3.The Effect of the Vocational Rehabilitation Program on the Quality of Life of the Chronic Schizophrenics.

OBJECTIVES: The purpose of this study was to examine the effect of the vocational rehabilitation program on the quality of life of the chronic schizophrenics, and to evaluate how chronic schizophrenics felt about their lives. METHOD: The subjects were 39 chronic schizophrenics who had participated in the vocational rehabilitation program(participant group) and now lived in the community. The control group were 43 chronic schizophrenics who were outpatients without vocational rehabilitation(non-participant group). The scale for quality of life was divided into two subscales, i.e., the objective and the subjective areas. The subscales are composed of living situation, family situations, interpersonal relations, work, health, finances, leisure activities, autonomy, and self-esteem. RESULTS: 1) In the demographic characteristics such as age, sex, duration of illness, and duration of education, two groups were not different significantly. However, dosage of antipsychotics were lower in participant group than non-participant group. 2) In the objective quality of life subscale, the scores of living situation, interpersonal relations, work, health, economic state, and leisures were higher in participant group than non-participant group. But, family relations score were lower in participant group than in non-participant group. autonomy were not different between two groups. 3) In the subjective quality of life subscale, the score of living situation, family relations, interpersonal relations, work, health, economic states, leisure activities, and autonomy were all higher in participant group than in non-partcipant group. 4)Self-esteem was significantly higher in participant group than in non-participant group. CONCLUSION: The results from this study suggest that the objective and subjective quality of life of participant group in the vocational rehabilitation program were higher than those of non-participant group. The vocational rehabilitation program of Yong-In Mental Hospital seems to have been effective for the chronic schizophrenics. And the psychosocial rehabilitation program, vocational rehabilitation program, and family education program should be developed more and implemented in the community.

4.Secondary Lymphedema after Femoral Venous Catheterization.

Secondary lymphedema is an acquired condition resulting from damage to or obstruction of previously normal lymphatic channels, resulting in interstitial accumulation of protein-rich fluid, subsequent inflammation, adipose tissue hypertrophy, and finally fibrosis. It usually develops from infection, trauma, or malignancy or is a malignancy related to therapy. It has been rarely reported after a femoral vein cannulation procedure. We report a case of secondary lymphedema in a lower extremity after a femoral vein cannulation trial in a patient with chronic kidney disease.

BACKGROUND: The prognosis after an aortic valve replacment can be affected significantly by the transprosthetic pressure gradient which is determined mainly by the size of the patients body and the prosthesis used. We analyzed the hemodynamic feature of two relatively new prosthese the ATS and the evensized Medtronic-Hall(M-H) valves by measuring the transprosthetic pressure gradient in the cases where small sizes (23mm or smaller) were used. MATERIAL AND METHOD: There were 94 patients who received whom aortic valve replacement with prosthesis smaller than 23 mm from October 1994 to June 1998. In these patients the transprosthetic pressure gradient clalculated from the pressure half time during postoperative Dopper echocardiographic examination was compared between the prostheses of different sizes. The body surface area of each patient was also taken into consideration. RESULT: The mean pressure gradient and body surface area in each group were 21.7+/-10.2 mmHg and 1.52+/-0.14m2 in ATS 19mm 11.4+/-6.5 mmHg and 1,57+/-0.20m2 in M-H 20mm 15.2+/-6.3 mmHg and 1.54+/-0.13m2 in ATS 21mm 9.3+/-2.5 mmHg and 1.63 +/-0.14m2 in M-H 22 mm and 12.9+/-5.3 mmHg and 1.69+/-0.13m2 in ATS 23mm. CONCLUSIONS: The 19mm ATS prosthesis showed significant trasprosthetic pressure gradient which is similar to the values previously reported with other bileaflet prosthesesm Close follow-up was needed in terms of exercise capacity and change in left ventiricular geometry. In patients with small aortic valve annulus the 20mm M-H valve is recomendable as an alternative to 19mm bileaflet valves because it has less pressure gradient with similar outer diameter.

9.Primary carcinoma of the fallopian tube coexisting with benign cystic teratoma of the ovary.

Primary carcinoma of the fallopian tube is a rare malignancy of the female genital tract and infrequently diagnosed before an operation. The majority of patients have extensive disease at the time of diagnosis. We have experienced incidentally a case of a carcinoma of the fallopian tube coexisting with a benign cystic teratoma of the ovary in a 25-year-old woman. We report this case with a brief review of literatures.

10.Effect of Simvastatin collagen graft on wound healing of defective bone.

PURPOSE: To observe and evaluate the effects of Simvastatin-induced osteogenesis on the wound healing of defective bone. MATERIALS AND METHODS: 64 defective bones were created in the parietal bone of 32 New Zealand White rabbits. The defects were grafted with collagen matrix carriers mixed with Simvastatin solution in the experimental group of 16 rabbits and with collagen matrix carriers mixed with water in the controlled group. The rabbits were terminated at an interval of 3, 5, 7, and 9 days, 2, 4, 6, and 8 weeks after the formation of defective bone. The wound healing was evaluated by soft X-ray radiography. The tissues within defective bones were evaluated through the analysis of flow cytometry for the manifestation of Runx2 and Osteocalcin, and observed histopathologically by using H-E stain and Masson-Trichrome stain. RESULTS: 1. In the experimental group, flow cytometry revealed more manifestation of Runx2 at 5, 7, and 9 days and Osteocalcin at 2 weeks than in the controlled groups, but there was few difference in comparison with the controlled group. 2. In the experimental group, flow cytometry revealed considerably more cells and erythrocytes at 5, 7, and 9 days in comparison with the controlled group. 3. In the experimental group, soft x-ray radiography revealed the extended formation of trabeculation at 2, 4, 6, and 8 weeks. 4. histopathological features of the experimental group showed more fibroblasts and newly formed vessels at 5 and 7 days, and the formation of osteoid tissues at 9 days, and the newly formed trabeculations at 4 and 6 weeks. CONCLUSION: As the induced osteogenesis by Simvastatin, there was few contrast of the manifestation between Runx2 and Osteocalcin based on the differentiation of osteoblasts. But it was considered that the more formation of cells and erythrocytes depending on newly formed vessels in the experimental group obviously had an effect on the bone regeneration.
