2.A Clinical Study of the Treatment of Ruptures of Deltoid Ligament Associated with Fracture of Distal Part of Fibula

A survey of the literature on the treatment of ruptures of deltoid ligament associated with fracture of distal part of fibula is controversial. Some authors advocated surgical repair of the ruptured deltoid ligament based on the theoretical consideration, while others advocated non-operative treatment based on the clinical consideration. We studied the results in forty-five patients who were treated for disruption of the deltoid ligament and a distal fibular fracture. The length of follow-up in our series was twelve to twenty-four months, with an average of fifteen months. When the fibular fracture was adequately reduced and medial clear space was returned to its normal width, the 91 percents both of patients of being treated with repair of deltoid ligament and patients of being treated without repair had a good or excellent results.

6.Clinical Review of Traumatic Pancreatic Injuries.

BACKGROUND: Due to its anatomical location and vague symptom, early diagnosis of traumatic pacreatic injury is difficult. Furthermore it is very difficult to carry out appropriate management for the injury, so morbidity and mortality rates of traumatic pancreatic injuries remain high. Patients and METHODS: We reviewed medical records of 35 cases of patients who had been diagnosed as pancreatic injury and operated at Department of Surgery, Busan National University College of Medicine between January 1995 and December 2000. RESULT: 1) The male to female ratio was 2.5:1 and the highest incidence occurred in the 4th decade(31.3%). 2) Thirty two cases(91.4%) were injured by blunt trauma and 3 cases(8.6%) were injured by penetrating trauma. 3) The most common clinical manifestation was abdominal pain. 4) Twenty two patients(62.9%) were operated within 24hours after injury, and the mean time interval between injury and operation was 26.4 hours. 5) The serum amylase levels higher than 150 somogi unit were recorded in 20 cases(57.1%) of the patients. 6) Twenty seven patients(77.1%) had another associated injury with an average of 2 associated intraabdominal injuries. The most frequently injured intraabdominal organ was mesentery. 7) Complications occurred in 20 of 35 patients(57.1%) and the most common complication was pancreas-related complications, such as fistula, pseudocyst, abscess, pancreatitis. 8) Six patients(17.1%) were died due to hypovolemic shock, sepsis, respiratory failure and renal failure. CONCLUSION: Early diagnosis and appropriate operative methods may reduce morbidity and mortality from traumatic pancreatic injury

7.Laparoscopic Treatment of Isolated Renal Hydatid Cyst.

An isolated unilocular renal hydatid cyst is very rare and to the best of our knowledge, no such case has been reported in the Korean literature. A 56 year old woman, who liked eating raw vegetables fertilized by dog feces, visited our department because of an incidentally found renal cyst. Ultrasonography and the computed tomography (CT) scan showed a typical daughter cyst within the 6cm sized mother cyst. We performed retroperitoneal laparoscopic cyst marsupilaization after aspiration of the cystic contents and injection of a scolecidal agent. No dissemination and no recurrence have occurred to date.

8.Incidentally Found Port Site Metastasis followings Laparoscopic Radical Nephrectomy for a Renal Cell Carcinoma.

Herein, a rare case of port site metastasis, following a laparoscopic radical nephrectomy in a 62-years old male patient with a stage pT2N0M0 and Fuhrman grade III renal cell carcinoma, is reported. The patient suffered port site metastasis, with local recurrence. The port site metastasis was incidentally found in the patient during the laparoscopic examination for a laparoscopic mass resection of a local recurrent tumor. The most important appropriate surgical techniques are those where the port site metastasis can be controlled.

9.Acquired Male Anterior Urethral Diverticulum with Giant Calculi and Cutaneous Fistula.

Urethral diverticulum is a relatively uncommon abnormality for a man. We report here on a case of acquired anterior urethral diverticulum with giant calculi and cutaneous fistula in a 64-years old male patient who had suffered from urethritis 10 years previously. Retrograde urethrography and urethroscopy confirmed the anterior urethral diverticulum with giant calculi, and these procedures ruled out the presence of any associated obstructive urethral anomalies. We performed urethral diverticulectomy, with removal of the calculi, and then we performed urethroplasty.

10.Fyn Tyrosine Kinase-mediated Tyrosine Phosphorylation of Roundabout (Robo), the Slit Receptor.

In this study, the molecular mechanism of tyrosine phosphorylation of Roundabout (Robo), the transmembrane receptor for slits, was investigated. The tyrosine phosphorylation of intracellular portion of Robo was increased by the treatment of tyrosine phosphatase inhibitors in human embryonic kidney cells transfected with Robo. The Robo tyrosine phosphorylation was inhibited by the treatment of Src family kinase inhibitor, PP2. The co-transfection of constitutively active form of Fyn, not the dominant negative form of Fyn, and Robo dramatically enhanced the tyrosine phosphorylation of Robo. Furthermore, the SH2 domain of Fyn, which binds to phosphorylated tyrosine residues, interact with Robo, and the interaction was increased by the inhibition of tyrosine phosphatases. These findings indicate that the tyrosine phosphorylation of Robo is regulated by Fyn.
