2.The Incidence of Venous Thromboembolism Following Shoulder Surgery: A Pilot Study.

BACKGROUND: To assess the incidence of venous thromboembolism (VTE) following shoulder surgery and to evaluate the role of postoperative duplex ultrasonography. METHODS: The study comprised a total of 224 patients who underwent shoulder surgery, including 180 shoulder arthroscopic surgeries, 28 shoulder arthroplasties, and 16 plate fixations for proximal humerus fracture between January 2014 and December 2014. The mean age of patients was 59.0 years, and there were 81 men and 143 women. Clinical data, including body mass index, blood tests, metabolic work-up for liver and renal function, previous, and present medical history, were evaluated. Duplex ultrasonography in the operative arm was performed on 2 to 4 days after surgery. RESULTS: The overall incidence of VTE following shoulder surgery was 0.45% (1/224). One patient with open reduction and plate fixation for proximal humerus fracture had asymptomatic deep vein thrombosis that showed complete remission after anticoagulant medication during a 2-month period. Four patients had asymptomatic superficial cephalic vein thrombosis and complete remission without any treatment. There was no case of pulmonary embolism. CONCLUSIONS: The incidence of VTE following shoulder surgery was extremely low in Asians. Duplex ultrasonography may be not considered a routine follow-up of shoulder surgery and can be selectively performed in high-risk or symptomatic patients for VTE.

7.Central Vein Occlusion in Hemodialysis Patients

As the increasing incidence of the double lumen catheterization (Quinton catheter) for temporal hemodialysis, central vein occlusion, a serious complication which can cause arteriovenous fistula failure, occurred more frequently. Severe arm edema, pain with venous hypertension and insufficient fistula function suggest central vein obstruction and this can be confirmed by color duplex sonography or venography. This kind of occlusion should be treated promptly because it is closely related with arteriovenous fistula function. Division and ligation of fistula is one option but to salvage the fistula is more recommendable. For this purpose, thrombolysis, percutaneous transluminal balloon angioplasty (PTA) with or without stent insertion, direct surgical approach to the obstructed vein, surgical bypass using autogenous vein or artificial graft are performed according to the site and degree of the obstruction. From January 1995 through December 1997, we experienced 9 cases of subclavian vein or innominated vein occlusion which were detected by angiogram in hemodialysis patients. Four cases were treated by PTA with one case of thrombolysis at the same time. Two of them developed recurred symptoms at 3 and 6 months after PTA. The previous fistula were ligated and made a new basilic vein reposition arteriovenous fiatulas were made at opposite arm. Among the remaining 5 cases, surgical bypass was performed between internal jugular vein and subclavian vein in 2 cases, between axillary vein and opposite subclavian vein in 3 cases using 8 mm PTFE graft. One out of 5 surgical bypass group showed recurred occlusion at 16 months and that patient died soon after that occlusion. Another one showed partial occlusion of bypassed graft but the symptoms were mild so we could continue hemodialysis without problems. The remained 3 cases were patent for more than 13 months of their follow up. In summary, to salvage the failed fistula by central vein obstruction, aggressive procedure such as surgical bypass using artificial graft to opposite central vein also be considered with PTA.

8.Spontaneous Rupture of Iliac Vein: A case report.

Spontaneous rupture of iliac vein is extremely rare condition. Until now, only 15 cases were reported worldwide. Most of the reported cases were developed in previously healthy middle to older aged women. No definite pathophysiology was proved but relation of inflammation and temporary increase of intraluminal pressure related with straining during posture change were proposed. We experienced one case developed in 65 year old otherwise healthy woman.

9.Relationship of dental crowding to tooth size and arch width.

The purpose of the present study was to investigate the relationship of dental crowding to tooth size and arch dimension in Korean subjects. Two groups of dental casts with Class I molar relationship, were selected on the basis of crowding. One group, consisting of 82 pairs of study cast (29 males and 53 females), exhibited at least 7 millimeters of crowding in each arch. A second group, consisting of 82 sets of study cast (37 males and 45 females), exhibited normal occlusion with little or no crowding. Mesiodistal tooth diameters, and buccal and lingual dental arch widths were measured and compared between the crowding and normal occlusion groups. Significant differences were observed between the two groups not only in arch widths but also in tooth sizes. The results of the present study suggest that both etraction and expansion can be used as a treatment approach for the crowding cases.

10.The Surgical Results of Balloon Catheter Dacryocystoplasty in Both Children and Adults.

PURPOSE: The purpose of this study was to evaluate the surgical results of balloon catheter dacryocystoplasty (DCP) for congenital nasolacrimal duct (NLD) obstruction in children and partial NLD obstruction in adults. METHODS: The authors performed balloon catheter DCP as a secondary treatment of eight children (nine lacrimal systems) over 12 months of age who had been failed by probing, and as a primary treatment of five adults (five lacrimal systems) whose lacrimal system was partially obstructed. In children, Lacricath was inflated at the level of the valve of Hasner and 5 mm proximal to that level, three times with eight atm (bars) for 90 seconds at each level. In adults, the procedure was the same for children, but if there is any doubt of common canalicular stenosis, inflation at that level was included. RESULTS: Seven of eight children (eight of nine lacrimal systems) and three of five adults (three of five lacrimal systems) showed objective and subjective improvement at the mean follow-up period of 5.3 months and 5.0 months, respectively. CONCLUSIONS: Balloon catheter DCP was thought to be an alternative to silicone tube intubation in children with NLD obstruction who had been failed by probing and also a useful primary treatment in adults with partial NLD obstruction in the office setting.
