Iron deficiency is the most common nutritional deficiency in children and adolescents. Pediatric collagenous gastritis (CG) is a rare gastrointestinal disorder. Abdominal pain associated with iron deficiency anemia (IDA) is a common clinical symptom.We present three cases of pediatric CG diagnosed during the treatment of recurrent IDA. During esophagogastroduodenoscopy (EGD), nodularity of the gastric corpus was observed, which is a characteristic finding in pediatric patients with CG.Although pediatric CG could not be diagnosed by general EGD biopsy and pathological examination, it was diagnosed after suspecting pediatric CG and performing appropriate diagnostic tests. We recommend EGD for pediatric patients with recurrent IDA, especially for those with gastrointestinal symptoms. Moreover, when performing EGD, conducting appropriate examinations for pediatric CG is necessary.

PURPOSE: The prognosis of non-Hodgkins lymphoma (NHL) in elderly patients seems to be poorer than that in patients aged less than 60 years. This may be due to the lower tolerance for combination chemotherapy in the elderly. Aggressive combination chemo-therapy, which is the treatment of choice in intermediate and high grade NHL of adulthood, may be associated with unpredictab1y severe and lethal toxicity and worsened quality of life in the elderly. We investigated the treatment responses, toxicities and prognostic factors of NHL in elderly patients treated with combination chemotherapy. MATERIALS AND METHODS: We treated 116 elderly (>60 yrs) patients with NHL between January 1986 and June 1996 with adriamycin-containing regimens, such as CHOP (cyclo- phosphamide, adriamycin, vincristine, prednisolone), BACOP (bleomycin, adriamycin, cyclophosphamide, vincristine, prednisolone), and mBACOP (methotrexate, bleomycin, adriamycin, cyclophosphamide, vincristine, prednisolone). Patients in this study ranged from 60 to 81 (median 67) years of age. Fifty-five percent of patients were in stage I or II and the rest (45%) were in stage III or IV. The histologic grade was predominantly (91%) of intermediate and high grade type. RESULTS: The treatment responses were complete (CR) in 55% and partial (PR) in 25%. The median durstion of CR was 32 (3-132) months. The CR rate was significantly higher in patients treated with RDI (relative dose intensity) > 75% than that in the patients treated with RDI < 75% (p 0.003), but there was no significant difference in CR rate between treatment regimens (p-0.38). At a median follow up of 48-months (range, 12 to 132 months), the estimated 5-year ovetall survival was 46%. Ann Arbor Stage (I, II vs III, IV), ECOG performance (0-1 vs 2-3), RDI (>75% vs <75%) and the treatment response were important prognostic factors in the univariate analysis, and the treament response (CR vs non-CR) was the only independent prognostic parameter in the multivariate analysis. The most frequent and severe toxicity associated with chemotherapy was infection with or without neutropenia. The rate of severe infection was significantly decreased in the patients supported with G/GM-CSF but not in the dose-reduction group (RDI<75% vs >75%). CONCLUSION: Our data suggests that achievement of the CR after combination chemotherpy is the most important prognostic factor in the elderly patients with NHL. Suboptimal chemotherapy (RDI<75%) reduced the complete remission rate without reducing the likelihood of developing severe toxicities. Optimal chemotherapy with supportive cares involving the use of hematopoietic growth factors may be needed to improve the treatment response and the survival in the elderly patients with aggressive NHL.

The authors performed this preliminary study to investigate the effect of softening E.C.T. and propofol was compared to pentothal for induction of anaesthesia for E.C.T on seizure duration. The results were follows ' 1) E.C.T. was performed in 60 psychiatric inpatients who were admitted during the study period. Of them 51.7% were diagnosed as schizophrenia, 21.6% as major depressive disorder, 16.7% as bipolar I disorder, manic and 10% of others. 2) Mean number of E.C.T. was 12.2 times a patient. 3) The most common target symptoms were persecutory delusion in schizophrenia, psychomotor retardation or agitation in major depressive disorder, and violent aggressive behavior in bipolar I disorder, manic. 4) Pre-ECT medication usually used were atropine 0.0093mg kg(-1), pentothal 2.76mg kg(-1) or propofol 1.42mg kg(-1). 5) The duration of seizure, as measured clinically, was reduced with propofol(20.5 sec) in comparison with pentothal(35.7 sec)(p<0.001). This suggest the possibility that additional treatment may be needed for the same clinical effect in psychiatric illness when propofol is used as the induction agent.

Burn patients basically go through the sequential stages of physiological, psychological and social emergency after the burn injury. In the present study, 88 burn patients were classified into three groups according to the stages as mentioned above. Then, we observed the states of anxiety, depression, inward activities, various psychological responses measured by SCL-90-R, and body image changes in each stage. The study results were as follows: 1) We found that depressive symptoms increased as the stage advanced. They were correlated with family conflicts and low level of education. The state of anxiety was not correlated with the staging. 2) There were differences between stages I and III in ANX, DEP, SOM, PHOB axes of SCL-90-R. 3) The mood of burn patients and the patients' cooperation to the treatment observed by the nurse was partially correlated with depression and anxiety, while the patients' relation to others and interpersonal relationship were not. 4) Variables such as the size of burned area, duration of hospitalization and darkening skin affected the patients' body image. 5) We found that burn patients have psychological difficulties. Therefore, the treatment of burn injury should be done in parallel with the psychological treatment by professionals.
