1.Morphological Studies on the Rabbits Ciliary Epithelium by Apraclonidine.

Apraclonidine is a potent alpha-adrenergic agonist that, like clonidine, is relatively selective for alpha-2 receptors. Apraclonidine has been shown to lower intraocular pressure by diminishing aqueous humor formation. The purpose of this study is to know the morphological changes in the ciliary epithelium by apraclonidine against aqueous formation. In group I, two drops of 1% apraclonidine hydrochloride were instilled on the right eye of the pigmented rabbits and the eyes were enucleated on 1,2.6 and 24 hours after instillation to find out the duration of action of the drug and the tissue response. In group II, one drop was instilled twice daily for seven days on the right eye and the eyes were enucleated on 1, 2 and 5 days after cessation of the instillation observing the cumulative effect of the drug and the tissue response. These eyes were studied with electron mIcroscopIc examiation. As a result, widening of cell membranes(basal infoldings) of the non-pigmented epithelium(NPE) were observed. This change was recovered with time, and seems to be the principal morphological changes of apraclonidine against aqueous formation in the ciliary body. In group II, the cellular changes were minimal except for well developed rough endoplasmic reticulum representing resumed secretary function of the cell.

6.A Study on Factors Affecting Cultural Competency of General Hospital Nurses.

PURPOSE: As Korea has been rapidly transformed to be a multicultural society, it becomes essential for nurses to develop cultural competency. The purpose of this descriptive study was to assess the cultural competency level of general hospital nurses and to examine its contributing factors adopting ecological model. METHOD: A convenience sample of 327 nurses from six general hospitals in Seoul metropolitan area was recruited between November 14 to 28, 2011. Cultural competency was measured using the Korean version of the Caffrey Cultural Competence in Healthcare Scale (CCCHS) and Cultural Competence Assessment(CCA). Hierarchical multiple regression analysis was performed to assess the influence of intrapersonal, cultural-experience, and organizational factors on cultural competency. RESULTS: Mean cultural competency in this study was 2.57(+/-0.43). Cultural competency was associated with job position (p=.044) at the intrapersonal level. At the cultural experience level, foreign language classes experience (p=.005), fluency in a foreign language (p=.000), experience of working with foreign health care professionals (p=.000) were associated cultural competency. At the organizational level, training program (p=.000), job description (p=.002), health education materials (p=.016), insufficient job control (p=.000), and interpersonal conflicts (p=.011) showed significant difference. Foreign language class experience, foreign language fluency, experience of working with foreign health care professionals, and job control were the most consistent predictors of cultural competence and explained 20.4% of the total variance in the proposed ecological model. CONCLUSIONS: The findings suggest that to enhance cultural competency in general hospital nurses, cultural experience and organizational factors need to be more fully considered than intrapersonal factors.

7.The Effect of Pantethine(Pantomin(R)) on Serum Cholesterol and Triglyceride in Hyperlipidemic Patients.

The incidence of atherosclerosis in Korea seems to be much increased due to diet change after national development. The pathogenesis of atherosclerosis is not clarified and there are many hypothesis but the most recent and reliable hypothesis is the ratio of HDL-cholesterol per total cholesterol. Under the basis of this hypothesis there have been much trials to administer the agents which has effect on lipid metabolism, so we tried pantethine on 30 patients who visited Han-Yang University Hospital Internal medicine Department, and the result as follows; 1) The mean age of study population was 50.1 years of age, mean body weight was 62kg, and mean height was 160.8cm. male was 17, and female 10. 2) The associated disease of study population was <19 with> cardiovascular disease, in 19, gastrointestinal disease 3, obesity in 1 and others in 4. 3) The undesirable effect of the drug was found on 3 patients; that is, constipation on 1 patient, dizziness on 1 patient, and skin eruption on 1 patient. 4) The mean serum lipids before and after pantethine administration(levels) are as following table. In conclusion, it seems that the effect of the drug which decreases the serum lipids is mild at initial but more increasing as the time goes by and constant, and we experience little side reaction except mild dizziness, constipation and skin eruption.

8.Purse-string Suture Technique for Jones Tube Fixation in Conjunctivodacryocystorhinostomy.

PURPOSE: To evaluate the usefulness of Jones tube fixation using a purse-string suture, and to minimize postoperative conjunctival incarceration into the collar opening of the tube and extrusion or intrusion of the tube in the early postoperative period. METHODS: Thirty-five conjunctivodacryocystorhinostomy (CDCR) with tube fixation by purse-string suture were performed over the period of 1 year with a mean follow-up of 8.3 months. At the end of endoscopic CDCR procedure, to secure the tube in both the surrounding conjunctiva and tissues in the canthus, a 5-0 Vicryl suture was passed through the conjunctiva and Tenon's capsule just lateral to the collar of the tube and then tied around the collar of the tube in the fashion of a purse string. Finally, it was externalized to the skin of the canthal area where it was fixed. The suture was left alone if it did not cause problems postoperatively. RESULTS: Of 35 cases, there were no cases of intrusion of the tube or clogging of the tube with the conjunctiva at 6 months postoperatively. Partially extrusion was found in only two cases at 6 and 9 months postoperatively. One case of inadvertent tube retrieval was noted while irrigating the tube at postoperative 1 month. CONCLUSIONS: This procedure is a simple and superior way of fixing a Jones tube to maintain tube position during the early postoperative period.

10.Normal Variation of Focal T2 Hyperintensities in Anterior Parietal Periventricular white Matter: Another 'Terminal Zones of IV!yelination'.

PURPOSE: It has been known that there are several areas of T2 hyperintensities in normal white matter of brain, such as terminal zones of myelination, ependymitis granularis, ones of posterior internal capsule, and perivascular space. The aim of our study is to demonstrate another region of T2 hyperintensities in normal pediatric age group. MATERIALS AND METHODS: We have studied brain MR for 10 normal volunteers and 35 patients without having intracranial lesions in pediatric age group(3-19 years). RESULTS: In 5 among 45 cases, focal T2 hyperintensities were seen in the parietal periventricular white matter beneath the postcentral gyri. They were noted as poorly defined, 5--10mm sized areas of increased signal intensities on T2 weighted axial images. They were also characterized by bilateral, posteromedially oriented, short band-like or oval areas. Interestingly, they were directly continuous with the T2 hyperintensity of posterior internal capsule. In spite of the relatively high frequency in the pediatric population as in our study, this finding has not been reported in the asymptomatic adults. CONCLUSION: The results show that the bilateral anterior parietal hyperintense areas may be another terminal zones of delayed myelination affecting the parietopontine tract. They should be differentiated from pathologic T2 hyperintensities by their characteristic findings.
