1.A Clinical Study on Intracompartmental Pressure of Leg Using Slit Catheter

Intracompartmental pressure of leg was measured by the slit catheter technique in one hundred fifty compartments of one hundred cases; fifty were from healthy young male adults, the others were from tibial fractured patients during three years from March 1983 to Fobruary 1986. The results were as follows; 1. Slit-catheter technique was found as a accurate method for continuous pressure monitoring. 2. The range of normal pressure was from zero to six millimeters of mercury(1.45mmHg±0.85). 3. Tissue pressure was 0–15mmHg(4.03mmHg±1.83) in unaffected side of fractured patients, 4–35mmHg(14.65mmHg±2.05) in affected side of fractured patients.

3.Rapid Screening of Vancomycin Resistant Enterococci with Chromogenic Agar (ChromID VRE).

BACKGROUND: Rapid screening of vancomycin-resistant enterococci (VRE) is very important for controlling and preventing the spread of VRE in hospitals. We compared the performance characteristics of a chromogenic agar (ChromID VRE, bioMerieux, France: CA) to that of Enterococcosel agar (supplemented with 6 microgram/mL of vancomycin :EA) for direct detection of VRE from stool swabs. METHODS: Total 125 rectal swabs were collected from 57 patients in the intensive care units of an 850-bed university hospital over a period of 3 months. The samples were inoculated on EA, CA and into broth enrichment containing 6 microgram/mL of vancomycin (BE). BE was subcultured on CA after overnight incubation. RESULTS: Eighty two samples from 22 patients were positive for VRE by BE. At 24 h, the sensitivity/specificity of EA and CA were 89%/100% and 72%/100%, respectively. At 48 h, the sensitivity/specificity of EA and CA were 94%/89% and 89%/100%, respectively. CONCLUSION: CA provides equivalent sensitivity comparable to EA for the recovery of VRE at 48 h incubation, and has additional advantage of being able to differentiate between vancomycine resistant E. faecium and E. faecalis.

5.The clinical significance of serum CA125 and CA19-9 levels in endometriosis.

OBJECTIVE: Because endometriosis is difficult to diagnose and has a high recurrence rate after treatment, a reliable serum marker of endometriosis is necessary. Therefore, the aim of this study is to measure the serum levels of CA125 and CA19-9 in patients with endometriosis before and after treatment and during recurrence, and to assess the usefulness of these levels in the diagnosis, clinical follow up and prediction of recurrence in endometriosis. METHODS: Eighty-eight patients who visited the department of Obstetrics and Gynecology of Ewha Mokdong Hospital from January 1994 to December 1998 and were diagnosed as endometriosis by laparoscopy or explo-laparotomy were enrolled as subjects. A retrospective analysis of serum CA125 and CA19-9 levels at 1 month before and 3 to 6 months after initiation of treatment was done. RESULTS: The serum CA125 and CA19-9 levels of endometriosis group(81.0+/-252.5, 36.6+/-53.4 ; mean+/-2SD, U/ml) before treatment was significantly higher than control group(11.6+/-12.8, 9.4+/-8.6)(p<0.05). Overall sensitivity rate for CA125, CA19-9 levels and both was 53.4%, 42.9% and 64.3% respectively. The sensitivity rate for endometriosis, stage 3 and 4(85.4%, 55.0%) was significantly higher than that, stage 1 and 2(p<0.05). The serum CA125 level in endometriosis group showed a significant increment according to stages(p<0.05) while the serum CA19-9 level showed an increasing trend(p=0.055) and both levels decreased significantly after treatment(p<0.05). The serum CA125 level was also higher at recurrence after treatment(p<0.05). CONCLUSIONS: The serum CA125 and CA19-9 levels are a useful marker for diagnosing severity of disease, monitoring efficacy of treatment and predicting recurrence in endometriosis.

6.Lhermitte-Duclos Disease in a Sudden Death: An autopsy case.

Lhermitte-Duclos disease of the cerebellum manifests itself as a slowly growing mass formed by abnormal neurons that appear more malformative than tumoral. Although most cases show satisfactory clinical evolution, several cases, not recognized in life, died abruptly, usually from respiratory arrest. We recently experienced a 19 year old male, who died suddenly during alcohol drinking. At autopsy, the cerebellum revealed thickening of the folia over two limited areas. Microscopically, the lesion was characterized by proliferation of abnormal neuronal cells with excessive myelination of their axons, disappearance of Purkinie cells and greatly reduced white mater. Theories regarding the pathogenesis of Lhermitte-Duclos disease were reviewed and discussed.

8.A Survey of Hepatitis B Surface Antigen and Hepatitis B Surface Antibody Positivity in ROK Air Force Pilots.

OBJECTIVE: The aim of this study was to detect the prevalence of hepatitis B surface antigen (HBsAg) and hepatitis B surface antibody (anti-HBs) in The Republic of Korea Air Force (ROKAF) pilots, and to investigate the factors related to occupational exposure. METHODS: For 720 pilots who visited aeromedical center, ROKAF for general health screening from January, 2001 to August, 2002, HBsAg and anti-HBs were tested using passive hemagglutination and passive hemagglutination, respectively. If HBsAg results had been positive or vague, we performed confirmation test by ASAN Easy Test(R) HBs kit. RESULTS: The overall prevalence of HBsAg and Anti-HBs was 1.8% and 49.4%, respectively. HBsAg positivity was 0.0% in 20-24 years, 0.8% in 25-29 years, 1.1% in 30-34 years, 4.4% in 35-39 years, 1.5% in 40-44 years, 5.4% in 45-49 years, 5.3% in 50-54 years and significantly different below and above thirties (P=0.024). The prevalence of HBsAg in pilots was not different with general adults. Anti-HBs positivity was 100.0% in 20-24 years, 43.9% in 25-29 years, 57.1% in 30-34 years, 50.0% in 35-39 years, 62.1% in 40-44 years, 50.0% in 45-49 years, 73.7% in 50-54 years and significantly increased with age (44.2% in twenties, 53.6% in thirties, 58.9% in more than 40) (P=0.005). The pilots susceptible to HBV infection who were negative in both HBsAg and anti-HBs were found in 49.0% of entire subjects. CONCLUSION: The prevalence of HBsAg and anti-HBs in pilots was lower than in general adults. The pilots tendered to be more susceptible to HBV infection than general adults. Hepatitis B vaccine should be integrated into pilot health programmes.

10.Comparison of Detection Methods and Culture Media for Isolation of Helicobacter pylori from Gastric Biopsy Specimens.

BACKGROUND: Helicobacter pylori(H. pylori) has been implicated in the pathogenesis of chronic active gastritis and peptic ulcer disease. All patients with ulcers who are infected with H. pylori receive antimicrobial therapy. Therefore diagnosis of H. pylori infection is imperative for the treatment gastritis or ulcer patients. We evaluated the four diagnostic methods and three culture media for the isolation of H. pylori. MATERIALS AND METHODS: Rapid urease test(CLO test), modified Gram stain, culture, and nested polymerase chain reaction (PCR) were performed with 108 gastroscopic biopsy specimens from patients with peptic ulcer or chronic gastritis. Among them 40 specimens were inoculated onto each of 5% sheep blood agar, e99 Yolk emulsion (EYE) agar, and 7% horse blood agar containing antibiotics. RESULTS: The positive rates were the highest by the PCR(72%), 67% by modified Gram stain, 64% by CLO test, and 57% by culture. Among the three media the horse blood agar (selective medial) gave the highest isolation rate (48%), followed by sheep blood agar (45%), and EYE agar (38%). CONCLUSION: Though PCR was the most sensitive method for the detection H. pylori modified Gram stain was sensitive enough, simple, rapid, and economical as the routine diagnostic method of H. pylori. For the culture of H. pylori combination of sheep blood agar as nonselective media and horse blood agar as selective media would show the highest isolation rate.
