1.The Analysis of Risk Factors and Significance of Tumor Necrosis Factor(TNF)-a in Henoch-Schonlein Nephritis.

A mediastinal mass was aspirated by fluoroscope-guided fine needle aspiration biopsy in a 47 years old female patient. The first aspiration smears were not diagnostic, because of hemorrhagic background and cell paucity. On the second aspiration, the smears were composed of some clusters of benign epithelial cells in hemorrhagic background. Cells were arranged in mostly solid sheets and tended to form glandular lumina in part. Their nuclei were round and vesicular. Nucleoli were not prominent. These findings were suggestive of benign glandular tissue, which was finally confirmed as mediastinal thyroid gland by open thoracotomy specimen.

2.A Case of Traumatic Epidermoid Cyst.

Traumatic epidermoid cysts are rare tumors occuring almost exclusively on the palms, palmar aspects of fingers, and soles. These cysts are thought to originate from a bit of epidermis implanted into the dermis after a penetrating or blunt wound. We report a case of typical traumatic epidermoid cyst on the sole of the right foot of a 23-year-old male student. The lesion was a coin-sized, tender, fluctuating cystic mass occuring 3 years after a penetrating wound by an iron nail. Histopathlogical examination revealed a deep dermal cyst lined by a thick, flattened, keratinizing wall with partial disruption accompanied by foreign-body reaction.

4.Effects of puromycin aminonucleoside on the cytoskeletal changes of glomerular epithelial cells.

PURPOSE:This study was designed to clarify the mechanism of proteinuria in nephrotic syndrome patients by using puromycin aminonucleoside (PAN) nephrosis model. METHODS:Following administration of various concentrations of PAN and antioxidants we observed the changes of podocyte cytoskeletons in cultured rat glomerular epithelial cells (GEpC) by method of scanning electron microscope, reactive oxyten species (ROS) analysis, permeability assay, confocal microscope, and Western blot assay. RESULTS:PAN not only induced the ultrastructural changes of GEpC, such as shortening and fusion of microvilli, but also separated the intercellular gaps and linear ZO-1. PAN induced oxidative stresses in time and dose dependent manners and increases of intercellular permeability in anti-oxidants inhibitable manners. High concentration of PAN induced not only actin polymerization and disorganization, but also the conglomerulation and internal dislocation of alpha-actinin protein. The intensities of fluorescences of ZO-1 protein were diminished and internalized by PAN in a dose-dependent manner, which were inhibited by anti anti-oxidants. CONCLUSION:PAN induced the changes of podocytes cytoskeleton and junctional barriers by way of increasing ROS in GEpC that resulted in increasing their permeability in a antioxidatn-inhibitable manner. Glomerular hyperpermeability induced by PAN mediateing through oxidative stresses is thought to take part in the mechanism of proteinuria in nephrotic syndrome. (Korean J Pediatr 2008;51:54-61)

5.A Case of Reiter' s Syndrome Combined with Lung Cancer.

Reiters syndrome is an unusual disease characterized a triad of nongonococcal urethritis, conjunctivitis, and arthritis in association with the mircoutaneous lesions of keratoderma blenorrhagica and balarintis circinata. We present herein a case of Reiters syndome combined yiti lung cancer. A 39-year-old man has experienced naigrating polyarthralgia and high fever ilitermittently for about 20 years, Hyperkeratotic erythemnous patches and plaques, which dyeliped about 1 year ago, have aggravated and expanded to the whole body. Histopathologic sections from the plaque on the right forc ari showed characteristic findings including thickened parakeratotic horny layer and spongiform micropustules of Kogoji He was associated with HLA-B27. On the chest X-ray, a thumbtip size mass was incidently found. Chest CT and bone scan findings supported lung cancer and multiele metastases.

6.Clinical Studies of Risperidone in Korea.

There have been 91 papers on risperidone in Korea, which were composed of 21 reviews, 9 preclinical studies, 18 case reports, and 43 clinical trials, since this new antipsychotic was introduced. Case reports focused mainly on adverse effects of risperidone, and clinical trials on it's efficacy and safety. Review of Korean clinical trials revealed the paucity of well designed controlled studies which examined the effective dose and compared the efficacy of risperidone with those of other drugs. Future research should adopt more reliable and standardized methods. The efficacy of risperidone for various clinical populations needs to be elucidated. Researchers need to make an effort to implicate the clinical use of risperidone. Long-term outcome studies are expected.

7.Factors Influencing Life Satisfaction for Mothers of Elementary School Children.

PURPOSE: The purpose of this study was to identify factors influencing life satisfaction for mothers with children in elementary school. METHODS: Participants were 243 mothers with a child in first and fifth degree of one elementary school in G city. Data were collected from March 20 to April 9, 2010. The instruments used were Satisfaction with Life Scale, Parent Satisfaction Scale, Korean ADHD Rating Scale, Parenting Alliance Inventory and Scale for Measuring Family Strengths. The data were analyzed using SPSS/WIN 14.0 computer program and included one-way ANOVA, Scheffe back-testing, Pearson's correlation coefficients and stepwise multiple regression. RESULTS: The life satisfaction of the mothers showed significantly positive correlations with parent role satisfaction, spousal support, and family strengths and negative correlations with ADHD. Significant predictors of life satisfaction for mothers with children in elementary school were spousal support and sharing of value systems, and these variables explained 37.0% of the variance in life satisfaction. CONCLUSION: The results of this study indicate that improvement of spousal support and sharing of the family value system should be helpful in promoting life satisfaction for mothers with children in elementary school.

8.A Case of Eccrine Angiomatous Hamartoma.

Eccrine angiomatous hamartoma is a distinct developmental anomaly showing, histologically, hyperplasia of eccrine sweat apparatus and of vascular elementa in the same lesion, and clinically, the angiomatous lesions with occasional tenderness and profuse local sweating. Recently, we examined an 18-year-old, otherwise healthy girl having, on the anterior aspect of the right leg, an extensive, irregularly marginated and somewhat bluish, tender, firm subcutaneous mass. The mass was studded with several overlying macular, verrucous or nodular angiomatous patches which showed conspicuous hyperhidrosie. Histological examination revealed marked hyperplasia, hypertrophy and cystic dilatations of eccrine sweat apparatus accompanied by proliferation of the capillary hemangiomatous component and ectopic, thick smooth muscle bundles in the deep dermis and subcutaneous tissue.

9.Primary Subacute Pyogenic Osteomyelitis of Long Bones

Primary subacute pyogenic osteomyelitis is defined as a bone infection of insidious onset lasting for more than several weeks without any acute systemic toxic reactions and is known to have the problem of diagnosis because there is no inflammatory symptoms or signs and X-ray findings are similar to various bone tumors. Authors reviewed 29 cases of primary subacute pyogenic osteomyelitis of long bones admitted at Fatima hospital during the years between 1976 and 1984. The osteomyelitis of infants and the osteomyelitis modified by antibiotics were excluded. The diagnosis of the cases was confirmed by bacteriological examination and/or tissue examination. The results were as follows. 1. The cases were 18 males and 11 females and 19 adults and 10 children. Male predominence was noted in children. 2. The involved bones were 11 tibias, 10 femurs and all other long bones. The involved sites were 18 metaphysis and 11 shafts. 3. All of the cases has insidious onset of local pain without any acute systemic symptoms and most of the cases has local tenderness but local deep swelling noted in about half of the cases. 4. There were many cases with elevated ESR and a few cases of slight leukocytosis. 5. The confirmed infecting organism was all staphylococcus in 16 cases of the cultures from the 25 lesions. 6. The X-ray findings were Brodies abscess in 10, illdefined cavity in 3, diffuse bones absorption in 5, diaphyseal lesion of adult with localized cortical sclerosis in 5 and with localized medullary abscess in 3 and diaphyseal lesion of children with localized medullary abscess and periosteal reaction in 3 cases. 7. All of the cases cured rapidly with rare recurrence after treatment by simple local excision in 4 and curettage in 22 cases combined with antibiotic therapy and local immobilization.

10.Macular Microvasculature Features of the Eyes of Healthy Korean Children Using Optical Coherence Tomography Angiography

Purpose:
We used optical coherence tomography angiography to analyze the macular microvasculature of normal eyes of healthy Korean children; we derived normative data stratified by age.
Methods:
We retrospectively analyzed the medical records of 68 children (126 eyes) aged from 3 to 15 years. Optical coherence tomography angiography was performed on 3-mm2 regions centered on the fovea. The vascular densities of the superficial and deep capillary plexuses and the foveal avascular zone were determined, and analyzed by age. We sought correlations between age, gender, axial length, and the spherical equivalent.
Results:
The foveal, superficial capillary plexus vessel densities were 17.41 ± 5.41, 20.53 ± 4.76, 21.78 ± 5.73, and 22.95 ± 3.91% in children aged 3-5, 6-8, 9-11, and 12-15 years, respectively. The vessel densities of the foveal deep capillary plexus were 30.20 ± 6.59, 34.54 ± 7.16, 35.81 ± 6.02, and 37.38 ± 5.16% in those aged 3-5, 6-8, 9-11, and 12-15 years, respectively. Significant differences by age were apparent (p = 0.002, p < 0.001, respectively). The foveal vessel densities of the superficial and deep capillary plexuses were positively correlated with axial length (R2 = 0.153, p < 0.001; R2 = 0.158, p < 0.001, respectively) and the foveal avascular zone area was negatively correlated with axial length (R2 = 0.085, p < 0.001).
Conclusions
The vessel densities of the superficial and deep capillary plexuses measured via optical coherence tomography angiography in normal eyes of healthy Korean children differed by age, and correlated significantly with axial length.
