3.Clinical and Histopathologic Study of Steroid Acne.

This study was performed to investigate clinical and histopathologic features of steroid acne, which was induced by systemic administration and topical application of corticosteroids. Thirty five cases of steroid acne visited to Department of Dermatology, Hangang Sacred Heart Hospital from September, 1g79 to June, 1984 were analyzed, and the results obtained can be summarized as follows: 1. The peak age of the subjects was third decade(42.9%) with an average age of 30. 1 years, and male to female ratio was l.9: 1,2. The skin lesions had unique clinical features that showed many, uniform sized, erythematous papules and pust;ules. 3 The predilection sites of steroid acne induced by systemic steroid therapy were anterior chest(93.1%), back(44,8%), neck(31.0%), shoulder(31.0%) and face (20.7%) 4 Among thirty five cases of steroid acne, twenty cases were induced by parenteral adrninistration of dexamethasone disodium phosphate(group A), nine cases by oral administration of prednisolone(group B), and six cases by topical application of three kinds of steroid creams(group C). 5. The mean induction time after starting steroid in group A(ll. 3 days) was shorter than those in group B and C(18.9 days and l4.8 days respective)y). The mean total dosage of used steroid in group A was 191. 3mg of dexamethasone disodium phosphate and that in group B was 515. 7mg of prednisolone. On histopathologic findings of twenty two skin biopsy specimens of the three groups, perivascular inflammatory reaction was the most common finding followed by intra-and peri-follicular inflammatory reaction, dermal vascular dilatation, necrosis of follicular epithelium, comedo, intraand periollicular abscess and rupture of follicle.

4.A Case of intramedullary spinal tuberculoma and multiple brain tuberculoma associated with pulmonary tuberculosis.

Tuberculomas in the spine are estimated to be 15 to 50 times less common than those occurring in the cranium. We experienced a case of intramedullary spinal tuberculoma and brain tuberculoma associated with pulmonary tuberculosis. A 39-year-old male was referred to the National Medical Center via emergency room because of urinary difficulty and lower limb weakness for 3 days. He had been treated with anti-tuberculosis regimens against pulmonary tuberculosis for 20 days. Spinal MRI revealed intradural intramedullary tuberculoma at T5. On the 21st day at the hospital, a generalized seizure attacked him. Brain MRI revealed multiple tuberculoma in both hemispheres, brainstem and cerebellum. He was treated anti-tuberculosis regimens and corticosteroids for 9 months. His condition improved clinically and radiologically. We report this case with a review of the literature.

5.A Case of Steven-Johnson Syndroe Associated with Cholestatic Hepatitis.

A 12-year-old boy developed cholestatic hepatitis with Steven-Johnson syndrome following the use of amoxicillin. The skin lesion and general condition were improved over 2 weeks, but jaundice was gradually aggrevated. We performed liver biopsy, on 30th hospital day, which showed cholestatic hepatitis. The patient improved gradually and liver function was normalized 5 months later.

6.A Case of Epidermal Nevus Syndrome.

Epidermal nevus syndrome is now delineated as a congenitally acquired syndrome consisting of deformities of the skin (various linear epidermal or organoid nevi), central nervous system, skeleton, cardiovascular system, and renal system. A 4-year-old girl had linear epidermal nevus on the right lower extremity, right ear lobe and anterior neck, and multiple skeletal abnorrnalities since birth. And she had an adult fist-sied, soft, nontender mass on the right lower abdomen since 2 years of age. Radiologic examination showed scoliosis of the thoracic spines, deformities of the lumbar spines, focal gigantisms of the left hand and right foot, and other skeletal abnormalities. Histological examination of the linear, verrucous lesion of the right leg showed hyperkeratosis and papillomatosis. The surgically removed abdominal mass revealed the finding of hydrosalpinx.

7.Characterization of a Replication Element in the Coat Protein ORF of Turnip Yellow Mosaic Virus.

Turnip yellow mosaic virus (TYMV) is a non-enveloped icosahedral virus that has a single 6.3 kb positive-strand RNA as a genome. Previously, it was observed that the recombinant construct TY-eGFP2, where an eGFP gene was inserted at the position downstream of the coat protein (CP) ORF of TYMV genome, barely replicated. The inhibition of replication was relieved by insertion of an additional copy of the 3' quarter of the CP ORF after the foreign sequence. In this study, we have examined if the 3' quarter of the CP ORF contains any replication elements. M-fold analysis predicted three stem-loop structures in this region. Analysis of the TY-eGFP2 constructs containing one or two of these stem-loop structures indicates that the secondary structure predicted in the region between nt-6139 and nt-6181, termed SL2, is essential for TYMV replication. The critical role of SL2 was confirmed by the observation that deletion of the 3' quarter of the CP ORF from the wild-type TYMV genome nearly abolished replication and that insertion of SL2 into the deletion mutant restored the replication. Mutations disrupting the stem of SL2 greatly reduced viral RNA replication, indicating that the secondary structure is essential for the enhancing activity.

9.Read-through Mutation in the Coat Protein ORF Suppresses Turnip Yellow Mosaic Virus Subgenomic RNA Accumulation.

We have previously observed that a sequence in coat protein (CP) ORF of Turnip yellow mosaic virus (TYMV) is required for efficient replication of the virus. The sequence was predicted to take a stem-loop structure, thus termed SL2. While examining various SL2 mutants, we observed that all the modifications resulting in extension of translation beyond the CP ORF significantly suppressed subgenomic RNA accumulation. The genomic RNA level, in contrast, was not affected. Introduction of an in-frame stop codon in the CP ORF of these constructs restored the level of subgenomic RNA. Overall, the results suggest that the read-through makes the subgenomic RNA unstable.
