1.Evaluation of frontal bone erosion of young rabbit and its recovery after tissue expansion.

Among the complications associated with tissue expansion, bony depression under the expander is frequently observed clinically, especially under the tough skin like scalp. To investigate the bony change during the expansion and its reversability, we divided 21 young rabbits into 7 groups. After the insertion of tissue expander on the frontal bone, each groups are sacrificed at 1,2,3 weeks after tissue expansion, 1,2,3 months after removal of tissue expander, and for control, 4 weeks after insertion of expander without expansion. Bony changes are evaluated by gross findings, radiographic findings, and histologic findings. Progressive tissue expansion induces significant gross bony changes in skull of young rabbit. This bony changes consist of erosion underlying tissue expander, with bony ridging and bone deposition at the periphery of the expander. These gross findings correlates with the histomorphologic findings; osteoclastic bone resorption under the expanders with periosteal reation at the periphery of the expanders. The bony changes is not simply a pressure deformation, but also is a remodeling effect. Increased bone resorption and inhibition of bone formation occur until the pressure is removed. After removal of the expanders, reparative bone remodeling begins and nearly complete healing of the cranial defects occurs. On the basis of this study, we conclude that tissue expansion causes significant but reversible effects on cranial bone of young rabbit.

2.Fifteen Cases of Fournier`s Gangrene.

PURPOSE: This study was made to evaluate the clinical characteristics and the treatment of the gangrene of the male genitalia manifested with progressive life threatening disease. MATERIALS AND METHODS: Between June 1992 and December 1998, medical records for patients with Fournier's gangrene were reviewed with respect to age at presentation, associated disease, and clinical course and the treatment. RESULTS: The age of patients ranged from 29 to 92 years. The prodromal sign noted were usually low abdominal and inguinal pain. There were no differences in the causative pathogens compared to those of previous reports. The common underlying diseases were diabetes and chronic liver disease. Hypoalbuminemia and leukocytosis were found in all. Early radical debridement and aggressive antibiotics therapy were performed in all cases. Reconstructive surgery of defected skin was performed 7 to 45 days(mean 33.6) after debridement and period of hospital stay ranged from 14 to 123 days(mean 35.8). The motality rate was 13.3%. CONCLUSIONS: It is thought that the motality rate of Fournier`s gangrene is reduced by early aggresseve debridement of the wound and empirical triple antibiotic therapy, and the comprehension of clinical course is important as well.

4.A Clinical Study of Replantation of Amputated Digits

Since the first successful replantation of incompletely amputated digit was performed by Kleinert in 1962, Komatsu and Tamai reported the successful replantation of a completely amputated thumb in 1965. Nowadays, further improvements in instrumentation and surgical skill are accomplished and replantation of digit has been popularized widely. But sometimes the functional results of replanted digits are not excellent expected and there is a place for discussing gain between the result and efforts to replantation. We have been analyzed 28 cases of finger replantation in 17 patients from June, 1982 through June, 1984 at the Department of Orthopedic Surgery, Kyungpook National University Hospital. The results were as follows: 1, Male were 12 and female were 5, and average age was 20.5 years old. 2. The most common injury was done by cutting machine. 3. The number of amputated fingers were as follows: thumb, 6 cases; index finger, 11 cases; middle finger 7 cases; ring finger, 3 cases; The level of amputation was as follows: Zone II, 6 cases; Zone III, 15 cases; Zone IV, 7 cases; 4. The maximum ischemic times for successful results of finger replantation were 19 hours. 5. The total ratio of successful replantation of digits in our cases was 82.1%. 6. The functional results of replantation including joint motion, sensory recovery, two point discrimination, pinch power, sweating were relatively satisfactory. (Normal 8, Fair 11, Poor 3, Protective 1 case) 7. The main cause of replantation failure was thrombosis of the vein.

5.Binding of Lectins to the Zona Pellucida on Sperm-oocytes Interaction in the Pig.

OBJECTIVE: Lectins are cell-agglutinating and sugar specific proteins or glycoproteins of non-immune origin that precipitate glycoconjugates having saccharides of appropriate complementarity. Because of these properties, plant lectins have been used to help characterize the carbohydrate moieties of glycoproteins in the zona pellucida (ZP) of several mammalian species including pigs. Treatment of oocytes with various lectins blocks sperm binding to the ZP in various mammalian species. This study was undertaken to examine the distribution of sugar residues in the ZP of pig oocytes matured in vitro and the ability of spermatozoa to bind to ZP and in vitro penetration in oocytes treated with fluorescein isothiocyanate (FITC)-labelled lectins. MATERIALS AND METHODS: The lectins of Banderiaea simplicifolia (BS-II, bind to beta-D-Nacetylglucosamine), Canavalin ensiformis (Con A, bind to alpha-D-Mannose), Lens culinaris (LCA, bind to alpha-D-Mannose), Ricinus communis (RCA-I, bind to beta-D-Galactose) and Ulex europaeus (UEA-I, bind to alpha-L-Fucose) were examined for spermatozoa penetration, binding capacity to ZP and distribution of lectins. RESULTS: The penetration rates were significantry (p<0.05) higher in control oocytes (63%) than those treated with all lectins, but penetration rates (40~49%) were simililar in group treated with lectins. The incidence of monospermy was similar in oocytes untreated and UEA-I, but it was higher in oocytes treated with BS-II, Con A, RCA-I and LCA. The porcine oocytes cultured for 48 h in TC-199 medium were freed from cumulus cells and treated for 30 min with fluorescein isothiocyanate-labelled lectins. When examined under fluorescein illumination, higher (p<0.001) proportions of oocytes showed fluorescein of zona pellucida after treatment with Con A (93%), LCA (93%) and RCA-I (100%) than BS-II (37%) and UEA-I (50%). All of the oocytes treated with RCA-I exhibited strong fluorescein in the outer region of the zona pellucida while those treated with LCA exhibited strong fluorescein throughout the zona pellucida. BS-II bounded mainly to the outer region and UEA-I bounded mainly to the inner region of the zona pellucida, with either strong or weak fluorescein. At 120 min after insemination in vitro, fewer spermatozoa were bound to the zona pellucida of the oocytes treated with BS-II, Con-A and RCA-I. Of the lectins, Con A most inhibited sperm binding. CONCLUSIONS: These results suggest that beta-D-Galactose residues in the porcine zona pellucida may act as primary sperm receptors and inducers of the sperm acrosome reaction and these sugar residues may be involved in the block to polyspermy.

6.A Case of Acardiac Twin.

Acardiac twinning affects 1 in 100 monozygotic twin pregnancies and 1 in 35,000 pregnancies overall. This condition is characterized by the absence or rudimentary development of fetal heart, and associated with various anomaly. The presence of an acardiac twin requires the normal (or "pump") twin to provide circulation for itself, as well as the acardiac sibling. The acardiac malformations are uniformly fatal in the affected twin, and mortality in the co-twin is as high as 55%. The principal perinatal problems associated with acardiac twinning are pump-twin congestive heart failure, maternal hydramnios, and preterm delivery. We recently experienced a case of acardius anceps associated with a normal male infant, so present with a brief review of the literature.

8.Gastric pseudolymphoma: report of 3 cases

The pseudolymphoma of the stomach is known to be a benign porliferation of lymphoid tissue, which can bemistaken hostoloigcally for malignant lymphoma. The etiology of pseudolymphoma is controversial, but it B believedto be a manifestation of chronic inflammatory process. Authors present 3 cases of gastric lymphoma. Impresssionsof upper gastrointestual series were as follows; lymphoma or chronic gastritis in one case, ulcerative carcinomain another case and early gastric carcinoma in the other case. Initial endoscopic findings suggested infiltratingcarcinoma, ulcerative carcinoma and two benign gastric ulcers, respectively. One case was associated wtih earlygastric carcinoma.

9.The Incidence of Deep Vein Thrombosis in the Lower Extremity

Deep vein thrombosis is a frequently lethal complication following musculoskeletal surgery and skeletal trauma. For this study on incidence of deep vein thrombosis, we examined the data on 40 patients with contrast venography. Thrombosis limited to the veins in the calf of the leg was shown in 10 patients by venography. None of these patients had clinical symptoms of deep vein thrombosis or indicating embolism. We therefore concluded that the incidence of deep vein thrombosis of the lower extremity after injury or operation in Korea is not so rare as was considered before.

10.Effect of Pretreatment of (-)-3-PPP on the Haloperidol-Induced Extracellular Dopamine Concentraions in the Nucleus Accumbens of Rats.

OBJECTIVES: To investigate the effects of (-)-3-PPP(0.5, 2, and 10mg/kg, s.c.) and haloperidol(0.1, 0.5, and 2mg/kg, s.c.) on the extracellular dopamine concentrations, and the effect of pretreatment with (-)-3-PPP(2mg/kg) on the haloperidol(2mg/kg)-induced extracellular dopamine concentrations in the nucleus accumbens(NAS) of free moving rats. METHODS: Dopamine levels in dialysate were determined with high pressure liquid chromatography(HPLC) with electrochemical detection(ECD). RESULTS: (1) (-)-3-PPP had dual actions depending on the doses : at 2mg/kg, it decreased and at 10mg/kg, increased extracellular dopamine concentrations ; (2) haloperidol at all doses increased dopamine levels with higher dose having a greater icrease ; and (3) pretreatment of (-)-3-PPP reduced the increase in dopamine levels elicited by acute treatment with haloperidol. CONCLUSIONS: These findings suggest that pretreatment of (-)-3-PPP in low dose could accelerate the onset of therapeutic effect of haloperidol by diminishing the haloperidol-induced dopamine release in the limbic system.
