consisted of 49 welders and the reference group, 49 workers matched with welders for age and educational level in automobile manufacturing company. Concentration of manganese in welding hume, in blood, and cumulative exposure index(CEI) were determined for each subject. For each group, computerized neurobehavioral test including Simple Reaction Time, Digit Symbol, Digit Span, and Finger Tapping Speed of Swedish Performance Evaluation System were applied. RESULTS: Exposure level of manganese in welders were very low(mean CEI 0.069). Welders exposed to manganese had slower response speed, slower motor steadiness, and shorter memory span than control group. But these differences were not statistically significant. The performance of Finger Tapping Speed tended to lower as environmental and biological exposure index got higher. Other neurobehavioral tests were found to have a little association with the exposure indices. CONCLUSIONS: There was no significant difference of neurobehavioral performance between welders and control group. This study indicates that Finger Tapping Speed measuring motor steadiness might be a one of useful tests for early detection of adverse effects of welding on central nervous system.

9.Intrauterine Device Migrating into the Bladder.

Migration of an intrauterine contraceptive device (IUD) into the urinary bladder is very rare. We experienced a case of a foreign-body migrating into the bladder of a 35-year-old women who has been suffering from frequency and dysuria for the recent 5 months. The IUD had originally been inserted 10 years ago. Removal of the IUD had been performed for achieving pregnancy, but the IUD was not detected at the correct site 7 years ago. One year later, the patient had given birth without complication. It is thought that it took less than 6 years for the IUD to invade the bladder wall.
