1.Action of several chemicals on the parasites eggs and larvae in Korean Pickle(Kimchi).

In Korea where night soil used as fertilizer for crops and vegetables they may be contaminated simultaneously with infective stage of ascaris, hookworm and other helminthes. In this circumstance, the ascaris eggs and hookworm larvae may adhere to certain kinds of leafy vegetables(Chyu, 1957) and may remain viable even after pickling in brine or in other substances (Han and Ko, 1952; Soh, 1960). It has been shown that Heliogen(Thitasut, 1961) will kill the parasite eggs and larvae, and Sodium nitrite (Kozai, 1960; Kim and Soh 1964) and Thiabendazole (Kutsmi, 1964 and 1965; Kim et al., 1966) also had ovicidal action. The present study has been planned to study the ovicidal and larvicidal actions of several chemicals in Korean pickle juice(Kimchi) and the results are summarized as follows: Heliogen solution(iodine 100 ppm) did not destroy the ascaris and hookworm eggs within 30 minutes at the temperature of 15-30 C, but the hookworm larvae were killed within 3 minutes under the same conditions. Sodium nitrite which was added to pickle juice at the rate of 0.1 percent destroyed more than 90 percent of ascaris fresh eggs within 10 days and the embryonated eggs were destroyed within 2 days. Hookworm eggs were destroyed within 3 days and larvae were killed within 7 hours. Thiabendazole which was added to pickle juice at the rate of 0.1-0.025 percent inhibited the infectivity of ascaris embryonated eggs within 24 hours and hookworm eggs were destroyed in the same concentrations of the reagent. Hydrogen ion concentration of pickle juice which was mixed with Sodium nitrite (0.1 percent) was not changed for 10 days. 1.5 mg of Sodium nitrite or Thiabendazole produced no toxicity in kidney and intestine of mice but slight pathological changes in the liver of the same animal.

3.Effect of Male Sex Hormones on Calcium Oxalate Nephrolithiasis in Ethylene Glycol-Treated Rats.

PURPOSE: Sexual differences in the incidence and crystalline composition of urinary stones in humans are well-known, but it is unclear why men have a higher incidence of calcium oxalate stones than women. We investigated the effects of male sex hormones on stone formation using an ethylene glycol (EG) - induced urolithiasis model in rats. MATERIALS AND METHODS: Adult male Sprague-Dawley rats were divided into 4 groups, each containing 10 rats. One group of rats was left untreated and served as control. The other 3 groups were fed a 1% ethylene glycol (EG) lithogenic diet for 4 weeks. Among these, one group was non-castrated, one group was castrated and one group was non-castrated and given finasteride orally. Serum testosterone, creatinine, electrolytes, 24-hour urine levels of oxalate and citrate, and creatinine clearance were measured. The crystal deposits were examined by light and polarizing microscopes. RESULTS: Testosterone promoted calcium oxalate stone formation in EG - treated rats. Finasteride administration significantly decreased urinary oxalate excretion and calcium oxalate deposition, compared with controls. Urinary citrate was significantly decreased in EG-treated rats, but was not influenced by castration or administration of finasteride. There were no significant differences in serum concentrations of creatinine, sodium, or potassium among the control and experimental groups. CONCLUSIONS: Our data suggest that testosterone promotes calcium oxalate stone formation, and that dihydrotestosterone may be partially responsible for the exaggerated hyperoxaluria in EG-treated rats. Additionally, male sex hormones have a lesser influence on urinary citrate than oxalate.

6.Treatment of Gonorrhoea.

Because of increasing resistance of circulating N. gonorrhoeae and frequent failures in the treatment of gonorrhoea, intensive work on gonorrhoea has become of paramount importance. During January 1980-April 1984, at the Choong-Ku VD Clinic in Seoul, 3,340 male patients with uncomplicated gonococcal urethritis were treated with various treatment regimens. Diagnosis of gonorrhoea and declaration of a treatment failure were made on the basis of positive urethral culture. In 1984, the prevalence of Penicillinase Producing N. gonorrhoeae (PPNG) was about 30%, The pretreatment minimun inhibitory concentration of various antibiotics were quite high. Even for non-PPNG urethritis standard penicillin regimens gave unsatisfactory results. For PPNG urethritis, only spectinomycin, cefoperazone and cefotaxim-probenecid regimens gave satisfactory results. No spectmomycin resistant strain of N. gonorrhoeae has been found since 1982 at the Choong-Ku VD Clinic. As an agent of single drug therapy, spectinomycin seems to be one of the most cost effective drugs in the treatment of uncomplicated gonorrhoea in men.

7.Treatment of Uncomplicated Male Gonococcal Urethritis: Kanamycin vs . Gentamicin.

Between December 18, 1985 and May 31, 1986 at the VD Clinic, Choong-ku Public Health Center, in Seoul, 274 male patients with uncomplicated gonococcal infection, including 124 penicillinase producing Neisseria gonorrhoeae(PPNG) infection, were entered into the study. They were divided randomly into the group A and the group B. In the group A, treated with kanamycin 2g, im regimen, 126 of 137 patients were followed and 86 patients(68.3%) were cured. In the group B, treated with gentamicin 240mg, im regimen, 125 of 137 patients were followed and 78 patients(62.4%) were cured. The cure rates in PPNG urethritis were 63.9%(39/61) in the group A and 50.9%(29/57) in the group B. The cure rates in non PPNG urethritis were 72.3%(47/65) in the group A and 72.1% (49/68) in the group B. No significant difference was observed in cure rates between two groups. It is suggested that. both kanamycin 2g, im regimen and gentamicin 240mg, im regimen is not suitable for a first line treatment for uncomplicated gonococcal urethritis.

8.Treatment of Uncomplicated Male Gonococcal Urethritis with Norfloxacin.

From November 18 to December 31, 1986, seventy-five male patients with unconplicated gonococcal urethritis at the Venereal Disease Clinic of Choong-ku Public Health Center in Seoul were allocated randomly into one of two treatment regimens, and seventy patients were followed. All thirty-five patients including PPNG infections, treated with single oral dose of norfloxacin 600mg, were recovered(100%). One of thirty-five patients treated with a single oral dose of norfloxacin 800mg, failed. This failed case was one of eighteen nonPPNG infections (failure rate of 5.6%). Susceptibility test with disks containing norfloxacin 10ug showed the inhibition zone greater than 30mm. It is suggested that a single oral dose of 600mg or 800mg norfloxacin has good effect in the treatment of gonococcal urethritis with minimal side effects. Because of high prevalence of PPNG, it can be recommended as the first line treatment for gonorrhoea in Korea.

10.A Case of Premature Ovarian Failure Assosiated with Gonadal Dysgenesis.

Prenature ovarian failure is a condition causing amenarrhea, hypoestrogenism, and elevated genadotropins in women younger than 40 years. A karyotype should be performed as part of basic laboratory evaluation for all patients with premature ovarian failure and prodromal premature ovarian failure. Development of a malignancy in a dysgenetic gonad is of major concern. The presence of a fragment of the Y chromosome is thought to be a key to the oncogenic potential of these gonads. The search for the testicular determining factor(TDF) has engendered much confusion about which part of the Y chromosome plays a role in malignancy. This was initially postulated to be the H- Y antigen. More recent data, however, localize the area near the centromere of the Y Chromosome, on the long arm(Yq). Malignant potential is clearly not linked to the testicular determining factor itself(SRY). This is a critical point in clinical medicine. Feilure to display SRY or a closely related sequence does not rule out the presence of the segment of the Y chromosome postulated to be associated with the development of malignancies. We have experienced a case of premature ovarian failure with chtomosomal abnormality involving Y chromosome fragment. So we report this case with a brief review of literatures.
