1.Linar Scleroderma: Report of Three Cases.

Three cases of linear scleroderma with typical clinical and histopathological findings were presented. Case 1: A 10 year old female had a "coupe de sabre" type of linear scleroderma on the right side of the frontal scalp and face and hemiatrophy of the right side of the face. Case 2: A 10 year old female had a type of scleroderma en bande on the palm and dorsum of the left hand and clubbed finger of the left third finger. Case 3: A 21 year old male had a linear type of skin atrophy and sclerosis of the right lower exteremity and mild walking disturbance.

3.Anatomy of Pulmonary Lobes and Fissure: A Study Utilizing HRCT.

PURPOSE: This study was performed to evaluate the features of pulmonary lobes and fissure in Korean and to compare with the previous results. MATERIALS AND METHODS: HRCT scans of 82 healthy Korean adults, performed with scan interval of 1.5/2.0 cm, were reviewed. RESULTS: Located mostly on inner aspect, incidences of incomplete oblique and horizontal fissure were 60% (Rt 48%, Lt 39%) and 51% respectively, lower than 70--80% and 60--75% of western studies. Superolateral oblique fissure, inferior accessory fissure, left horizontal fissure and other accessory fissure were shown in 1%, 13%, 7%, and 8% respectively. Lower lobe upper part faced laterally (Rt 91%, Lt 82%) while lower part faced medially (Rt 61%, Lt 79%). Right middle lobe ws highest posteromedially (49%) and was convex (97%). CONCLUSION: Features of pulmonary lobes and fissure in Korean were similar to the results of previous reports but with lower incidence of variation.

4.Clinical charateristics of elderly patients with plmonary tuberculosis.

BACKGROUND: The prevalence of pulmonary tuberculosis among the elderly is increasing in Korea and in the developed countries due to the increased elderly population and their predispositions to chronic disease, poverty and decreased immunity. To define the characteristics of pulmonary tuberculosis in the elderly, we evaluated the clinical spectrum of pulmonary tuberculosis. METHODS: We analyzed 92 patients retrospectively that were diagnosed as active pulmonary tuberculosis over the age of 65. The analysis involved patient's profiles, clinical manifestations, coexisting diseases, diagnostic methods, anti-TB medications and their side effects, and treatment outcomes. RESULTS: The results were as follows:- 1) The ratio of male to female was 2.1:1(62:30 cases) 2) Chief complaints were a cough (47.8%),dyspnea(40.2%), sputum(38.0%), chest pain(12.0%), anorexia(10.9%), and fever(9.8%). 3) 38(41.3%) of cases had a past history of pulmonary tuberculosis. 4) The coexisting diseases were:-COPD, 25 cases(27.2%);pneumonia, 17 cases(18.5%);DM, 13 cases(14.1%);and malignancy, 10 cases(10.9%). 5) The positivity of Mantoux test(5 TU, PPD-S) was 82.7%. 6) Pulmonary tuberculosis was diagnosed using the following methods : sputum AFB (Acid Fast Bacillus) smear 42.4%, sputum TB(M.Tuberculosis) culture 15.2%, sputum TB PCR (Polymerase Chain Reaction) 10.9%, bronchial washing AFB smear 2.1%, chest radiology only 25.0%. 7) Locations of radiologic lesions were RULF, 50 cases;RLLF, 50 cases, mostly, then LLLF;26 cases were leastly involved. 8) The coexisting tuberculosis were endobronchial TB(8.7%), TB pleurisy(7.6%) miliary TB(5.4%), intestinal TB(2.2%), renal TB(1.1%) 9) The proportion of treatment regimen with 1st line drug and 2nd line drug were 92.3% and 7.6%, respectively. 10) The outcome of treatment were as follows:cured 31.5%, expired 13.0%, no return 47.8%, follow-up now 7.6%. CONCLUSION: The pulmonary tuberculosis in the elderly has atypical patterns with chronic coexisting diseases. Therefore, the possibility of pulmonary tuberculosis should be considered in elderly patients with pulmonary symptoms.

5.Hand-Assisted Laparoscopic Live Donor Nephrectomy.

PURPOSE: Hand-assisted laparoscopic live donor nephrectomy (HALDN) are being performed at our institution in an attempt to make live donor more attractive to the potential donor. We review the results of HALD N retrospectively and compare them with those obtained using the standard open approach. MATERIALS AND METHODS: 9 consecutive HALDN were performed from February 2000 through June 2000. The control group consisted of the 22 open donor nephrectomy at the same institution from January 1999 through December 1999 immediately before the initiation of HALDN. Live donors were considered candidates for HALDN if they possessed left normal renal anatomy with single renal vessels and ureter. Among 9 HALDN patients, one patient had two main renal arteries and another one patient had a polar artery. We evaluated sex, age, body weight, BMI (Body Mass Index) basically. And we evaluated renal ischemic time, operation time, to normal activities, postoperative serum creatinine level, and postoperative complications. RESULTS: There was no significant difference in male-female ratio, age distribution and weight-height ratio between two groups. Warm ischemic time for HALDN group was 3 to 4 min and it was not different with open surgery. 6.Hypereosinophilic Syndrome on the Extremities Associated with Abnormal Peripheral Blood Circulation.

Hypereosinophilic syndrome is characterized by marked eosinohia and eosinopbilic infiltrition of various organ-systems without any identifiable underlyin sathologic conditions such as hypersensitivity, drug eruption, parasitic infestat,ion, malignant tuinc, and autoimrnune and infectious d iseases. A 31 year-old male had weight loss, abnormal peripheral cirrultiction both extremities and two distinct skin lesions. One showed rice sized, erythematous follicular popules on both extremit.ies and trunk and the other showed a walnut sized, gangrenous ulcer on the left 2nd finger tip. The patients skin lesions and circulation defect. improved after retrnent with steroid and the blood eosinophilia returned to normal.

7.Secondary Biliary Cirrhosis in A Case with Intraductal Growth of Hepatocellular Carcinoma.

A case of hepatocellular carcinoma (HCC) complicating with secondary biliary cirrhosis due to intrahepatic bile duct growth of the tumor is presented. The patient was 34 years-old male who had been suffered from right upper quadrant pain with intermittent fluctuating jaundice for 4 months. Left lobectomy specimen contained a single, 4 cm sized, relatively well circumscribed and partly protruding nodular HCC in the medial seqment of left lobe. It compressed the left hepatic duct, and the intra and extra-hepatic ducts were completely plugged with tumor casts (extending to the common bile duct at the operating table). Remaining hepatic parenclyma was the seat of advanced secondary biliary cirrhosis in which were abundance of HBsAg-containing hepatocytes possibily representing a HBsAg carrier status. Biliary cirrhosis in HCC is an extremely rare manifestation, reflecting the delayed adequate clinical procedure by unusual pattern of obstructive jaundice. In such a case presenting as persisting and fluctuating obstructive jaundice in HCC, an intrabile duct tumor growth should be suspected.

8.The effects of Magnoliae cortex and Zea Mays L. extract mixtures on experimentally induced periodontitis of beagle dog.

It has been reported that Magnoliae cortex extract has antibacterial and antimicrobial activity against pathogenic microbes and Zea Mays L. extract is effective for improving gingival tissue health. The purpose of this study was to examine the anti-inflammatory and antimicrobial effects of Zea Mays L. and Magnoliae cortex extract mixtures through experimental periodontitis induced beagle dog model. Nine beagle dogs with experimentally induced periodontitis were selected. Baseline clinical indices which includes plaque index, gingival index, probing pocket depth, clinical attachment level, gingival fluid flow rate were recorded and microbial assays were done. Magnoliae cortex and Zea Mays L., mixed at 2:1 ratio in 105mg capsular dosage, were taken by 3 capsule (Group I) or 6 capsule dosages (Group II) three times a day. After 4,8,12 weeks, clinical indices were recorded. All data of clinical indices were compared through one-way ANOVA with 95% confidence level. Clinical indices of group I and II showed significantly better results than those of control group. There were no significant differences between group I and II. In conclusion, it was confirmed that mixture of Magnoliae cortex and Zea Mays L. (mix ratio 2:1) possessed clinical improving effects to periodontitis.

9.Large Cavernous Hemangioma of the Subscapularis Muscle: A Case Report.

We report a case of intramuscular hemangioma in the subscapularis muscle and the resulting impairment of shoulder function in an adult patient. A nineteen-year-old female complained of shoulder pain and the development of a mass in the absence of previous trauma. Physical examinations, including lift-off and belly-press tests, showed abnormality. X-ray showed multiple calcifications in the front of the scapula. Magnetic resonance imaging showed a soft-tissue mass occupying almost the entire intramuscular portion of the subscapularis muscle. An arthroscopic examination excluded the possibility of a joint invasion, after which the entire mass was successfully removed by open excision. The displacement of the subscapularis by the mass was relieved after the surgery. Pathological diagnosis of the tissue confirmed a cavernous hemangioma. Both shoulder pain and function was improved after operation. There was no evidence of recurrence even at the 2-year follow-up. Rare forms of hemangioma adjacent to the shoulder joint could be successfully managed with surgical excision. Differential diagnosis, such as synovial chondromatosis, pigmented villonodular synovitis, and malignant sarcoma, should also be considered.

10.A Clinicopathological Study of Chronic Cutaneous Lupus Erythematosus.

BACKGROUND: Chronic cutaneous lupus erythematosus(CCLE) is a well-known disease entity. But there has been no data about its clinical behavior and histopathologic features in Korea. OBJECTIVES: This study was conducted to elucidate the clinical, laboratory, and histopathologic features of CCLE, and the relationship between CCLE and SLE. MATERIALS AND METHODS: We investigated 48 cases of CCLE that visited the department of dermatology at the Seoul National University Hospital from January 1990 to June 1997.
