1.Candida albicans test for the screening of phototoxicity in anthistamines.

BACKGROUND: Antihistamine drugs are used widely in many conditions. Although some antihistamines may cause a photosensitive reaction,many physicians are not awae of it. OBJECTIVE: For examination of the phototoxic potential of antihistamines, we performed the Candida albiecrns test which is simple, cheap, and good for the screening of many drugs. MEHTODS: Thirty microliters of each solute of various antihistamines were applied to the Sabraud dextrose agar plate in which Candida albicans were applied diffusly. Four hours after the application, 60J/cm fo UVA was irradiated for two days. The irradiated. plates and nonirradiated control ones were incubated in a dark room for 48 hours, and examined for lear zones arround the drug, which means a positive results for the phototoxic potential of the drugs. RESULTS: Mequitazine, thiethylperazine, perphenazine and cllorromazine showed positive results, whereas others did not. An additional Candida albicans test using 0.1%, 0.01%, and 0.001% of the positive drugs revealed tht chlorpromazine, thiethylperazine aderphenazine showed positive results at 0.1%, but negative at 0.01 and 0.001%. Mequitazine was niegative at 0.1, 0.01, and 0,001%, Additional studies of the Candida albicans test using 5% and 10% of the diphenhydramine and dimenhydrinate, those were known photosensitizers but they slowed negative results at this study and revealed very weak posit,ive result in 10% diphenhydramine. CONCLUSION: A photosensitive reaction such as photoallergy and persistent light react,ion may be triggered by the phenothiazine antihistamines. Negative result in 1%, and very weak positive results in 10% diphenhydramine may be due to different mechanism of phototoxicity, or the low phototoxic potential of diphenhydrainine.

2.Intra-arterial Cisplatin Chemotherapy in Uterine Cervix Cancer after Selective Internal lilac Arteriography.

PURPOSE: Colposcopic response of preoperative uterine cervix carcinomas to intra-arterial cisplatin infusion chemotherapy was analyzed to evaluate the efficacy of this therapy after selective pelvic arteriography. MATERIALS AND METHODS: Nine patients of stage 1/11 cervical carcinomas were treated twice with intra-arterial cisplatin chemotherapy of a 3-week interval, with every week follow-up of colposcopy and histology until the radical hysterectomy and pelvic lymphadenectomy on the 6th week. Cisplatin of 50mg/m~ was slowly infused via autoinjector for 10 minutes with the catheter tip placed in both internal iliac arteries around uterine artery branching. RESULTS: Colposcopic regression rate of tumor size was as follows:more than 2/3 decrease of original size in 2 patients, 2/3-1/2 in 2 patients, less than 1/2 in 4 patients, but in one patient minimal increase in tumor size was recognized. Colposcopic and postoperative microscopic pathology revealed degeneration and necrosis of the tumor cells and/or adjacent lymph nodes and fibrosis of surrounding tissues. All patients have been doing well until postoperative 25 months without recurrence or significant complications. CONCLUSION: Preoperative intra-arterial cisplatin chemotherapy after selective internal iliac rteriography is helpful to decrease tumor size and improve histologic response and prognosis in stage I and II cervical carcinomas.

4.A case of chlorpropamide-induced photosensitivity.

We report a case of a photosensitivity reaction in a 76-year-old inale induced by chlorpropamide ingestion. The patient had erythematous scaly patches on the sur. -exposed areas, A phototest revealed the decreased minimal erythemal dose(MED) to UVA(5J/cm). A photopatch test and photo-scartch test with 1% chlorpropamide ointment and 0.1% chlorpropamic!e .olution were all negative. An oral provocation test was performed, which showed a positive result with marked decrease of MED to UVA (5J/cm). After the cessation of chlorpropamide, his skin lesions were improved markedly with complete loss of photosensitivity. Macular hypopigmentations (leukcme.anoderma) appeared on the previous erythematous patchy ereas, but disappeared during the follow-up period.

6.Partial Unilateral Lentiginosis.

Partial Unilateral Lentiginosis (PUL) is a rare pigmentary disorder characterized by the numerous lentigines confined to a body segment, with a sharp demarcation at the midline. We report two cases of PUL. A 38-year-old woman had asymptomatic discrete small hyperpigmented macules that were scattered on the T7~L1 dermatomes on the left side of her trunk and were clearly demarcated in the midline on both anterior and posterior sides. In the other case, an 18- year-old woman had hyperpigmented macules that were scattered on the left neck, shoulder, and anterior chest. Other anomalies including neurofibromatosis, neurologic anomalies, and multiple lentiginous syndrome were not related in both cases. We report two cases of PUL having no other anomalies.

7.Three Cases of Chronic Actinic Dermatitis Treated with Systemic PUVA Therapy.

Chronic actinic dermatitis is an uncommon disabling ecxematous photosensitive eruption. The skin lesion is characterized by persistent eczematous eruptions on sun exposed skin with possible extension into nonexposed areas. The phototest shows decreased minimal erythema doses (MED) to UVB and possibly UVA. Histological features demonstrate chronic eczema with or without the presence of lymphoma like changes. The present cases showed pruritic erythematous patches and plaques on sun exposed areas and showed decreased minimal erythema doses to both UVB and UVA. We herein report three cases of chronic actinic dermatitis treated with systemic PUVA therapy.

8.A Case of Keratosis Punctata Palmaris.

We report a rare case of sporadic keratosis punctata palmaris et plantaris in a 49-year-old woman. Thirty years ago, firm semitranslucent papules developed on the middle portion of her palms without any subjective symptom. The lesions had flat-topped smooth surfaces. They were concentrated along the margins of the hands, especially on the lateral surfaces of both index fingers and finger tips/ Ten years ago, similar hyperkeratotic papules with or without central craters developed on the weight bearing portions of her soles. Her family and past histories were negative. The biopsy specimen revealed a dense hyperderatotic stratum corneum which fitted into a cup shaped depression in the stratum malpighii and a turning inward of the rete ridges at the margin. The Masson trichrome and elastic strains showed no peicific dermal changes.

10.Evaluation of the Phototoxic Potential of Some Quinolone Antibiotics.

BACKGROUND: The photsensitizing effect of quinolones has been recognized since their introdulation as an antibacterial agents. Recently several new second eneration antibacterial agents of this pharmacological class have become available for therapy, and are gaining increasing impotance. OBJECTIVE: To reveal the phototoxic potentials of some new quinolones by photohemolysis test, estimation of fluorescenc spectra, and Candida albicans test. METHODS: Nalidixic acid and four second-generation quinolones(ciprofloxacin, enoxacini, norfloxacin, and ofloxacitid were examined by fluorescence spertra which measured t.he phototoxc potentials by photochemial instability, photohemolsis test for the phototoxic properties against cell membranes and Candida tlbicans test for phototoxic properties against DNA. RESULTS: All drugs showed a fluorescence spectra within 360 nm to 450 nm, and in the photohemolysis test, all studied drug except ofloxacin got above 5% hemolytic value, and all drugs showed clear zone. in Candida albicans test after 48hours. CONCLUSION: These results suggested that all tested drugs were photochemically unstable. According to the mechanisris of cellular phototoxicity, ciprofloxacin, enoxacin, and norfloxacin was phtototoxic to nucleus and cell membrane, whereas ofloxacin was phototoxic to nucleus only.
