1.Subtrochanteric Osteotomy in Congenital Coxa Vara Report of 3 Cases

Three cases of congenital coxa vara were treated by subtroch-anterie abduction osteotomy with percutaneous pin fixation of both fragments. The indications of the corrective subtrochanteric osteotomy for coxa vara deformity were the neck-shaft angle of 100 degress or less, the presence of marked gluteus medius limp and the vertical neck defect with progression of the varus deformity. Through a lateral approach, the lateral aspect of the proximal end to the femoral shaft was exposed and 2 Steinmann pins were inserted through the upper and lower levels to the proposed osteotomy. The distal pin was inserted perpendicular to the long axis of the femur and the proximal pin at an algle of desired correction to the distal pin and a subtrochanteric transverse osteotomy was completed. Two Steinmann pins were paralell to each other by adducting the proximal fragment and abducting the distal fragment. and the lateral cortex of the proximal fragment was impacted into the marrow cavity of the distal fragment. The Steinmann pins were then incorporated in the hip spica cast. The object of the operation was to correct the coxa vara, to change the stress on the vertical fissure in the neck from shear to compression and to overcome shortening of leg. With this technically simple method, the authors achieved adequate correction of the deformity. The ostetomies healed rapidly and the medullary canal was re-established in all cases, The gait of each patient was markedly improved and the Trendelenburg test become negative in all of them. One of these patients showed a mild recurrence of varus deformity and limp 2 years following surgery.

2.Brittain Ischiofemoral Arthrodesis

The indication of hip arthrodesis has tendency to decrease since the total hip replacement proved to be very effective, but the arthrodesis is inevitable for the infectious arthritis especially in young individuals to obtain painless stable joint. Authors reviewed 11 cases of Brittain ischiofemoral arthrodesis performed for the advanced hip tuberculosis during the years between 1972 to 1978 at Fatima hospital. These cases were 6 males and 5 females having ages from 5 to 31 with an average of 16 years old. The immobilization after surgery was rest having hip spica cast for the periods from 12 weeks to 17 weekes averaging 14 weeks and the crutch walking with gradual weight bearing as tolerated was recommended after discarding the cast for the cases who have united osteotomy, well maintained graft, and no evidence of the disease progession, even though the joint is not fused. Eventuaily good bony ankylosis as well as union of the osteotomy was found in 7 cases including a case who had fracture of the osteotomy after a minor trauma during the early days of walking without crutches which became united after having an another hip spica cast for 3 months. Three cases revealed the fibrous ankylosis with good union of the osteotomy but they had painless stable joints. In one of these fibrous ankylosis the tibial graft failed to be fixed in ischium and in the rest two cases bony ankylosis was expected later on because the last examinations were at 6 months and 7 months after surgery. The most unsatisfactory result was the case who discarded the cast at home and a non-union of the osteotomy was found at the first visit on five and a half years after surgery, when she refused further treatment of the non-union because there had been surprisingly little pain with usual daily activities. Brittain arthrodesis considered to be an effective procedure for the advanced unilateral hip tuberculosis to obtain the painless stable joint permitting early ambuiation.

3.Development Frequency of Penicillinase-producing Neisseria Gonorrhoeae (Jung-gu Community Health Center in Seoul, 1994-1998).

BACKGROUND: Gonorrhea is one of the most common venereal diseases in the world. Antibioticresistance development has been an issue with the penicillinase-producing Neisseria gonorrhoeae(PPNG) development. It is possible for PPNG to be resistant to other medications. Therefore, it is significant to determine its frequency rate and its disposition. OBJECTIVES: In this surveillance, we examined overall development frequency of PPNG from 1981 ti 1998. Comparing current five years' frequency rate with that of previous five year, we predicted possible development rate and described the procedures the physicians must take into account in gonorrhea treatment. METHODS: We examined the development frequency of PPNG in male patients of the venereal disease clinic of Jung-gu Community Health Center in Seoul from 1981 to 1998. Also, we analyzed the overall tendency with time series analysis using statistics package, SPSS 7.5.Grouping the years in five-year units, development frequency of PPNG of current five years (1994-1998) was compared with that of previous years. RESULTS: For the result of monthly analysis of frequency rate of PPNG development from 1981 to 1998, the rate tendency came out with a sligh rise (slope : 0.15). And for the result of comparison of grouped years, in current five years, its rate was 51.6%, marking a significant decline from that of the previous five years, which was 57.2% (p-value : <0.0001). CONCLUSION: The rate of current five years was reported to have declined from the previous years. However, the rate is still over 50%. Therefore, the government should continue the PPNG surveillance with test of sensitivity of the medications in use. Moreover, to eliminate gonorrhea, the education on high risk groups should continue and physicians should use a dose of adequate antibiotics in treatment.

4.Influence of Acute and Chronic Carbon Monoxide Poisoning on Reproductive Organs of the White Rats: Enzymological Study 2. Lactate Dehydrogenase Activity in the Prostate.

Influence of acute and chronic carbon monoxide poisoning on the rat prostate has been studied enzymologically and the following results were obtained: 1. An increasing tendency of the total lactate dehydrogenase activity in the prostate tissue was observed is the chronic poisoning groups. 2. The lactate dehydrogenase isoenzyme distribution pattern was not remarkably influenced by chronic poisoning. 3. The lactate dehydrogenase isoenzyme distribution pattern is the prostate tissue was as LDH5> LDH4> LDH3> LDH2> LDH1.

Influence of acute and chronic carbon monoxide poisoning on the rat testis has been studied enzymologically and the following results were obtained: Experimental animals were divided as 1) control, C1 (6 animals) 2) acute poisoning with 0.5% carbon monoxide until death, C2 (10 animals) 3) chronic poisoning with 0.05% carbon monoxide for 2 hours daily E1 (5 animals)...3 days/week for 8 weeks E2 (6 animals)...3 days/week for 4 weeks 4) acute poisoning until death with 0.5% carbon monoxide after chronic poisoning with 0.05% carbon monoxide E3 (5 animals)...chronic poisoning for 8 weeks plus acute poisoning E4 (5 animals)...chronic poisoning for 4 weeks plus acute poisoningUsing Neiland's method, the total activities and distribution patterns of lactic dehydrogenase in rat testis were measured. All of the data were analyzed statistically. 1. The total activities of lactic acid dehydrogenase in all experimental groups seemed to increase. In the group E3, the increase was statistically significant.(p<0.05) 2. No significant changes were observed in the lactic acid dehydrogenase isoenzyme distribution pattern of the testis. both is the control and experimental groups. 3. The lactic acid dehydrogenase distribution patterns were as fraction l>2>4>3>5 4. No significant histopathological changes were observed in the rat testis.

6.Experimental Study on Tissue Mast Cell Change of Ratskin Elicited by Mechanical Irritation Under General and Local Ansthesia.

Normal albino rats weighing 180-200 gm were used for a series of this investigation on change of skin mast cell under general and local anesthesia. At first, left side abdominal skin was prepared by hair cutting with scissors about 3cm square and eight irritations by mean of biting with tip of forceps mosquito were applied along the prepared skin margin. Skin specimens were submitted from central area which surrounded by irritated points and also from intact contra-lateral area in 15, 30, 60 and l20 minutes invervals. Submiitted skin specimens were fixed in 10% formalin solution, then were stained with 1% toluidine blue. The number of mast cell was counted under 450X of microscopic examination. CONCLUSION l. There were two stages in changes of skin tissue mast cell number on normal saline solution injected area and irritated area by mean of biting with tip of forceps without any types of anesthesia. i) Decreasing stage: Number of skin tissue mast cell was decreascd and reached to the lowest level at 30 minutes after each irritation. ii) Increasing stage: Number of skin tissue mast cell was increased to the highest level at 120 minutes after each irritation. 2. General ether anesthesia could not infIuence on the change of mast cell number after mechanical biting irritation. 3, Local anesthesia with 2% procaine hydrochloride solution revealed slightly suppressing effect to increasing number of mast cell at 60 and 120 minutes after mechanical irritation. Hnwever, it was not evidence at 15 and 30 minutes after mechanical irritation. 4. There was no significant change of rnast cell number in the intact contralateraI skin area within 120 minutes under gencral and local anesthesia. 5. The number of degranulated mast cells did not correlated with counted total number of mast cells.
