Congenital muscular torticollis is a distinct entity, in which the primary pathologic picture is limited to the sternocleidomastoid muscle. Associated deformities of the face, head and cervical spine are secondary in character, resulting from an abnormal position of the head both prior and subsequent to birth. The etiology of congenital muscular torticollis is not clear yet. There are various opinions about adequate age and methods for treatment. From January 1970 to December 1984, 53 cases of congenital muscular torticollis were treated surgically at the Department of Orthopedic Surgery, Severance Hospital. From January 1980 to December 1984, 10S cases of congenita muscular torticollis were treated conservatively at the same hospital. Totally, 161 cases were studied and analyzed retrospectively and the results are summerized as follows: 1. Among the 161 cases, thers were 88 males and 73 females. The lesion was on the left side in 89 cases. 2. There were 85 cases of normal spontaneous vaginal delivery and 58 cases of complicated labor including 34 cases of breech delivery. 3. There were 6 cases associated with other congenital anomalies; one of them was combined with congenital subluxation of the hip joint. 4. Results of conservative physiotherapy were good under 1 year of age and results of surgical correction were good under 6 years of age. 5. The failure of conservative physiotherapy did not influence the results of surgical correction performed later. 6. The results of reoperation, when the initial surgical correction failed, were not as good. 7. Facial asymmetry correction results were especially good under 6 years of age when much growth potential remained. 8. There were no differences in results according to the various kinds of cervical braces although the results were good when the cervical brace was worn over 3 months. 9. On pathologic examination, there was no hemorrhagic reaction. The muscle was atrophied and was replaced by fibrous bands. Therefore, congemtal muscular torticollis is thought to be caused by abnormal intrauterine position rather than by birth trauma, and it is expected to get a good result by treating congenital muscular torticollis as early as possible.

Nevus lipomatosus cutaneous superficialis is a very rare skin disease, which usually is present at birth. The lesion shows groups of asymptomatic soft papules or nodules, which have a smooth or folded surface and are skin colored or yellowish. The area of predilection is the pelvic girdle (especially the buttock and sacrococcygeal areas). Microscopically, nevus lipomatosus cutaneous superficialis reveals groups of etopic fat cell nests within the dermis. Only one case was reported in Korea(1969). The authors observed a case of typical nevus lipomatosus cutaneous superficialis. The patient was 19 year-old male, who ha,ve had asymptomatic soft papules and nodules on the buttock, lower back and scalp since 14 years of his age. Routine laboratory findings were within normal limit. Histopathological findings showed etopic fat cell nests within mid and lower dermis (Fig. 3) Diagnosis was confirmed by clinical and histopathological findings. Litercature was briefly reviewed for the discussion.

Calcaneal fractures involving subtalar joint can be associated with prolonged and severe disability. Many different methods have been tried for the treatment in order to search for better results. From March 1983 to December 1985, 8 cases of calcaneal fractures involving subtalar joint were treated by open rcduction and internal fixation. and the results were as follows: 1. Satisfactory results were obtained from 7 cases out of 8. 2. Anatomic reduction was obtained by open reduction and internal fixation. 3. When medial approach failed, combined lateral and medial approaches gave better visualization of articular fecets and ensured anatomic reduction.

PURPOSE: We aimed to measure the visible angle (VA) outside the surgical field of view using preoperative magnetic resonance imaging (MRI), and to relate structural data to clinical outcome. MATERIALS AND METHODS: We retrospectively analyzed data obtained from 322 consecutive patients who underwent robot-assisted laparoscopic radical prostatectomy (RALP) or radical retropubic prostatectomy (RRP) for prostate cancer from January 2011 to June 2013, with at least 1 year of follow-up. Patients were divided into 4 groups, according to median VA values and surgical approach. VA was defined as the angle between the horizontal line on view, rotated to 90∘ from the midsagittal T2-weighted sequence image, and the tangent line of the highest protruding surface of the symphysis pubis. RESULTS: The larger VA was, the shorter mean operative time (213.2 minutes vs. 174.2 minutes, p=0.002) was significantly and the smaller VA was, the higher complication rate (10.8% vs. 1.6%, p=0.043) was in RRP patients. There were no significant differences in operative outcomes in RALP patients. CONCLUSIONS: VA in midsagittal T2-weighted sequence predict operative time and complication rates in retropubic radical prostatectomy, and thus, the value may be used as surgical indications for retropubic radical prostatectomy.

PURPOSE: This study aimed to evaluate the relationship between the serum IgE level and allergic sensitization/bronchial hyperresponsiveness (BHR) in preschool children with asthma. METHODS: One hundred nine asthmatic children age 4-6 years were enrolled. Total serum IgE levels, and specific IgE level to food and inhalant allergens were tested using Pharmacia Auto CAP system RAST(R)FEIA, methacholine bronchial challenge test (PCw=Provocative concentration at wheezing. PCsat=Provocative concentration at 5% fall of oxygen saturation) were performed in all subjects. They were divided into 3 groups according to the serum IgE level:less than 120 IU/mL (group A, n=38), 120 to 1,000 IU/mL (group B, n=45), and greater than 1,000 IU/mL (group C, n=26). The relationships with allergic sensitization and BHR were evaluatied among the 3 groups. RESULTS: Sensitization to foods and inhalant allergens were related to a higher total serum IgE levels. However, BHR and severity of asthma were not related to higher total serum IgE levels, except in group B. Presence of atopic dermatitis, high serum eosinophil (>450/mm3), allergic sensitization to foods and inhalants were related to high serum IgE greater than 1,000 IU/mL. (odds ratio ; 95% confidence interval=4.4;1.7-11.2, 3.2;1.2-9.2, 4.8;1.7-13.2, 5.5;1.2-24.9) CONCLUSION: Moderately elevated total serum IgE was related to allergic sensitization, severity of asthma and BHR. However highly elevated total serum IgE was related to only allergic sensitization. This may be attributed to other factors in preschool children with asthma.

PURPOSE: To evaluate the early clinical results of lumbar microdiscectomy using minimally invasive tubular retractor (METRx-MD system, Medtronic Sofamor Danek, Memphis, TN), and to validate the merits of minimally invasive spinal surgery. MATERIALS AND METHODS: From April, 2003 to April 2004 we retrospectively studied a consecutive series of 45 patients who underwent lumbar microdiscectomy using minimally invasive tubular retractor. In all cases, minimally invasive approach using the tubular retractor were performed with a 2 cm sized paramedian incision. The following data were collected: clinical outcomes, operative time, intraoperative blood loss, need for blood replacement, time needed before ambulation, length of hospital stay, and complications. The clinical outcomes were assessed by the modified MacNab criteria. RESULTS: Minimally invasive tubular microdiscectomy was performed in 45 patients over a 12-month period with an average follow-up of approximately 8 months. The clinical outcomes assessed by MacNab criteria were excellent in 33 patients (73%), good in 10 patients (22%). The average operative time was 63 minutes (range, 35 to 95 minutes). The average blood loss was 62 mL (range, 50 to 110 mL). None of the patients needed blood replacement. With the exception of 2 patients, all patients could walk at the day of surgery. The average hospital stay was 2.3 days. None of the patients had dural tear, wound problem, or other complications. CONCLUSION: Lumbar microdiscectomy using tubular retractor can offer a useful modality for the treatment of lumbar herniated disc with the merits of minimally invasive spinal surgery. Further long-term, randomized, prospective investigations are needed to fully evaluate the impact of this technique.
