2.Stasis Dermatitis: A Case Report.

A case of stasis dermatitis with hepatomegaly is reported. The patient is 50 year old male. About 3 years ago a dark-brownish macule on the left shin was appeared with the severe itching sensation and spread to the margin with edema, ulceration, and appeared the varricous veins on the margin, Biopsy on the ulceration revealed the hemociderin deposition and fibrosis in the dermis. The physical examination revealed that the liver was enlarged 3cm, below the right costal margin and liver biopsy showed early cirrhotic change, but without impairment of liver function. He was treated with massage, elastic bandage for 2 weeks with good response.

4.Clinical Importance of the Resection Margin Distance in Gastric Cancer Patients.

PURPOSE: The way in which the resection margin distance for gastric cancer patients who undergo a gastric resection influences the recurrence rate, aspects of recurrence, and the prognosis according to the characteristic of the tumor is not known. We aim to find a standard for tailor-made treatment after selecting patients in this point of view who need a more sufficient resection margin. MATERIALS AND METHODS: A retrospective study was done on 1,472 patients who underwent a gastrectomy due to gastric cancer at our hospital from 1992 to 2005. The median follow-up period was 37 months. RESULTS: There were no significant differences in the recurrence rate, the aspects of recurrence, and the 5-year survival rate between early gastric cancer (EGC) patients with a resection margin distance of less than 2 cm compared with EGC patients with a resection margin distance of greater than 2 cm. However, significant differences in the survival rate were found in advanced gastric cancer (AGC) patients when the patients were classified into groups with resection margin distances less than or greater than 3 cm (P=0.02). Significant differences were noted especially in cases of diffuse histologic-type tumors located in the lower third of the stomach and in cases with Borrmann type-3 and -4 tumors. CONCLUSION: The distance between the tumor resection margin and the proximal gastric resection margin has no significant influence on the survival rate in EGC patients if the resection margin is negative. However, to improve a patient's survival rate, it is important to guarantee a resection margin of more than 3 cm in AGC patients, especially when the tumor is a diffuse histologic type located in the lower third of the stomach or a Borrmann type 3 and 4.

5.Topical Treatment of Verrucae and Molluscum Contagiosum with Cantharidin.

The pathologic changes of verrucae and molluscum contagiosum are limitted chiefly to epidermis, therefore treatment with cantharidin which forms acantholytic bulla is successful. Canthrides is dried, powdered blister beetle, and cantharidin is its purified active ingredient. Its primary cutaneous effect is acantholysis and death of epidermal cells. For topical treatment, catharidin was prepared as a 0.9% solution in mixture of 50% of acetone, 25% of ether and 25% flexibIe collodion U.S.P. The therapeutic efficacy of cantharidin was excellent to eliminate the lesions of verrucae and molluscum contagiosum, showing the complete recovery of 114 out of 117 lesions of verrucae, and 520 out of 550 lesions of molluscum contagiosum with 1 to 3 topical applications. From the above results, it should be pointed out that cantharidin has the several advantages at clinical uses for the treatment of verrucae and molluscum contagiosum: 1 No residual scarring. 2. No pain. 3. Excellent therapeut!c efficacy.

7.SIADH Caused by the Synergistic Effect of S-1 and Thiazide.

Hyponatremia is a dangerous electrolyte disturbance in patients on chemotherapy and may cause sudden death if not detected early. SIADH (syndrome of inappropriate antidiuretic hormone) is one of the known causes of hyponatremia in patients undergoing chemotherapy. Few chemotherapeutic agents, however, are reported to cause SIADH. The current study reports that SIADH developed in a 55 year old woman on S-1 (80 mg/m2) and cisplatin (60 mg/m2) chemotherapy for the peritoneal metastasis of gastric cancer. The patient underwent a total gastrectomy, a splenectomy, and a segmental resection of the transverse colon for gastric cancer. She had used thiazide and betablocker to treat hypertension for 12 years. She admitted to our hospital with complaining of general weakness, dysarthria, loss of appetite, and urinary discomfort. The serum level of sodium and potassium were 94 mEq/L and 2.2 mEq/L respectively. The hyponatremia completely resolved uneventfully after 3% saline infusion, which led to normalized electrolyte balance. The patient was discharged on the 13th hospital day.

8.Subclassification of Stage IV Gastric Cancer According to the Presence of Distant Metastasis (IVa and IVb).

PURPOSE: The TNM staging system showed limitation in stratifying patients into different prognostic groups with gastric cancer. Since the treatment for stage IV gastric cancer with distant metastasis (M1) is defined as non-curative one, we hypothesized that the survival rate of stage IV gastric cancer with M1 is different to that of stage IV gastric cancer with no distant metastasis (M0), which will provide a rationale to subdivide stage IV into IVa and IVb. MATERIALS AND METHODS: From June 1992 to December 2005, of 1,630 gastric cancer patients who underwent surgery, 308 patients with stage IV gastric cancer were selected and analyzed. The clinicopathologic characteristics and survival of the patients, according to distant metastasis, were determined retrospectively. Median follow-up period was 13 months (range: 1~154 month). RESULTS: 5 year survival rate of M0 and M1 group was 35% and 16% respectively with statistic significance (P=0.0000). When the survival rate of M0 group was analyzed according to the difference of T and M factor, T1-3N3M0 and T4N1-2M0 group showed no significant statistical difference (P=0.1898). CONCLUSION: Given the result in this study, we suggest that the stage IV gastric cancer be subclassified into stage IVa and IVb according to M factor.

9.The Effect of Experimental Varicocele on Fertility in Adolescent Rats.

Varicocele is considered as one of common etiology for subfertility in men and recently it is acknowledged that varicocele first developed in puberty exert more severe damage to testicular structure than that in adulthood, but its ultimate effect on the fertility is not well known. We used animal model to compare the effect of varicocele on the fertility and histology in the adolescent with that in adulthood. Sprague-Dawley rats were treated to create varicocele at 7 weeks and 12 weeks of age by partial ligation of left renal vein. After 4 weeks of sexual maturation period passed, each male rat was mated with two female rats for 21days and then scarified to examine the testicular volume loss and histologic change. All female rats were also sacrificed for the evaluation of pregnancy rate, litter side and fetal weight. Pregnancy rate in adolescent rats were 47.1% and there was significant difference in fertility between control rats and rats with varicocele. Interestingly the impregnating ability of adolescent rats was correlated with the mean diameter of seminiferous tubules of both testicles. ipsilateral testicular volume loss and histologic changes were more severe in adolescent rats than in adult rate. In summary, varicocele created in adolescent Sprague-Dawley rats is considered to be implicated. in their adulthood fertility, probably due to its more severe damage to testicular volume and histologic change(especially tubular narrowing) than in adult rats.

10.Clinicopathologic Characteristics according to the Type of Recurrence in Curatively-resected Gastric Cancer Patients.

PURPOSE: Evaluating the clinicopathologic characteristics of recurred gastric cancer is essential for early detection of the recurrence and for better clinical outcome. The aim of this study was to establish the patterns of the recurrence, the timing of the recurrence, and the clinical result after recurrence according to the clinicopathologic characteristics of gastric cancer. MATERIALS AND METHODS: From June 1992 to December 2005, of the 1338 gastric cancer patients who underwent a curative gastric resection, 241 patients who recurred during the follow-up period were selected and their cases were analyzed. The clinicopathologic characteristics of the patients, the time to recurrence after operation and survival were determined retrospectively according to the type of recurrence. RESULTS: For the recurrent group, the numbers of total gastrectomies, advanced stages, lymphatic and/or venous infiltrations, whole stomach cancer cases, large tumors, undifferentiated tumors, and Borrmann type 4 tumors were higher than they were for the non-recurrent group, and the differences were statistically significant. When the recurrence types were classified as peritoneal seeding, hematogenous recurrence, and locoregional recurrence, independent risk factors were female gender, stage III, upper third, and whlole stomach cancer, and undifferentiated-type, diffuse-type, and Borrmann type 4 tumors for peritoneal seeding; early gastric cancer, stage I for hematogenous recurrence; and Borrmann type 1, 2, and 3 tumors for locoregional recurrence. Survival duration after detection of the recurrence was shorter for peritoneal seeding than for hematogenous or locoregional recurrence (7.0 months vs. 9.5 months and 12.5 months). CONCLUSION: For early detection of the recurrence after curative surgery for gastric cancer, it is important to recognize that the high risk factors for recurrence vary with the clinicopathologic data for the patients.
