1.Comparison of Usefulness of Laboratory Diagnosis in Ancanthamoeba Keratitis.

Laboratory diagnostic methods, applied for the diagnosis of Acanthamoeba Keratitis, were evaluated for their usefulness in 16 patients of suspicious Acanthamoeba keratitis. Wet smear, Acridine orange(AO) stain, Gram stain and culture on nonnuturent agar plate were routinely used in all patients, and among them, and used saline of 7 contact lens not ideal for the corneal scraping specimens. AO and Gram stains were very useful in the identification of acanthamoeba, and culture on nonnutrient agar plates was essential to confirm this infection. Light and electron microscopic examinations were also useful in patients with negative results of ordinary diagnostic techniques. Suspicion of Acanthamoeba infection in patients that are recalcitrant to antibiotic treatment or related to contact lens wear, is the mont important step for the diagnosis of Acanthamoeba Keratitis. And also examination of corneal specimens by experienced observer is prerequisite for the accurate diagnosis of Acanthamoeba keratitis.

2.Contamination of Contact Lens or Contact Lens Storage Case in Contact Lens Related Infectious Keratitis.

In 44 out of 218 cases of contact lens related infectious keratitis from 19 hospitals throughout the country, contact lenses or contact lens storge cases were cultured. Microorganism was detected in 40 cases[90.9%]. Two or more organisms were isolated in 31 cases[77.5%]. Pseudomonas was the most common organism isolated from contact lens or contact lens storage medium[31 out of 84, 45.2%], followed by Serratia[15 out of 84, 17.9%], fungi [4], and acanthamoeba[4]. Acanthamoeba was found only in one hospital. Antibiotic sensitivity test for isolated pseudomonas showed that 96%of cases was sensitive to ciprofloxacin and 88%to ceftazidime.

3.Epidemiology of Contact Lens Related Infectious Keratitis(1995.4 ~1997.9): Multi-center Study.

Epidemiologic study of contact lens related infectious keratitis was performed in 19 hospitals which were located nation wide, Two hundred eighteen cases of contact lens related infectious keratitis(male 55 patients, female 163 patients) were diagnosed among 649 infectious keratitis patients between April 1995 and September 1997. Patients were mainly in twenties (54.6%) and residents of Seoul and Pusan(65.6%). Their major occupations were student(31.7%) and office worker(25.5%). Soft contact lens was involved in infectious corneal ulcer in 96.8% and disposable lens in 13.3%. Lenses were puchased at optical shop in most cases (72%). Unfortunately, 51.6% of patients was not aware of possible contact lens complication and 66.7% was not able to use contact lens properly due to improper education. Pseudomonas was isolated from corneal ulcer in 65.4% of cases and Serratia in 9%. This study reveals that contact lens related infectious keratitis can be devastation disease especially socially active age group, and proper education of both contact lens usage and proper lens dispense system should be needed.

Entrapment neuropathy of the suprascapular nerve is rare and frequently overlooked in the differential diagnosis of shoulder pain. There have been few published reports on a ganglion compressing the suprascapular nerve. We experienced a case of entrapment neuropathy of the suprascapular nerve by a ganglion that compressed the inferior branch of suprascapular nerve at the spinoglenoid notch.

5.A Case of ST-Segment Elevation in a Patient with Subarachnoid Hemorrhage.

There are several EKG changes in cerebrovascular disease(CVD). The wide prominent of inverted T wave is frequently developed in patients with CVD. There were case reports for CVD in patient with ST-segment elevation without myocardial infarction, but the exact mechanism is unknown. EKG abnormalities associated with subarachnoid hemorrhage were first described by Byer et al, in 1947.1 We report the case of 72 years old female patient who developed subarachnoid hemorrhage and intraventricular hemorrhage with ST-segment elevation.

6.The Patients With Headache in Emergency Department.

BACKGROUND: Headache is a common complaint in patients presenting to the emergency department(ED). Many patients suffered sustained headache even after presenting to the ED. This study investigate whether the patients were received analgesics and timely adequate. METHOD: We analyzed 131 adolescent and adult patients with non-traumatic headache without any neurologic deficit in two emergency department during 6 months period retrospectively. RESULTS: The positive and negative predictive value according to primary impression were 47% and 74% respectively. The mean time to take brain computed tomography(CT) after ED presentation was 120 minutes. The patients with primary impression of intracranial hemorrhage(mainly SAH) were the most fast(mean time 88+/- 70 minutes). 28(21%) patients were received analgesics before taking brain CT(after ED presentation), and 70(53%) patients after CT, 27(54%) patients after cerebrospinal fluid examination. The mean time interval were 43, 126 and 149 minutes on each group being received the analgesics. CONCLUSION: According to this study many patients suffered sustained headache before being classified to certain type of disorder. It may due to the reasons that there were no definite guidelines treating headache, the pain itself, or the physician treats patients as physician's way.

7.Acute dapsone intoxication: The dosage of activated charcoal and methylene blue.

BACKGROUND: Methemoglobinemia(MetHb) induced by dapsone overdose is not uncommon in Korea, especially in rural area. For treatment of dapsone-induced methemoglobinemia, methylene blue(MB) and activated charcoal(AC) should be used. To date, no reports have compared the amount of MB used between MB alone with MB & AC combined group(MB+AC). And also between moderate (MetHb<35%) and severe (MetHb>35%) intoxicated group defined by initial MetHb level. Authors hypothesized that less amount of MB can be used if MB and AC was used together and larger amount of MB is necessary to reduce MetHb level to asymptomatic level in severely intoxicated group. METHODS: From Jan 1990 to Dec 1996, a total of 54 patients who received treatment for dapsone intoxication were subject of study, The study was done retrospective chart analysis for initial MetHb level, total amount of MB and AC. Wilcoxon rank sum test and Chi-sqiare test was used to compare the total dosage of MB used for each group. Linear regression analysis was used between initial MetHb and the total amount of MB. Results were considered statistically significant when p<0.05. RESULTS: For MB alone and MB+AC group, the differences in total amount of MB used were statistically significant with mean dosage of 7.14+/-1.1mg/kg and 4.28+/-0.7mg/kg, respectively. And total amount of MB used between moderate and severe intoxicated group, the differences were statistically significant with mean dosage of 5.16+/-1.1mg/kg vs. 10.98+/-1.9mg/kg, respectively. There was significant correlation between initial MetHb level (X) and the amount of methylene blue (Y), Y=0.3X-2.42 (r2=0.41, p=0.0001) in MB alone, Y=0.186X-1.95(r2=0.21, p=0.034) in MB+AC respectively. CONCLUSION: For methemoglobinemia induced by dapsone, total amount of MB can be reduced especially in severe Intoxicated group if AC use was combined in treatment modality. There was significant correlationship between initial MetHb level and total amount of MB used.

8.Cellulitis in Young Adults.

While the microbiology of cellulitis in the pediatric population and systemic diseases(DM, Cirrhosis, etc) is well known,the causative bacteria and clinical aspect of cellulitis in young adults are less descrihed. this study was undertaken to identify the causative organisms and clinical aspect of adult cellulits. We analysed 121 medical records of 115 young adult patients with cellulitis who had been admitted at the department of orthopaedic surgery, National Police hospital from Jan. 1994. to Dec. 1996. The results were as follows. 1. The age of patient was between 21 yrs and 25 yrs, and the sex of all patients was male. 2. The lower leg and foot were the most frequently involved site of cellulitis as 92 cases(74.2%). 3. The potals were suspected in 65 cases(56.5%),of them, traumas were detected in 43 cases(66.1%), non-traumatic skin lesions in 23 cases(33.9%). 4. In most cases, initial systemic symptoms (fever, chill, headache, etc.) and local symptoms (local heating, tenderness, etc.) were found. 5. The laboratory findings revealed leukocytosis in 19%, elevated ESR in 94%, positive CRP in 80%. 6. Microorganism were detected in 16 of 38 pus cultures. Gram positive microorganisms were detected in 15 of 16 cases culture positive. 7. 6.7% of patients with cellulitis experienced recurrence in same lesions. In conclusion, the retrospective analysis of 115 young adult patients with cellulitis showed that primary cause of young adult cellulit was circulatory disturbance and lymphatic obstruction of the lower extremities most often involved by trauma. therefore, supportive treatments such as rest,elevation,ice packs are considered prior to antibiotic treatment.antibiotic treatment aimed at gram-positive cocci appears to be sufficient.

9.Chondroblastoma of the patella: a case report.

Tumor and tumor-like lesion of the patella are extremely rare. The following have been reported: giant cell tumor; osteoblastoma; osteoid osteoma; chondroblastoma; solitary osteochon droma; chondroma; ganglion; simple bone cyst; aneurysmal bone cyst; hyperparathyrodism (brown tumor); malignant lymphoma; haemangioendothelioma; haemangima; primary osteosarcoma; plasmocytoma and metastases. Chodroblastoma in patella were very rare and first repoted by Jerone Cohren in 1963. On review of our literatures, we could not find reported case in Korea. The purpose of this paper is to present an unusual case of chondroblastoma of the patella.

10.Ulnar Lengthening in the Treatment of Kienbock's Disease

Ulnar lengthening, one of the operative treatment methods for Kienbock's disease, is based on the theory that ulnar minus variance is a significant causative factor of Kienbock's disease. It is considered that this method can promote the revascularization of the devascularizied lunate by reducing the forces concentrated at the wrist joint and that over-all carpal collapse is halted or reversed when the ulnar part of the carpus is supported better. 4 patients with Kienbock's disease treated by ulnar lengthening with iliac bone graft were reviewed after follow-up ranging from12 months to 30months. In all of 4 patients, relief of pain was satisfactory and there was no remarkable complication.
